THIS IS A SOURCES SOUGHT NOTICE ONLY. This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. The responses received from interested contractors will assist the Government in determining the appropriate acquisition method. The Department of Veterans Affairs (VA), NCO 21- is conducting a market research survey to identify potential qualified sources capable of providing a Brand Name or Equal for Reclining Rotary Chair and Radio Ear Amps at VA Palo Alto Health Care System at 3801 Miranda Ave. Palo Alto, CA 94304. REQUIRED ITEMS: CLIN 0001 Description: Orion Reclining Rotary Chair with Video Nystagmography (VNG) with labor, setup, and training (Please specify if it is all inclusive or separate line items) Manufacturer: Micromedical by Interacoustics Item#: 8526830 CLIN 0002 Description: Aqua Stim Caloric Irrigator Manufacturer: Interacoustics Item#: 8504102 CLIN 0003 Description: AP70 Amplifier Manufacturer: Radio Ear Item#: 8100659 Salient Characteristics of reclining rotary chair: The system must be open, e.g., not enclosed in a booth or room. Must have the ability to test the following in one all-encompassing equipment: Oculomotor Test Battery, SHA VOR Test= Sinusoidal Harmonic Acceleration Vestibular Ocular Reflex Test, Sinusoidal Pendular Test, Step Rotation Test, Caloric Testing, Positional Testing, Dix Hall Pike Test, Video Head Impulse Test. Ability to treat vestibular conditions using the canalith repositioning maneuver. The chair must allow for the patient to sit upright, recline at 30 degrees, and lay flat. The chair must be able to rotate a minimum of 20 degrees/second and a maximum of 200 degrees per second or at test frequencies of 0.01 Hz up to 0.64 Hz. The chair must include restraints to keep the patient in place while it moves. The system must include goggles that include cameras to record eye movements. The goggles must include Frenzel lenses. The goggles must be able to be used with vision and without. The system must include caloric irrigators. The system must include the software to run the equipment through the various tests, software to record and analyze the tests, and software to interpret the results. Salient Characteristics of Radio Ear AP70: Must have hum elimination quality. The testing in the sound booth has to be at a very quiet level The internal circuitry of the amplifier must allow for zero signal distortion during sound field testing in the audio booth. Must be compatible with the Audiometric equipment used at the Palo Alto network VA (GSI Audiostar Pro). RESPONSES MUST INCLUDE THE FOLLOWING INFORMATION: Company Name, Address, Size Standard, UEI#, Cage Code Point of Contact name, phone number, and email address NAICS Code 334510 and Small Business Size Standard 1,250 employees Authorized Distributor letter (If applicable) If not the manufacturer, a statement that the business will not pay more than 50 percent of the amount paid by the Government for contract performance to subcontractors that are not similarly situated entities. Responses are due to Moneque Rodriguez at moneque.rodriguez@va.gov by 3:00 PM Hawaii Time on Friday, May 27, 2022. No submissions will be accepted after this date and time. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. Respondents are solely responsible for all expenses associated with responding to this RFI.
