- ********Solicitation updated 01 June 2022 to incorporate Solicitation Amendment FA469022Q003500001 and Attach 3_FA469022Q0035 - Questions and Answers 26 May 2022.� The new documents are available in the solicitation attachments below.� The solicitation due date is not extended as a result of this amendment.**************** The 28th Contracting Squadron, Ellsworth AFB, South Dakota, has issued this Solicitation, FA469022Q0035, to compete and award a single award Firm Fixed Price (FFP) contract using FAR Part 12-Acquisition of Commercial Items, FAR Part 13-Simplified Acquisition Procedures, and FAR Part 37-Service Contracting Procedures for the 28th Medical Group DISA Approved Fax Server Solution requirement. This requirement will be a 90 day delivery period after award, with CLIN specific period of performances for incidental services included within the requirement. This will be a 100% Small Business set aside solicited under North American Industry Classification System (NAICS) code 334111 with a size standard of 1250 employees. To be eligible to compete for Small Business set-aside awards, vendors must be certified in accordance with current Small Business Administration (SBA) regulations. Attached to this Solicitation Notice is the Standard Form 1449 Solicitation, the Attch 1 - Performance Work Statement (PWS) for this requirement, Attch 2 � Wage Determination. All questions and comments on this Solicitation must be submitted in writing to lauri.lykken.1@us.af.mil no later than 26 May 2022 by 3:00pm MDT.� ��Please title all emails with questions with the following subject line:� �Question - FA469022Q0035 � Solicitation_28 MDG DISA Approved Fax Server Solution�.� Questions will be answered only via posting of an amendment to this solicitation on www.sam.gov . All interested and responsible entities are invited to submit a quote that will be considered by the 28th Contracting Office at Ellsworth AFB, South Dakota. The award will be based on the criteria established in the solicitation. Vendor quotes and all items required as listed within the Addendum to 52.212-1, Instruction to Offerors, are due to be submitted not later than 3:30pm on 10 June 2022 to laurie.lykken.1@us.af.mil. Please title all submissions with the following subject line: �Quote_ FA469022Q0035 � Solicitation_28 MDG DISA Approved Fax Server Solution�. Small Business contractors submitting a quote must have and list within the quote the vendors assigned Cage Code, and be registered and ACTIVE in the System of Award Management (SAM) at www.sam.gov in order to receive an award for this procurement.
