- DEPARTMENT OF VETERANS AFFAIRS VETERANS INTEGRATED SERVICE NETWORK (VISN) 16 REQUEST FOR INFORMATION 36C25622Q0904 REQUESTED RESPONSES: The intent of this Request for Information is to establish sources in order to define the procurement strategy. Interested contractors are requested to respond in accordance with the following Please respond to this RFI if you are capable of providing the brand name products listed in the Performance Work Statement found in the background section below. In the response, please cite business size status. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large), proof that supplies are domestically made, proof that supplies are from a domestic small business if offeror is not the manufacturer, and unique entity identifier used in SAM.gov. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing these types of supplies. Please include the following: (a) Proof of OEM or OEM distributor of brand name supplies. (b) examples of prior completed Government contracts, references, and other related information. BACKGROUND: Performance Work Statement Background: We are requesting the purchase of a quantity of 3 each Medtronic INVOS PM7100 Cerebral Oximeters or equivalent. We are requesting these units because they are required by the VHSO Surgical Service for use during Orthopedic surgeries to monitor oxygen levels in patients. Scope: The Contractor will furnish all labor, transportation, parts, and expertise necessary to provide the VHSO with 3 each Cerebral Oximeters and the necessary training. Salient Characteristics for each Cerebral Oximeter Unit: The Monitor for each unit must feature a projective LCD multi-touch screen and must have at least 1280x800 pixel resolution. Monitor must include a Li-ion battery and AC power adapter. Monitor must feature a 3.0 USB and 2.0 USB in docking port. The docking station must have a DC in, USB 2.0 ports and VGA ports. A stand for each unit is required for mounting. The Preamplifier must include a preamplifier cable, a re-usable sensor cable, and a power cable. Power requirements for each unit must be 100V-240V with a 50Hz-60Hz input frequency. The battery for each unit must be capable of at least 1 hour of operating time and must be capable of recharging in 24 hours. Delivery Schedule: The requested equipment is required by 06/18/22 and will be delivered to the VHSO as soon as possible. The Contractor will ensure all deliverables are delivered to the following address: Veterans Healthcare System of the Ozarks, Warehouse Building 8, 1100 N. College Avenue, Fayetteville, AR 72703. Contractor Requirements: The Contractor shall not perform any service or provide any item that is outside the scope of work without the written approval of the Contracting Officer. Should the Contractor have to come to the VHSO for any reason other than delivery, the Contractor shall be required to log in with VA Police Dispatch (Building 1, first floor) upon arrival to obtain a contractor badge. The Contractor will always visibly wear the badge while on VHSO premises and will be required to turn in the badge before leaving VHSO premises.
