- This is a SOURCES SOUGHT notice for market research purposes only. The U.S. Department of Agriculture (USDA) Animal and Plant Health Inspection Service (APHIS) is seeking all interested sources capable of providing professional services support of the occupational health program at the National Bio and Agro-Defense Facility (NBAF) in Manhattan, KS. Services will be provided to approximately 450 workers at NBAF.� The NBAF medical clinic is currently supported by two Government-employed Registered Nurses who will continue to provide services within the scope of their practice. The services required for this project are outlined below. See also attached �Minimum Qualifications of Professionals�. Onsite Mid-level Provider to perform occupational medical examinations and consulting services 20 hours/week Monday - Friday at NBAF. �Services include the following examinations: �Pre-Employment, Baseline, Periodic, Situational, Termination, Return to Work, Workers� Compensation, and Simple Exposure, and Specialty. The contractor must comply with the Nurse Practice Act regulation or the Kansas Board of Healing Arts requirement for physician direction and supervision. Tier 2 � Public Trust -Moderate Background Investigation is required for the on-site mid-level provider. Offsite Board-certified Occupational Health Physician to serve in the capacity as competent medical authority (CMA) and consultant for all NBAF medical clinic operations. Services include telephonic consultation, M-F between 6 a.m. and 6 p.m. on an as-needed basis; reviewing/co-signing medical forms; and providing oversight and standing orders including the onsite NBAF immunization program. On-Call Mid-Level Provider medical consultation. Services shall be provided daily on an as-needed basis including nights and weekends (hours to be determined). � Off-site radiologic services on an as-needed basis at the contractor�s facility. The facility must be located within 20 miles from the NBAF campus (1980 Denison Ave, Manhattan, KS, 66502). Services include providing posteroanterior (PA) (1-view) only and posteroanterior (PA) and lateral (LAT) (2-view). The Government contemplates a firm-fixed-price indefinite delivery indefinite quantity (IDIQ) type contract with a base year and four option years. Services are anticipated to begin September 2022. All interested parties are invited to submit capability statements. Industry feedback on all aspects of the planned procurement is encouraged. The following outline is recommended for capability statements (10-page limit): Business name, address, UEI (SAM), GSA contract number (if applicable) A statement of capability that demonstrates the ability to perform these services; ability to recruit and retain qualified medical personnel and substitutes; and ability to perform radiologic services at a facility located within 20 miles of NBAF.� Include a description of the aspects of this project your company is capable of self-performing as a prime contractor. Examples of relevant past performance. Include dates, dollar amounts, description of services performed, point of contact. Describe how the example is relative to this project. (Note: Prior Government contract work is not required for submitting a response.) The acquisition strategy has not yet been determined. USDA APHIS reserves the right to determine if an acquisition strategy, such as a small business set-aside, is appropriate based upon the capability information provided in response to this notice, and other pertinent information gathered by the contracting officer. Respondents should ensure the information provided is detailed, current, accurate, and complete. This notice does not constitute a solicitation. This invitation for capability statements is for planning purposes only. Response to this notice is strictly voluntary. The Government will not reimburse any party for its participation in this effort. Information submitted in response to this notice is subject to disclosure under the Freedom of Information Act. Respondents are advised that the Government is under no obligation to acknowledge or provide feedback with respect to any information submitted under this notice. Telephonic requests for information will not be honored.
