- INSTRUCTIONS FOR CALCULATION OF SELF-PERFORMED WORK IAW Limitations on Subcontracting (VAAR 852.219-10) OFFEROR S SELF PERFORMED ESTIMATED CALCULATIONS At the time of proposal Offeror s are required to complete. Offeror s total proposed price: $ __________ Amount projected to be paid to non VIP listed SDVOSBs: $ __________ Offeror s percentage of estimated Non VIP work (b/a X 100 = %) __________% Offeror s percentage of estimated self-performed work (100 c = %) __________% A/E-Services. In the case of a contract for services (except construction), it will not pay more than 50% of the amount paid by the government to it to firms that are not VIP-listed VOSBs. General construction. In the case of a contract for general construction, it will not pay more than 85% of the amount paid by the government to it to firms that are not VIP-listed SDVOSBs. Special trade contractors. In the case of a contract for special trade contractors, it will not pay more than 75% of the amount paid by the government to it to firms that are not VIP-listed SDVOSBs. PTRP Water Softener Repair - 36C25722Q0566 Questions and Answers 1. Infection Control: Please provide a project-specific Infection Control Risk Assessment (ICRA) for this project. For example, will barriers and negative air be required during construction? The corridor that was reviewed during the site visit will have to be blocked off during construction if barriers are installed. An ICRA will be in issued by VA 2. Temporary Services: Are any temporary utilities needed during construction? NO 3. New Plumbing Drawing: Please provide a drawing showing what the new construction is supposed to be. This needs to show tie-in points, valves, piping, etc. It is difficult to know exactly what the VA wants without some type of drawing. A rough copy of said drawings will be provided 4. Outage Plan: Does the VA have a plan for the outage to tie into the existing water lines? Are there main valves available to shut down the system so it can be drained? When can this be done without disrupting patients? An outage plan will be in place. 5. Existing Water Softener: Where do the new lines tap into the water softener supply and return side? Taps are noted on drawings 6. Existing Water Softener: Does the existing water softener have the capacity to treat the additional water supply? Water softener will be assessed by our engineers, more to come 7. Existing Chase: What is in the existing chase between the water softener closet and the elevator? Is that space available to run the water lines? Yes, you can choose to use available space at chase 8. Requesting site visit be re-scheduled so that all interested vendors can attend. Request granted, May 18th at 0900 9. Please provide a sign in sheet for the site visit scheduled held yesterday at 11am
