- Indian Health Service (IHS), Phoenix Area Office on behalf of Colorado River Service Unit is conducting this Sources Sought to identify contractors who possess the capabilities to furnish and install new equipment including the distribution cable network, along with other appurtenances necessary to receive cable TV service. This is not a Request for Proposal (RFP). Indian Health Service is utilizing this tool as part of market research. Your responses will assist IHS in the development of its possible RFQ/RFP and in determining whether any type of small business set-aside is possible for this procurement or whether full and open competitive procedures should be utilized.� The subcontracting limitations are outlined in FAR 52.219-3, 52.219-14, 52.219-27, 52.219-29, and/or 52.219-30, as appropriate. DRAFT STATEMENT OF WORK Cable Television Services EXISTING EQUIPMENT, PARKER INDIAN HEALTH CENTER The Parker Indian Health Center has a total of 27 televisions installed throughout the facility with a majority of the TVs located in waiting rooms and inpatient rooms. The TVs are connected to a cable that is located in the Black IDF closet and the other end of the cable is spliced to all the locations of where there is cable access.� The current cable channels are setup to show (100+) per TV, with a majority of the TVs experiencing interference on the channels. NEW SERVICE, PARKER INDIAN HEALTH CENTER The Contractor shall furnish and install new equipment including the distribution cable network, along with other appurtenances necessary to receive cable TV service.� A site visit shall be conducted to test the existing cable for interference with other electronic devices and examine the state of the existing cable.� The Contractor shall replace all coaxial cables at the PIHC that are determined to be in poor condition or unsafe.� The existing 36 televisions located throughout the facility do not need to be replaced and are out of scope. The Contractor shall also provide cable TV service including several programming options consisting of a minimum of 12 channels or the base entertainment package provided by (Brand Name or Equivalent) with local channels.� The video feed must be able to integrate digital and analog channels at the same time without interference.� The service shall include emergency alert capabilities to instruct employees and patients channels at the same time without interference. Off-site maintenance may be performed remotely, as-needed, to keep the software up-to-date at all times.� REQUIREMENTS � PIHC LOCATIONS: C.1 �CHANNELS:� The Contractor shall provide 12 of the following channels minimum: C.1.1� CNN C.1.2� KSAZ Fox 10 (Phoenix, AZ) C.1.3.� TNT C.1.4.� Discovery C.1.5.� ESPN C.1.6� KTVK-3 TV (Phoenix, AZ) C.1.7.� KPHO-CBS5 (Phoenix, AZ) C.1.8.� The Learning Channel (TLC) C.1.9.� USA Network (USA) C.1.10. Weather Channel C.1.11. HGTV C.1.12. Disney C.1.13. KPNX (Phoenix, AZ) ��������������������� C.2� INSTALLATION SCHEDULE: The installation of cable equipment shall commence no earlier than 8:00 a.m. and continue no later than 5:00 p.m., Monday thru Friday (excluding Federal holidays), or as agreed upon between the Contractor and the Government Point of Contact (POC). C.3. SERVICE/REPAIR/TROUBLESHOOTING: C.3.1. The Contractor shall provide technical support, training, service, testing, calibration, and repair in accordance with manufacturer recommendations required to maintain continued operation of the satellite television service throughout the Period of Performance. C.3.2. The Contractor shall provide a phone number for one (1) hour response for over the phone technical support.� The Contractor shall direct the Government POC, who will assist the Contractor when possible, to perform tasks that are necessary to avoid a service call and expedite repairs.� The Contractor is responsible for providing direction to the Government POC.� The Government POC shall make efforts in good faith, and the Contractor remains responsible for repairs even if additional damage is done during the troubleshooting effort. C.3.3. The Contractor shall provide 48 hour response for repair or service.� If over the phone technical support is unsuccessful, the contractor shall arrive on site for repairs to the cable service within 48 hours. POINT OF CONTACT:� Information Technology Department ����� D.1 The Point of Contact or representative shall be responsible for the following: D.1.1. Monitoring the contractor�s technical progress, including the surveillance and assessment of performance and recommending changes to the Contracting Officer. D.1.2. Performing technical evaluation, as required. D.1.3. Performing technical (daily and final) inspection and acceptance as required. D.1.4. Assisting the contractor in the resolution of technical problems encountered during the performance. D.1.5. The Point of Contact shall accept or reject the satellite equipment and service. Once the Point of Contact has accepted the equipment and service, the point of contact shall certify receipt and acceptance of the equipment and services on the properly prepared invoice. D.1.6. The Point of Contact shall initiate service requests, and when appropriate attempt to provide basic troubleshooting with over the phone technical support as determined appropriate.
