Description
This Sources Sought Notice is to seek potential socio-economic concerns who may be able to provide Certified Diagnostic Medical Physicist services on behalf of the Department of Veterans Affairs.  This market research survey is not a Request for Quotes/Proposals, Proposal Abstracts, or Invitation for Bids. This notice does not guarantee any future procurements/awards with the Department of Veterans Affairs. The Department of Veterans Affairs is conducting a MARKET RESEARCH SURVEY to obtain information regarding: (1) the availability and capability of qualified large business sources (2) the availability and capability of qualified small business sources; (3) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concern, (4) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. General Description of Services:  The Contractor shall furnish all labor, material, supplies, equipment, and qualified personnel to  provide on-site certified Diagnostic Medical Physicist  support services to conduct Diagnostic Imaging and Nuclear Medicine testing and audits for existing equipment (Radiographic, Fluoroscopic, and Computed Tomography (CT)).  All services will be under the direct supervision of a Board-Certified Medical Physicist.  The FDA, and New York State Department of Health require testing of diagnostic x-ray equipment to ensure diagnostic imaging quality, and a safe environment of radiation safety for patients, staff, and visitors. The VA requires that these requirements be satisfied.  Contractor shall offer a diverse, multi-modality experienced team of qualified Diagnostic Medical Physicist professionals. In addition, the Certified Diagnostic Medical Physicists is required to complete monitoring and reporting related to Radiation Safety Program.  The Contracted period of performance shall be one (1) base Year, and four (4) one-year options periods if offered by the Contracting.  This procurement is being conducted under FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisitions. NAIC Codes:   The intention is to gain knowledge of the current market capabilities in relation to NAICS Codes 541690. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: Name and address of Agency: POC and Title: Phone/Email: SAM Unique Entity Identifier  (UEI) Code:  Socio Economic Concern status, Identify as one of the following listed below: Service-Disabled Veteran Owned Small Business Veteran Owned Small Business 8(a) Small Business HUB Zone Small Business Women Owned Small Business Small Business Large Business NAICS Codes: Capability Statement Questions: Does the company currently have the capability to provide Certified Diagnostic Medical Physicist Services? Does the company currently have capabilities to provide five or more Certified Diagnostic Medical Physicists that have a minimum of eight years experience within a sixty-five (65) mile radius from the Bronx VAMC or capable of arriving at any site within 1.5 hours upon receipt of notification that same day?  According to the Clinical Engineer, services have been required to be performed on the same day and has had as many as eight Diagnostic Physicists onsite to perform testing s and audits. Per Diem is not authorized. If there are no Certified Diagnostic Medical Physicists within the catchment area, how would the agency be  able to provide services within the 1.5-hour window for an unscheduled visit?  Please Note, All Physicists must be credentialled and have been granted a National Criminal  History Check in order to provide services. Does the company have experience with various pricing methodologies for quality control, Radiation Safety Officer Assistance, Equipment testing and audits, annual briefing on Medical Centers Radiation Safety Program, Technical Assistance and annual imaging protocols for Computed Tomography services and reporting of nuclear medicine equipment? If so, please provide samples of the different types of pricing methodologies used. Does the agency have any past experience within the last three years of providing same or similar services for the Dept of VA or any commercial agency? If so, provide the following information: Name of agency performing services for Contract Dates: Contract Number Brief Description of Duties that were provided: Will your company be a joint venture with another company? State whether your organization would perform this contract in accordance with limitations on subcontracting outlined in 13 CFR 125.6(a)(1).
