- SOW INSTALL FLOW METERS BLDG.#501 STATEMENT OF NEED: The VAGLAHS requires installation of water flow meters for a cooling tower STATEMENT OF WORK: TOWER BLOWDOWN METER Install new customer provided 1.5 flow meter using new CPVC Y strainer and CPVC pipe fittings. Install a CPVC union, ball valve, fittings and CPVC pipe to Y-strainer flush connection. Position Y-strainer flush ball valve so that it can be operated easily from ground level. Anchor the CPVC y-strainer flush piping to wall using unistrut supports. Test new flow meter for proper operation. TOWER MAKE-UP WATER METER Isolate existing defective 3 flow meter Disconnect and remove existing 3 flanged water flow meter and associated 3 flanged strainer. Provide and install new 3 NPT bronze y-strainer using new 3 brass pipe nipples. Install new customer provided 3 flanged water flow meter using existing 3 NPT brass flanges and associated 3 brass pipe nipples. Provide and install new di-electric flange kits for flanged water flow meter. Provide and pipe in Y-strainer 1 1/2 flush ball valve and associated brass fittings and pipe nipples Reposition flow meter station pipe supports as required. Test new flow meter for proper operation PLACE OF PERFORMANCE 11301 Wilshire Blvd. Los Angeles, CA 90073 Building #501 Additional Requirements 4.1 Contractor shall obtain all necessary licenses required to perform this work. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Contractor shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employees fault or negligence. 4.2 The Contractor must comply with all applicable Federal, State and local requirements regarding Worker s health and safety. The requirements include, but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR), Parts 1910 and 1926. The Contractor is solely responsible for determining the legal requirements that apply to activities and must ensure safe and healthful working conditions for its employees. Changes 5.1 The awarded Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof. 5.2 Delay in Performing Contract Services. Contractor should fully understand that the participating VA employees are responsible for providing patient care, thus making advance scheduling a challenge. Contractor shall work closely with the designated COR, to ensure that scheduling is utilized for optimum results. Documentation of delays to provide contract services shall be provided to the COR for a concurring signature from the COR. Contractor s personnel shall be responsible for proper documentation of delays. The document must state the scheduled time that services were required, the reason for the delay, and the actual time that contract services could be provided. Work Hours: Authorization shall be obtained from the Contracting Officer in advance for any work performed outside of normal working hours to arrange for all services at the facility. If this authorization is given for the convenience of the Contractor, there shall be no additional charge for labor, travel time, or other expenses. When a holiday falls on a Saturday, the preceding Friday is observed as a legal holiday by U.S. Government agencies. Also included, would be any other day specifically declared by the President of the United States of America to be a National Holiday. Parking: It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Police. The VA shall not validate or make reimbursement for parking violations of the contractor s personnel under any circumstances. Identification Badges: All Contractor personnel are required to wear identification (I.D.) badges during the entire time they are on the VA Healthcare System grounds. I.D. badges must have an identification picture and shall state the name of the individual and the company represented. All VA provided I.D. badges shall be returned at the end of the contract or upon completion of service. Failure to wear ID badges may result in removal from any of the VA facilities and/or otherwise referred to Government property. Smoking: Smoking is strictly prohibited on the grounds of any VHA facility. Per VHA Directive 1085 dated March 5, 2019, it is VHA policy that all VHA health care facilities, including hospitals, community clinics, administrative offices, and Vet Centers, will be smoke-free for patients, visitors, contractors, volunteers, and vendors effective October 1, 2019. There will no longer be designated smoking areas. VA Policy: Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of VA regulations may result in citations answerable in the United States (Federal) District Court. VHA Supplemental Contract Requirements for Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
