- SOURCES SOUGHT ANNOUNCEMENT: The Department of Veteran Affairs Medical Center, Lebanon VAMC, 1700 South Lincoln Ave, Lebanon, PA 17042, requires contractors to provide services as part of its Community Based Health Care for Homeless Veterans (HCHV) program, in accordance with VHA Handbook 1162.09. The goal of the HCHV program is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient therapeutic services to meet the needs of those Veterans. Lebanon VAMC is seeking to award a five-year Indefinite Delivery / Indefinite Quantity (IDIQ) contract. Contracted Emergency Residential Services (CERS) programs target and prioritize homeless Veterans transitioning from literal street homelessness, Veterans being discharged from institutions, including those in need of medical respite, and Veterans who recently became homeless and require safe and stable living arrangements while they seek permanent housing. CERS Programs, either directly or through linkage with community and other VA services, provide low demand time-limited services such as supporting mental health stabilization, Substance Use Disorder (SUD) treatment services, enhancement of independent living skills, vocational training, and employment services. Emphasis is placed on referral and placement in permanent housing or longer-term residential programs utilizing VA and/or community resources. Lengths of stay in CERS typically range from 90 to 180 days with the option to extend based on clinical need. The purpose of this sources sought is to obtain interests from contractors who can provide housing, outreach services, case management services, and rehabilitative services within 60 miles of Lebanon VAMC for Veterans experiencing homelessness and/or chronic homelessness including Veterans diagnosed with severe mental illness and/or substance use disorders, in community-based residential facilities. This announcement is issued for informational planning purposes only and is not a request for quote or announcement of a solicitation. Submitting information for this announcement is voluntary and VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted as a result of this notice. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. The purpose of this market research is to identify capable sources that are both experienced and qualified to provide the required services stated in the PWS. The contractor shall have all requirements as outline by the attached Draft PWS. Capability Statement and Documentation: Companies are invited to provide their capabilities, experience, and knowledge in providing Community Based Health Care for Homeless Veterans (HCHV) Services. Sources are asked to provide the following information: 1. Company name, address, website, SAM UEI number, business type, socio-economic status (e.g., SDVOSB, VOSB, Woman-Owned, Disadvantaged Small Business, 8(a), general small business, large, business, etc.), and person of contact including telephone number and email address). 2. Indicate and explain your capability of providing the service requirements. 3. Indicate your company s size under NAICS 624221. 4. Please indicate any government contracts held including government agency, contract number, dollar value, period of performance, and government contact information. In addition to a Capability Statement which includes the above information, submissions can include, but not limited to, brochures etc. as attachments.
