Notice of Intent to Award Sole Source Project Title: Delivery of The Phillips X3 Hemodynamic System to the Corporal Michael J Cresenz VA Medical Center, 3900 Woodland Ave, Philadelphia, PA 19104 Announcement No: 36C24422Q0677 The Corporal Michael J Cresenz VA Medical Center intends to award a sole-source contract under the authority of FAR 6.302-1 to Four Points Technology, LLC 14900 Conference Center Dr. Ste 100, Chantilly, VA 20151. This firm-fixed-price delivery order will be awarded against the Governmentwide Acquisition Contract NASA SEWP NNG15SD22B for a commercial-of-the-shelf(COTS) for the Philips Xper, which is a hemodynamic / information management system that supports all phases of procedural documentation and invasive monitoring necessary for Cardiac Catheterization procedures that meet procedural needs as well as VHA/JC standards for procedural documentation. The period of performance for this contract is anticipated to be a 12month period. The total estimated value is less than $400,000.00 This sole-source contracting action is pursuing a waiver from placing an order on a competitive basis pursuant to FAR 8.405-2 for the requirement mentioned above based on the application of FAR 8.405- 6(a)(1)(i)(B). This hereby constitutes a synopsis of the options included in this requirement. Responses to this notice must demonstrate clear and convincing evidence that competition would be advantageous to the Government, would not be cost or time prohibitive, and would not hinder the mission objective of the national solid waste recycling measurement initiative. The determination not to compete in this proposed contract action is solely within the discretion of the Government. Information received will generally be considered to determine whether to conduct a competitive procurement. The Government will not be responsible for any costs incurred by responding to this notice. This is NOT a request for quotes or proposals. STATEMENT OF WORK (SOW) MEDICINE SERVICE/ CARDIOLOGY DIVISON DEPARTMENT OF VETERANS AFFAIRS CORPORAL MICHAEL J CRESCENZ VA MEDICAL CENTER 3900 WOODLAND AVE, PHILADELPHIA, PA 19104 GENERAL DESCRIPTION: The Cardiac Catheterization Laboratory of Michael J Crescenz Medical Center is under the Cardiology Division of the Medicine Service line, directed by Dr. Andrew Epstein. The Cardiac Interventional Cardiology Physicians are licensed and credentialed to provide care to patients undergoing cardiac catheterization and cardiac interventional procedures. The equipment specific to this request is called the X3 Hemodynamic System by Philips Healthcare. The Philips X3 Hemodynamic System is used for diagnostic and therapeutic cardiac catheterization procedures for diagnosing and treating heart disease. The system, when connected to the patient, provides continuous monitoring, hemodynamic recording using multiple, configurable pressure channels and is used to record and report all phases of the cardiac procedure. This system enables the end users to calculate pressures inside the heart and lungs, calculate heart valve area and cardiac output. It also serves as a clinical documentation system that allows the nurses and technologist to document all phases of the procedure, equipment used, vital signs, etc. This upgrade will enhance the operator s ability to diagnose various cardiac conditions, provide time-stamped clinical documentation and provide a full procedural record for that transfers to the patient chart when completed. SUPPLIES/SERVICES The Philips X3 Hemodynamic System is for use in the cardiac catheterization laboratory during diagnostic and therapeutic procedures. The Philips XIM Hemodynamic System is currently used in the Invasive Cardiac Labs. This request is for an upgrade to the latest software and hardware refresh and to bring the system to a Microsoft Supported Operating System and current technology. Crescenz Veterans Affairs Medical Center (CMCVAMC) is looking to upgrade our existing hemodynamic monitoring/recording system to a Microsoft Security supported platform, with software and hardware enhancements to include the latest technology. The purchase must include end-user training for cardiology staff. All hardware, software components must be compatible with existing Philips Monitoring Systems used throughout the hospital and the existing imaging system, Siemens ARTIS Q bi-plane. Delivery Location CMCVAMC Philadelphia 3900 Woodland Avenue Philadelphia PA, 19104 Warranty and Support Warranty of the equipment must be provided for a minimum of one year upon receipt, acceptance of equipment. The warranty must cover all hardware and software components of the system. Warranty and support should include a 4-hour call back response during normal business hours of 7:30AM- 4:30 PM Monday thru Friday (except holidays). Any service reports sent to all appropriate points of contact. The vendor shall provide and install all manufacturer recommended software upgrades and changes included in the cost of the system during the warranty/contract period. The vendor shall provide two (2) sets of user manuals and technical manuals to VA Biomedical Engineering and site end user. Repairs or services that are performed during normal working hours should be covered by warranty/contract. Installation All work and installation shall be coordinated with the department Nurse Manager, site Biomedical Engineering team and the Contracting Officer Representative (COR) and a phasing and work-schedule shall be provided. All work is to be executed as to not interfere or interfere as little as possible with normal functions of the Medical Center and department. The contractor shall keep work, storage and staging areas clean and neat. Testing The contractor shall verify to Biomedical Engineering and Cardiology Staff that the system meets all requirements through demonstration and validation. The contractor shall have no access to VA sensitive information during testing. Training The contractor shall be responsible for providing on-site user training for the equipment. Each session of end-user training is to be scheduled at the CMC VAMC discretion and training shall have a one-year window. Work Hours Normal business hours are 7:30 AM to 4:30 PM Monday thru Friday excluding Federal Holidays. Work completed outside this time must be requested through Facility Biomedical Engineering.
