- VHAPG Part 813.5 Simplified Acquisition Procedures for Certain Commercial Items Attachment 1: Request for Sole Source Justification Template >SAT-$7.5M under FAR13.5 Last Updated: 02/01/22 DEPARTMENT OF VETERANS AFFAIRS SOLE SOURCE JUSTIFICATION UNDER SIMPLIFIED PROCEDURES FOR CERTAIN COMMERCIAL ITEMS IN ACCORDANCE WITH FAR 13.5 Acquisition Plan Action ID: 36C248-22-AP-2104 1. Contracting Activity: Department of Veterans Affairs, VISN 08, James A. Haley Veterans Hospital (JAHVH), Dental Department, 13000 Bruce B. Downs, Blvd. Tampa, Fl. 33612. Reference 2237# 673-22-2-5414-0008. 2. Nature and/or Description of the Action Being Processed: This is a one-time request brand name only request for Other Than Full and Open Competition to procure Dental Delivery Chairs and Delivery Systems, manufactured by A-Dec Inc, a large business. The equipment will provide routine and emergent dental treatments for Veteran Community Patients at the New Port Richey CBOC. 3. Description of Supplies/Services Required to Meet the Agency s Needs: New Port Richey Dental Department currently utilizes Dental Chairs and Delivery Systems manufactured by A-Dec Inc. The equipment will be used for dental cleanings and routine and dental emergency procedures completed by the NPR CBOC and local providers. This request is to outfit the new New Port Richey CBOC Consolidation. This system will allow the Dental team to have continuity of care for the patient community while not requiring additional cost and training for the dental team staff. These are the only dental chairs and delivery systems with the setup and configuration in the market that meet the government s needs. 4. Statutory Authority Permitting Restricted Competition: FAR 13.5 Simplified Procedures for Certain Commercial Items. The statutory authority for applying the Simplified Procedures for Commercial Items of FAR 13.5 is 41 U.S.C. § 1901 and is implemented by FAR 13.106-1(b)(2) for restricting competition on this procurement. Competition is restricted on this procurement for the reason below: ( X ) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements ( ) Unusual and Compelling Urgency ( ) Industrial Mobilization, Engineering, Developmental or Research Capability or Expert Services ( ) International Agreement ( ) Authorized or Required by Statute ( ) National Security ( ) Public Interest 5. Demonstration that the Contractor s Unique Qualifications or Nature of the Acquisition Requires the Use of the Authority Cited Above (applicability of authority): A-Dec dental chairs are being used throughout New port Richey Clinic. Additional Dental Chairs and Delivery Systems manufactured by A-Dec Inc., are required to support the need of patients being treated at other manufacturer does not offer this system and chair configuration. The providers already utilize this brand in the current facility and surrounding facilities. Providing ADEC in the new facility ensures that the providers can seamlessly move from one patient care facility to another without learning a new system. Biomed on site is trained to maintain the ADEC brand, drastically reducing department and patient care downtime. The New Port Richey CBOC clinics have been designed and built around the configuration of the ADec system, which has a unique utility setup. Award to any other brand would increase the project's scope, time, and cost. ADec dental system is the only dental system that could meet the government's needs without the Veterans Health Administration experiencing substantial duplication of the cost that could not be expected to be recovered through competition and can cause an unduly delay in patient support. A-Dec, Dental System Unique Features: DELIVERY SYSTEM: Integrated four position handpiece control block is capable of operating up to four active handpiece positions Each handpiece position has an individual drive air and water adjustment Removable water cartridge valves A single sliding locking tab secures the handpiece tubing to the control block requiring no tools and allows easy access to connect or disconnect tubing. Unique water-flow system eliminates dead ends that can harbor stagnant water Positive Positioning lets doctors smoothly place the delivery where they want and keeps it in place LED Coolant indicator lights up when a handpiece is lifted out of the holder, so there s no more second guessing which knob to use when making adjustments Customizable placement with six positions on the control head, so doctors can integrate almost any devices they choose Removable tray holder is integrated stackable and customizable Left/Right convertible without the use of tools Data Control System (DCS) is used to control all of the electrical components (fiber optics, heated syringe, touch pad, camera, electric micro-motors, and dental light). DCS allows plug and play add-ons Chair lockout stop feature prevents the accidental movement of the chair when a handpiece or instrument is in use Silicone handpiece tubing contains anti-microbial AlphaSan and quick-disconnects secured to the control block A 2.0 liter, quick-connect water bottle is designed with an interior pickup tube which reduces the chance of cross-contamination The system flush toggle has a flow-through design that eliminates stagnant water and flushes all tubings simultaneously. All interior water lines used throughout delivery system contain antimicrobial AlphaSan which protects against bacterial growth on interior and exterior surfaces. Space to integrate up to six clinical devices The flex arm enables easy routing of cables and tubings USB extension cable is provided in delivery system umbilical Handpiece holders are independently adjustable a full 45 degrees to allow precise positioning of instruments ASSISTANT S ARM: Articulated 6-axis arm (long arm) and 5-axis (short arm) allows for vertical and horizontal adjustments and greater than 270° range of positioning of the assistant s instrumentation, promoting improved ergonomics and safety for the assistant , and accommodating both two and four handed dentistry. Standard multi-function touchpad provides secondary control of the chair, light, and accessory functions Capability of holding ancillary devices (such as curing light, warm water syringe, etc.) DENTAL CHAIR: Integrated 300 Watt power supply provides electrical requirements for chair-mounted dental light and ancillary products Chair swivels 60° (30° each side of center) 2-position armrests, mounted mid-thigh position, design articulates forward for patient access Santoprene surface Other brands of dental chairs and delivery systems such as Midmark, DentalEZ, and Summit lack the required features and configurations that are required by the New Port Richey CBOC Dental Department. 6. Description of Efforts Made to ensure that offers are solicited from as many potential sources as deemed practicable: Market research was performed using the following techniques/methods to identify potential sources: Vendor Information Pages (VIP) Sources Sought posted at https://beta.sam.gov. Searched GSA eLibrary @www.gsa.gov. Searched Website utilizing Google engine search. Telephone calls to customer (Kimrey, Robert , searched FPDS website at https://www.fpds.gov/fpdsng_cms/index.php/en and NAC website at https://www.va.gov/opal/nac/index.asp. Contacted ADEC sales rep, Perry Greenway @ perry.greenway@a-dec.com. Contacted other VISN to inquire if they have purchased same or similar A-DEC equipment. The methods used were suitable to locate capable SDVOSB s to support the need of New Port Richey. 7. Determination by the CO that the Anticipated Cost to the Government will be Fair and Reasonable: The solicitation will be posted for brand name only; the CO is confident that two or more quotes will be received from capable SDVOSB s. Therefore, the solicitation will be set aside for SDVOSB s. Price Fair and Reasonableness will be based upon a comparison of quotes received. 8. Description of the Market Research Conducted and the Results, or a Statement of the Reasons Market Research Was Not Conducted: Market research was conducted using Vendor Information Pages (VIP); Sources sought were posted on Beta.SAM.gov, and the GSA website was searched at www.gsa.gov and online Websites. The market research via VIP results produced positive responses from SDVOSB s. GSA market research was negative. An online website search revealed other types of dental chair systems are available. However, different brands of dental chair systems will not satisfy the need of NPR clinics. 9. Any Other Facts Supporting the Use of Other than Full and Open Competition: NA 10.
