- Description
- The Albuquerque Area Indian Health Service, Santa Fe Indian Health Center, Santa Clara Health Center and Cochiti Health Clinic has a requirement for Pharmacy Reverse Distributor Services. This is a combined synopsis for commercial items for services prepared in accordance with format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is 75H70722Q00045 and is issued as a Request for Proposal (RFP), unless otherwise indicated herein. The solicitation document and incorporated provision and clauses are those in effect through Federal Acquisition Circular 2021-04. The associated North American Industrial Classification System (NAICS) code for this procurement is 541614 with a business size standard $16.5. This requirement is a Set-Aside for Small Business Indian Firms and only proposals from qualified offerors will be considered. Offers will be solicited from Small Business Indian Firm concerns.If we do not receive enough competition and/or reasonably priced offers from the Small Business Indian Firm set-aside, we will then evaluate proposals and consider for award to other Small Business concerns. Any acquisition resulting from this cascading set-aside will be from such a concern as follows: First Tier: Small Business; Second Tier: Service Disabled Veteran-Owned Business; Third Tier: HUBZONE and Fourth: All Other Large Business Concerns. ONLY ONE AWARD WILL RESULT FROM THIS SOLICITATION. FOB Destination shall be for the following Albuquerque Area Indian Health Service facility: Santa Fe Indian Health Center (SFIHC) Attn: Pharmacy Department 1700 Cerrillos Road Santa Fe, New Mexico 87505 Santa Clara Health Center (SCHC) Attn: Pharmacy Department 400 State Road 30 Espanola, New Mexico 87532 Cochiti Health Clinic (CHC) Attn: Pharmacy Department 270 Windmill Road Cochiti Pueblo, New Mexico 87072 **SEE ATTACHED PRICE SCHEDULE AND SCOPE OF WORK FOR ADDITIONAL DETAILS** This solicitation will result in a Single Award with One (1) Base Year from date of award and Four (4) 12-month option periods based on Satisfactory Performance ratings. State Taxes for labor/service costs (The Indian Health Service is exempt from New Mexico State Taxes for materials and supplies.) This solicitation requires registration with the System of Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines.
