- NOTICE OF INTENT TO SOLE SOURCE The Department of Veterans Affairs intends to issue sole-source firm-fixed priced to Stryker Communications Inc., 571 Silveron Blvd. Flower Mound, TX 75028-4095. A sole source procurement will be issued to Stryker Communications Inc. because they are the only vendor capable of fully performing the service. Due to the proprietary & specialized nature of the work it must be conducted by personnel with a high level of training & qualifications. This procurement is being conducted in accordance with FAR Part 8.405-6(a)(1)(i)(C) Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements. The NAICS code is 811219 and has a size standard of $20.5 Million for this procurement. DESCRIPTION/SPECIFICATION/WORK STATEMENT STATEMENT OF WORK PART GENERAL INFORMATION INTRODUCTION Mountain Home VAMC is in need of a vendor to provide turn-key installation of structural Unistrut with a Stryker S-Series Gas and Electric Boom in the Emergency Room at the James H. Quillen VAMC, TN. BACKGROUND The Stryker S-Series Gas and Electric Boom is being installed to support the installation of a CT Scanner for the Emergency Department expansion project, the Stryker S-Series Gas and Electric Boom is the standard configuration used throughout the hospital. SCOPE OF WORK - Contractor shall provide all services, labor, and associated materials to facilitate as follows: Work to be performed as indicated below: a) Install structural ceiling unistrut for boom meeting manufacturer s engineering specifications b) Mount boom to structural Unistrut c) Connect Gas Outlets d) Connect 110V electrical wiring to boom STATEMENT OF WORK PART SUPPORTING INFORMATION Place of Performance James H Quillen VAMC, Buildings 204 or vendor s facility. Period of Performance Performance needs to begin as soon as possible following the contract award. Work will be accomplished per Engineering Project Manager timelines. Scheduling will be coordinated and approved by the Engineering Project Manager. Contractor Furnished Materials All labor, parts, supplies, and tools to complete the work identified. Items must be fully compatible and fully warranted. Security Requirements Per IT Checklist 6500.6 the C&A requirements do not apply and a Security Accreditation Package is not required. 2. CONFORMANCE STANDARDS: Contract service shall ensure that the equipment functions in conformance with the latest published edition of NFPA-99, OSHA, includes performance standards and specifications. 4. PAYMENT: Invoices will be paid in arrears on a monthly basis. FujiFilm will follow the VA's electronic invoice submission process through the Tungsten (fka OB10) network. https://www.tungsten-network.com/customer-campaigns/veteransaffairs/ INVOICE REQUIREMENTS. Payments will be made by the VA, paid directly to the contractor, in accordance with the Prompt Payment Act. Invoices shall be submitted in an original to the FSC in Austin, Texas. To constitute a proper invoice, the invoice must include the following information and/or attached documentation: Name of business concern and invoice date. Contract number. Purchase Order number. Price, payment terms and any discounts, rebates or concessions that apply. Delivery terms (FOB Destination). 5. ADDITIONAL INFORMATION The C&A requirements do not apply, and that a Security Accreditation Package is not required. Other important considerations: Cite any other information that is necessary for potential respondents to understand the nature of the potential requirement or proposed acquisition. Capability statement /information sought: Response to this announcement shall not to exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide a product and related services; (2) The respondents DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc,) pursuant to the applicable NAICS code; (3) Information should be provided electronically in a Microsoft Word or Adobe PDF format. This notice of intent is not a request for competitive quotes. However, interested parties may identify their interest and capability to respond to the requirement and submit quotations with the subject line ""Special Notice # 36C24922Q0327 by 10:00 a.m. CST on 05/10/2022 to: darrell.wright@va.gov. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses received to this notice is solely within the discretion of the Government.
