- This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. Project Title: Doctor Stools with swing arms for the Southern Arizona Healthcare System Description: Southern Arizona VA Healthcare System is seeking to purchase 50 quantities of a Doctor Stools with swing arms. Project Location: Southern Arizona VA Healthcare System, 3601 S 6th Avenue, Tucson, Arizona 85723 Solicitation Type - Number: Request for Quotes (RFQ) - 36C26222Q0747. Set Aside: This solicitation is issued as a Service-Disabled Veteran owned Business set aside. Applicable NAICS code: 337127, Institutional Furniture Manufacturing Small Business Size Standard: 500 employees Type of Contract: Firm Fixed Price Period of Performance: 60 days Delivery Date from award date. The Veteran s Integrated Service Network 22 is seeking a contractor to provide doctor stools for the Southern Arizona VA Healthcare System. GENERAL SCOPE The Veterans Health Administration (VHA) has a requirement for healthcare furniture for various areas of the hospitals in the Southern Arizona VA Healthcare System. The contractor shall provide all labor, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity. PRODUCT General All finished surfaces shall be free of scratches, mars, dents, or blemishes, and withstand staining, and exhibit to flaking, cracking, or loss or adhesion. Furnishings shall have smooth finishes with no hazardous projections, sharp corners, or detail, which can be hazardous and cause personal injury or damage to clothing. The Stool must have the following salient characteristics: a. Round seat with 3-way wrap around arm stool. b. 5-star base c. Casters appropriate for hard floor d. Seat Height: 18 - 25 , Seat Diameter: 15-18 , Seat Cushion Thickness: 2-3 , Caster Base Diameter: 23 . All Measurements +/- 1 e. Weight Capacity a minimum of 300lbs. f. Sealed seat pan g. 360 degree swivel h. Vinyl or equal color to be neutral tan; must be cleanable with Cavi wipes without degrading the vinyl. Adjustments: Hydraulic lift seat with access from all points of seat diameter. Warranty: 24/7 Continuous Shift, 10 year part replacement with 5 year cylinder guarantee Basis of Design: Reliance Exam Stools with Swing Arm, Model 5356. Open specification, alternates accepted.
