Description
The Department of Veterans Affairs Safety Office is requesting Inspection and Maintenance of Firefighting Equipment and Emergency Showers located at the South Texas Veterans Health Care System 7400 Merton Minter Boulevard San Antonio, Texas 78229 Audie L. Murphy Campus (ALMC) and Kerrville Campus (KC).� A� walktrough has been scheduled for May 24, 2022 from 10:00a.m. - 1:00p.m.� Adhere to E.4 52.212-2� EVALUATION�COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES (NOV 2021) for the evaluation criteria. The C&A requirements do not apply, and a Security Accreditation Package is not required. Information Systems Officer, Information Protection: The contractor will not have access to VA Desktop computers, nor will they have access to online resources belonging to the government while conducting services in the application of complex adaptive system theory to health care organizations. The certification and accreditation requirements do not apply to this procurement nor is a security accreditation package required. Privacy Officer: The contractor will not have access to Patient Health Information (PHI) nor will they have the capability of accessing patient information during the services provided to the VA. All research data available for Contractor analyses is de-identified. Records Manager: There will be no federal records created, maintained, used or disposition with this contract; regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] 1. The successful contractor will conduct inspection and maintenance of automatic hood fire suppression systems for the following locations at Audey L. Murphy Campus (ALMC). a. Nutrition & Food Service, (N&FS) Main Kitchen, b. Patriot Canteen c. Bartter Clinical Research Kitchen. d. Hazardous Materials Storage Building Service is to be provided semi-annually with the first inspection due NLT April 2022 and the second inspection due NLT October 2022 in accordance with (IAW) National Fire Protection Association (NFPA) 96, 11.2. 2. The successful contractor will conduct inspection, and maintenance of automatic fire suppression systems for the following locations at Kerrville Campus (KC). a. N&FS Main Kitchen b. Canteen c. Paint Spray Booth d. Chemical Storage Room e. KTCC Kitchen, Building 96 f. KTCC Rehabilitation Center Kitchen, Building 11 g. KTCC Recreation Center Kitchen, Building 11 h. Mammography Van Service is to be provided semi-annually with the first inspection NLT May 2022 and the second inspection NLT November 2022 IAW NFPA 96, 11.2 3. The successful contractor will conduct monthly inspections (not exceeding 31 days.) and maintenance of portable fire extinguishers at the ALMC. Service is to be provided from May 31, 2022 to May 30, 2023. All inspection and documentation shall be conducted IAW NFPA 10. 5. The successful contractor will activate monthly 35 emergency showers for a period long enough to verify operation and ensure that flushing fluid is available at ALMC. Service is to be provided from May 31, 2022 to May 30, 2023. IAW American National Standard for Emergency Eyewash and Shower Equipment/International Safety Equipment Association (ANSI/ISEA) Z358.1, and VHA Directive 2009-026 Emergency Eyewash and Shower Equipment. 6. The successful contractor will conduct monthly inspections (not exceeding 31 days.) and maintenance of portable fire extinguisher at the KC. Service is to be provided from May 31, 2022 to May 30, 2023. All inspection and documentation shall be conducted IAW NFPA 10. 7. The successful contractor will activate monthly 10 emergency showers for a period long enough to verify operation and ensure that flushing fluid is available at KC. Service is to be provided from March 31, 2022 to March 31, 2023 IAW ANSI/ISEA Z358.1-2009 and VHA Directive 2009-026 Emergency Eyewash and Shower Equipment. All inspection and documentation shall be conducted IAW American National Standard for Emergency Eyewash and Shower Equipment/International Safety Equipment Association (ANSI/ISEA) Z358.1. Part 2: Documentation(s): The inspection and maintenance of automatic hood fire suppression systems will be conducted in accordance with NFPA 96, 11.2. Monthly, annual, 6-year, and 12-year, maintenance of portable fire extinguishers will be conducted in accordance with NFPA 10. Part 3 Qualification(s): To be considered for this contract, prospective bidders must provide experienced inspectors who are thoroughly knowledgeable in the design, installation, use and flushing of emergency shower equipment as prepared in the ANSI/ISEA Z358.1-2009 and VHA Directive. Prospective bidder must be able to demonstrate a minimum of 2 years of experience in conducting inspection and maintenance of automatic hood fire suppression systems IAW with NFPA 96, 11.2. Prospective bidder must be able to demonstrate a minimum of 2 years of experience in conducting monthly, annual, 6 year and 12-year inspection and maintenance of portable fire extinguishers IAW with NFPA 10. Part 4- Cost: The prospective bidder will submit a breakdown of costs per location for completion of the required inspection and maintenance. The contract will be for one year starting April 24, 2022 through April 23, 2023 with the option to renew for 4 years. Part 5- COVID: VHA Supplemental Contract Requirements for Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors 1. Contractor employees who work in or travel to VHA locations must comply with the following: a. Documentation requirements: 1) If fully vaccinated, shall show proof of vaccination i. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination 2) If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test. 3) Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. 4) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. 2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the following website: COVID-19 Screening Tool. Regularly check the website for updates. a. Contractor employees who work away from VA locations, but who will have direct patient contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. 3. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
