- AMENDMENT 0001 The purpose of this amendment is to: 1. Provide answers to questions received from industry. 2. Change On-Call CLINs (0002, 1002, 2002, 3002, 4002) to Overtime CLINs. 3. Advise prospective contractors that no more questions will be answered. All other terms and conditions remain unchanged. -------------- The Regional Health Contracting Office Pacific at Fort Shafter, Hawaii intends to issue a contract for a firm-fixed price, personal services contract for Case/Utilization Manager services for the 51st Medical Group located on Osan Air Base in South Korea. This requirement is for a period of performance on or around 1 June 2022 � 31 May 2023 (base period), with four (4) one-year option periods and FAR 52.217-8, Option to Extend Services. Required qualifications for Case/Utilization Manager services includes but is not limited to: 1. Education: The Case/Utilization Manager shall be a graduate of a Baccalaureate Degree program in nursing from a nationally accredited educational institution authorized to grant baccalaureate degrees approved by USAF Surgeon General. 2. Licensure/Certification/Registration: The contractor must possess a current and fully active unrestricted license to practice as a registered nurse. The Case/Utilization Manager shall possess and maintain throughout the course of the contract a current, active, valid, unrestricted (with no limitations, stipulations, or pending adverse actions) state nursing license in one of the 50 United States, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands. License must be recognized and accepted by the U.S. State or Territory of practice in accordance with governing regulations. 3. The Case/Utilization Manager shall possess and maintain throughout the course of the contract a current Basic Life Support (BLS) certification through an American Heart Association or American Red Cross approved training program. The Military Training Facility will provide BLS refresher courses. 4. Experience: The Case/Utilization Manager shall meet one of the below requirements: 1. Possesses at least three (3) years of nursing experience within the last five (5) years in: inpatient or outpatient nursing requiring discharge planning, utilization management or closely related fields. 2. Possesses 18 months of case management experience within the past five (5) years. 3. Possesses one of the following certifications: Commission for Case Manager Certification � Certified Case Manager (CCM); American Nurses Credentialing Center Nurse Case Manager (RN-NCM); National Academy of Certified Care Managers � Case Manager Certified (CMC). The proposed acquisition will be solicited as unrestricted. The NAICS code for this proposed procurement is 621399 and the annual size standard is $8.0 M.� gNo paper copies of the solicitation will be issued and no solicitation mailing list will be complied. NOTICE TO OFFERORS All sources may submit a quote as applicable. However, the Republic of Korea (ROK)- United States (U.S.) Status of Forces Agreement (SOFA), may preclude the U.S. Government from granting Invited Contractor or Technical Representative Status to any offeror, for this requirement. All sources submitting a quote, including U.S. sources, must already have OR confirm their ability to obtain the requisite business licenses directly from the ROK Government, and conduct business in accordance with all applicable ROK laws, including but not limited to, rules and policies regarding labor, taxation, and immigration. If an initial quote does not fully demonstrate this authorization and capacity, it will not be further considered for award. Whether an offeror demonstrates this authorization and capacity shall be determined by the Contracting Officer.
