Description
Pearl Harbor Naval Shipyard and Intermediate Maintenance Facility (PHNSY & IMF) requests for quote (RFQ) N32253-22-Q-0024 with the intent to award a Firm Fixed-Price contract for the Contractor to provide for (1) one base year and (4) four option years, PERSONAL ASSISTANCE SERVICES (PAS) and PERSONAL ASSISTANT (PA) services to assist with performing activities of daily living that an individual would typically perform if he or she did not have a disability. Tasks may include assistance with personal care such as eating and using the restroom, assisting with physical tasks and unclassified cognitive tasks such as facilitation communication access with a reader.� The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 624120, Services for the Elderly and Persons with Disabilities. �The small business size standard for this NAICS code is $12,000,000. The Product Service Code is R799, Support - Management: Other. All offerors are required to submit the following: All firms or individuals responding must be registered with the System for Award Management (SAM) (https://www.sam.gov) by the award date. The solicitation document and incorporated provisions and clauses will be those in effect through Federal Acquisition Circular (FAC) 2022-05, DFARS 02/28/2022, and NMCARS 18-21. Fill out the unit price and total amount on the solicitation document (Standard Form 1449) for the base year CLIN 0001 and 0002 and the four (4) option years for CLIN 1001, 1002, 2001, 2002, 3001, 3002, 4001, and 4002.� Please note: For cost-reimbursable travel CLINs 0002, 1002, 2002, 3002, and 4002, please input $5,000 for the unit price and amount. A price quote including a breakdown of proposed number of FTEs, labor categories, and labor rates.� Please note that 40 hours per week of service is expected. A statement regarding technical approach on completing the work in accordance with the Performance Work Statement (PWS). Please also address how 40 hours per week of support will be ensured for the following: �Coverage if contractor staff is out sick Rotating and intermittent shifts Short notice requests Five (5) pages maximum Submit resumes demonstrating proven ability to provide a qualified PAS. Two (2) pages maximum Past Performance Submit a list with a minimum of two (2) relevant, recent (within the last 3 years) contracts that are currently in process or have been completed by the offeror. These contracts shall be the same or similar in scope, complexity and magnitude to the current requirement. Government contracts are preferred, but commercial references are also acceptable. The following information shall be included for each contract: Name of the contracting activity or commercial firm Date of award Contract number Total contract value Contract type Short description of the requirement Description of why contract is considered relevant Completion of FAR 52.204-24, 52.204-26 and 52.212-3. An OPSEC Plan (Attachment 6) will be required from the awardee (only) at time of award. Contractors shall not include the OPSEC Plan with their proposal submission. Rather, once the intended awardee is known, the Government will notify the intended awardee with instructions on completing the OPSEC Plan and submitting it through DoD SAFE. The OPSEC Plan will be considered CUI once filled in. Therefore, it shall not be submitted through email. Quotes will be evaluated in accordance with FAR 52.212-2 as listed in the attached solicitation. Quotes are due no later than Wednesday, May 11,�2022 at 10:00 a.m. HST, and must be sent via e-mail to both the primary and secondary points of contacts. Questions in regards to the solicitation must be submitted via e-mail only to both the primary and secondary points of contacts. No telephonic requests.
