- Description
- (i) This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is FA6606-22-R-0005 and is being issued as a request for proposals (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-05 and Defense Federal Acquisition Regulation Supplement 2022-05355, and Air Force Federal Acquisition Circular 2021-0726. (iv) There are no set-asides for this acquisition and it will be advertised under full and open unrestricted competition regulations IAW FAR 6.1. The associated NAICS code is 621111, Offices of Physicians (except Mental Health Specialists). (v) The following are the line item numbers, items, quantities, and units of measure: *Please utilize the attachment titled �01-Bid Schedule�* 0001 � FIREFIGHTER PHYSICALS, 75 EACH 0002 � FOLLOW-UP EXAMS, 1 JOB (Not a biddable item) (vi) Description of requirements: *See attachment titled �02-PWS for Firefighter Physicals� for full explanation of requirements* The government anticipates one single award Firm Fixed Price contract. This requirement stems from the need for Pre-employment, annual, termination, and fitness-for-duty examinations for current and potential Firefighters/EM/Bio employees at Westover ARB. These examinations should include a questionnaire that captures employee baseline information on medical status and history, an annual physical examination, weight and body composition, annual blood tests, urine laboratory tests, Pulmonary function testing (spirometry), Electrocardiograms (ECGs), sleep disturbance screening, cancer and cardiovascular disease risk reduction, hormone imbalance awareness consultation, behavioral health screening, and cancer screening (colon, prostate, lung, cervical, bladder, oral, thyroid, and skin). The contractor shall furnish all personnel, equipment, materials, and facilities necessary to perform examinations as required by NFPA Standard 1582, OPM standards, DOD and AF regulations. �� � (vii) Estimated dates of service are 16May22 thru 15May23. Delivery and acceptance will occur at the examination location. (viii) Provision 52.212-1, Instructions to Offerors-Commercial Products and Commercial Services, applies to this acquisition. *See attachment titled �03-Provisions and Clauses� regarding any addenda to the provision.* (ix) Provision at 52.212-2, Evaluation-Commercial Products and Commercial Services applies to this acquisition. *See attachment titled �03-Provisions and Clauses� for the specific evaluation criteria.* (x) Offerors shall complete the provisions�52.204-24 Representation Regarding Certain Telecommunications and Video Surveillance Services or Equipment,�52.204-26 Covered Telecommunications Equipment or Services-Representation, 52.212-3, Offeror Representations and Certifications-Commercial Products and Commercial Services, with their offer. *See attachment titled �04-Provisions and Clauses_Fill In� to complete the required provision.* (xi) Clause 52.212-4, Contract Terms and Conditions-Commercial Products and Commercial Services, applies to this acquisition. *See attachment titled �03-Provisions and Clauses� regarding any addenda to the clause.* (xii) Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Products and Commercial Services, applies to this acquisition. �*See attachment titled �03-Provisions and Clauses� to review additional cited FAR clauses.* (xiii) Any standard warranties for services rendered for this acquisition shall be consistent with customary commercial practices. (xiv) Defense Priorities and Allocations System (DPAS) and assigned ratings are not applicable to this requirement. (xv) All proposals are due no later than, 2:00 pm, Eastern Standard Time on 06MAY2022. Proposals MUST be emailed to 439confpk@us.af.mil . Any proposals not sent to this email by the stated date and time will not be considered for award.� (xvi) Requests for Information (RFI) concerning this requirement shall be sent to 439confpk@us.af.mil . Ensure to reference FA6606-22-R-0005, Firefighter Physicals in the subject line to ensure it is forwarded to the appropriate personnel. Answers to all RFIs will be posted with the solicitation on SAM.gov. Any RFIs must be submitted to the email address above no later than 2:00 pm, Eastern Standard Time on 03MAY2022.
