Description
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. (ii) The solicitation number 36C26222Q0865 is issued as a Request for Quotation (RFQ) in conjunction with FAR 13.5 Simplified Acquisition Procedures and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract for a base with up to two option year periods. (iii) The provisions and clauses in this document are in effect through the Federal Acquisition Circular 2022-06, May 1 ,2022. (iv) This solicitation intends to award a contract through full and open competition to a business associated with NAICS Code 325412. (v) See Enclosure 2 which includes a vendor offer segment and FAR 52.212-3 Offeror Representations and Certifications Commercial Items (Oct 2018). See Enclosure 3 for a listing of brand name salient characteristics and other requirements. (vi) The Department of Veterans Affairs (VA) seeks to make a procurement for SIR-Sphere Y-90 Resin Microspheres for the VA Healthcare System Southern Arizona Veterans Affairs Healthcare System (VASAHS) in Tucson, AZ. Offeror quotations must demonstrate meeting the salient physical, functional, or performance characteristic specified in this solicitation, otherwise it will be considered non-responsive. The information identified provided is intended to be descriptive, not restrictive and to indicate the quality that will be satisfactory. The interested parties bear full responsibility to ensure their submission demonstrates to the government that they can provide the brand name or equal to supplies being requested. Background: The Interventional Radiology Department at the VAMC will be treating patients with primary and metastatic liver disease with Yttrium-90 resin microspheres. The facility s Radionuclide Use Permit is #02-06186-01. The VAMC is interested in establishing a firm fixed price contract for a base plus 2 option years. Scope: Contractor shall provide all unit dose Yttrium-90 resin microspheres used by Nuclear Medicine and Interventional Radiology, at the VAMC in Tucson, Arizona. (viii) Delivery Requirements: The Contractor shall deliver ordered Radiopharmaceutical doses to: Place of Delivery: VA Southern Arizona Healthcare System, 3601 South 6th Avenue, Building 38 Nuclear Medicine, Hot Lab. Tucson, Arizona. Period of Performance/Delivery Timeframe: The period of performance will start shortly after award for One (1) Year Base Period with the Government having the option to exercise (2) two (1) one-year renewal periods in conjunction with FAR 52.217-9. Additional Delivery Requirements: All orders are to be delivered by 0730 (AZT) on the day specified or prior to the day specified to ensure proper inventory and receipt of the shipment is done by qualified personnel. Provide a daily packing slip accompanying all Radiopharmaceutical shipments to the Nuclear Medicine section for all orders placed. Provide courier service that is in accordance with Department of Transportation, Nuclear Regulatory Commission and other applicable regulating authorities enabling the delivery of all Yttrium-90 resin microspheres. All couriers shall be licensed and shall submit all required paperwork in order to be granted access to VAMC. (vii) The provision at 52.212-1, Instructions to Offerors Commercial Items, applies to this solicitation. Addenda: Submit any questions regarding this solicitation must be received electronically through email to Jonathan.Ford1@va.gov at Jonathan.Ford1@va.gov no later than Wednesday, May 11, 2022, 2:00 PM PST. No questions will be answered after this date/time unless determined to be in the best interest of the Government as determined by the Contracting Officer. Telephonic questions will not be accepted or returned. Submit written offers in accordance with the following instructions, as well as Addendum to FAR 52.212-1, Instruction to Offerors. Oral offers will not be accepted. Complete and submit enclosure 2 via email no later than May 16, 2022, 12:00 PM PST to Jonathan.Ford1@va.gov Ensure to reference solicitation number 36C26222Q0865 within subject line of the email. Any offeror quotes received after the prescribed date/time shall be considered non-responsive and will not be considered for award. Ensure the following are completed and/or included in the quote packet: A. Vendor Information B. Acknowledgement of amendments C. Concurrence/non-concurrence with solicitation terms, conditions, and provisions D. Provide documentation mentioned in the instructions below for section (viii) a d. (viii) Offerors shall submit the following via email: a. Offerors shall review Attachment A conforming to requirements stated by the salient characteristics and the instructions below. b. Offerors shall answer yes or no under the section D, on Page 2 in Enclosure 3. c. If providing an equivalent product, offerors shall either provide information regarding how their product meets the salient characteristics/requirements in Enclosure 3, as well as provide an authorized distributor letter from the manufacturer. d. Offeror shall provide verification that their items are not Gray Market such as examples in Enclosure 1, page X, section Gray Market. e.
