SOURCES SOUGHT � REQUEST FOR INFORMATION This is NOT a solicitation or request for quote (RFQ).� This is NOT a request for proposal (RFP).� This Sources Sought is for market research purposes only.� No award will be made off this posting. Responders are solely responsible for all expenses associated with responding to this RFI.� No classified information should be included in the RFI response. The Montana Army National Guard is seeking an occupational health organization, with a physician board certified in occupational medicine to provide medical examinations as directed by the Occupational Health Nurse. This organization shall provide services to areas in Montana near or possibly on location where Federal Technicians work. The contract type is undetermined; a BPA or IDIQ are being considered. INTERESTED VENDORS PLEASE PROVIDE THE FOLLOWING Capability statement describing ability to perform occupational health medical surveillance screening exams Socio-economic status in relation to NAICS 621399-Offices of All Other Miscellaneous Health Practitioners or a similar NAICS. CAGE code and UEI (SAM Unique Entity Identifier) If applicable, FSS/GSA contract number Proof of ability to provide a board-certified occupational medicine provider If applicable, subcontracting plan Recent (within the last three years) and relevant past performance information where the offeror has provided the same or substantially similar services. For each past performance reference include the date of sale, description, dollar value, client name, client address, client point of contact name, client point of contact mailing address and phone number, and e-mail address (if different from that provided for client) Informational pricing is encouraged DRAFT REQUIREMENT The purpose of the Occupational Medical Surveillance Examination Program is to provide job-related Occupational Health Examinations and Medical Surveillance Services to the MTARNG full-time support (FTS) and/or temporary personnel who are exposed to OSHA level Permissible Exposure Limit (PEL) health hazards in the work environment. MTARNG technicians must be monitored for the impacts of known and potential workplace exposures and be provided medical surveillance evaluations based on job code and/or identified exposures. The medical surveillance program is a risk based medical screening and examination process to monitor for adverse health effects due to work related exposures and to improve and maintain technician health. The program incorporates various elements that identifies technician workplace exposures to physical, chemical, biological, and radiological hazards, the effects of such exposure and directs measures to prevent occupational illnesses and injuries associated with these exposures. Program requirements vary based on the individual�s job code, the type of work performed, and the exposures at the worksite. Contractor shall provide new hire, periodic, and termination occupational health medical screening exams.� The technicians in need of the exams are located throughout the state of Montana to include Kalispell, Missoula, Helena, Belgrade, Billings, and Culbertson.� The contractor will need to have screening locations in these specified areas or have the means to travel to these locations when medical screening exams are required.� The services requested are for testing, screening, and evaluation purposes only. Medical treatment is not required nor authorized. Personnel conducting exams shall be current and appropriately licensed, trained, boarded, and certified to perform medical surveillance screening and testing.� The personnel shall operate under the direction of a Board-Certified Occupational Medicine Provider. Services that may be required for the medical surveillance screening exam may include: audiogram, chest Xray, spirometry, pulmonary function test, vision screening, medical history, height, weight, vital signs, body system assessment, respirator use clearance, fit testing, �and labs to include: complete blood count with differential, liver function testing, comprehensive metabolic panel, heavy metals, blood zinc protoporphyrin, blood lead, blood cadmium, urine cadmium, beta 2 microglobulin, and urinalysis (dipstick). Medical reports shall be provided to the MTARNG OHN with results and any recommendations for further care or follow-up. Medical testing requirements for medical surveillance examinations must include, but are not limited to the following. The OHN will arrange for the specific examination components with the provider/clinic for each appointment. The contractor will provide a document with costs per line item test. Audiogram - IAW Department of the Army Pamphlet (DA PAM) 40-501, Department of Defense (DoD) 6055.05-M. Chest x-ray (CXR) - IAW DoD 6055.05-M. Standard CXR. Single view/ B reader CXR as per individual technician actual or potential exposure. Laboratory Testing - IAW DoD 6055.05-M. Complete Blood Count with differential (CBC w/diff) Liver Function Testing Comprehensive Metabolic Panel (CMP) Heavy Metals Blood Zinc Protoporphyrin (ZPP) Blood Lead (BL) Blood Cadmium (CdB) Urine Cadmium (CdU) Beta 2 Microglobulin Urinalysis (UA) - dipstick Spirometry/Pulmonary Function Test (PFT) - IAW DoD 6055.05-M Vision Screening - IAW DoD 6055.05-M, DA PAM 40-506 (corrected near and distant monocular and binocular visual acuity, depth perception, color, confrontational visual fields) Medical screening will consist of the following IAW DoD 6055.05-M and rendered on a case by case basis as arranged by the OHN. Provider will utilize historical records in the evaluation and examination of employee in the event a termination examination is requested. Medical History � medical history including past, current and family status, home exposures, work exposures, work related complaints (past and current), hearing, ergonomic, and use of PPE at work. 1Physical Examination � height and weight, vital signs and assessment of body systems including vision, hearing, cardiopulmonary and musculoskeletal assessments. Screening Examination � interview with technician to discuss available results and questionnaires and annotate notes in medical record. Medical Clearance for Respirator Use � IAW 29 CFR � 1910.13, App C and AR 11-34; review of spirometry results and OSHA Respirator Medical Questionnaire, as applicable. Written medical clearance for respirator use signed by physician. Fit Testing - contractor personnel will be appropriately trained to perform qualitative and quantitative fit testing IAW appendix A of 29 CFR � 1910-134 for requested fit testing procedures. Fitness for Duty (FFD) - IAW DoD 6055.5-M; provide written documentation of physician's determination of the technician's ability to medically and physically perform the assigned duties including essential job functions without substantial risk of harm to themselves, others or the workplace. Medical examination results that are received after the service date will be reviewed by Board Certified Occupational Medicine Physician (BCOMP) and be provided to MTARNG OHN with any recommendations for further care or follow-up.
