Description

This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Background: The Department of Veterans Affairs Medical Center, NCO 9 Network Contracting Office, on behalf of the Memphis VA Medical Center is conducting market research through this source sought notice to seek sources to identify potential experienced businesses that are capable of meeting the requirements per the Statement of Work (SOW) below Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 621910 and sized standard is 16.5M NOTE: Respondent claiming SDVOSB and VOSB status must be registered and CVE verified in VetBiz Registry www.vetbiz.gov. GENERAL INFORMATION 1. Title of Project: Ambulance Service (Special Mode Transportation; SMT) 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The overall project purpose and objectives is to provided Special Mode Transportation for the Memphis VA Health Care System. B. BLANKET PURCHASE AGREEMENT This agreement will be a Firm Fixed Price (FFP) Blanket Purchase Agreement (BPA). Funds will load onto the agreement as an estimate of potential usage. They will only be dispensed upon actual verified invoice is submitted. Any unused funds will be reduced after each performance period. The attached price schedule is the agreed-on price to be used in invoicing. Memphis Medical Facility has also supplied an estimate of prior years usage for cost data on the price schedule. Period of Performance: Base Year: 10/01/2022- 09/30/2023 1st Option Year: 10/01/2023 09/30/2024 2nd Option Year: 10/01/2024 09/30/2025 3rd Option Year: 10/01/2025 09/30/2026 4th Option Year: 10/01/2026 09/30/2027 STATEMENT OF WORK Special Mode Transportation Service (Memphis) The Memphis Veterans Administration Healthcare System (VA HCS) provides Special Mode Transportation Services to Veterans based upon medical necessity. The Contractor shall provide all supplies, materials, equipment, labor, insurance, licenses, quality control, supervision, management, and transportation. To furnish ambulance transportation service (both ALS and BLS) to eligible VA beneficiaries of the Memphis VA Medical Center, Memphis. Tennessee. Contractor shall provide ambulance service twenty-four (24) hours a day seven days a week. Service will include transportation for beneficiaries of the Memphis VA HCS in a 53-county tri-state (Tennessee, Arkansas, and Mississippi) area catchment area (see Attachment 1 Memphis VA HCS Catchment Area Map). The catchment area includes: North Community Based Outpatient Clinic (CBOC), 3461 Austin Peay Hwy, Memphis TN, 38127. South CBOC 1689 Nonconnah Blvd, Memphis TN, 38132. Dyersburg, TN CBOC, 1065 Vendall Rd, Dyersburg TN, 38024. Jackson, TN CBOC, 180 Old Hickory Blvd, Jackson TN, 38305. Savannah, TN CBOC, 70 Harbert Drive North, Savannah TN, 38372. Tupelo, MS CBOC, 1114 Commonwealth Blvd, Tupelo MS, 38804. Holly Springs, MS CBOC, 1700 Crescent Meadows Drive, Holly Springs, MS 38635. Helena, AR CBOC, 131 Quarles Lane, West Helena AR, 72342. Jonesboro, AR CBOC, 2908 S. Caraway Road, Jonesboro AR, 72401. ALS- Advanced Life Support and BLS-Basic Life Support transport will be determined by the VA provider making the request for transportation. 2. Contractor shall furnish linen supplies and/or disposable linen supplies. 3. Contractor shall pick up and escort the patient to his/her appropriate ward, nursing home, or to the travel office when ordered and required. 4. Contractor shall be required to transport patients' personal belongings of reasonable weight end quantity, such as adjustable walkers, shower chairs, wheelchairs, suitcases, and small portable television sets. 5. When a patient is transported with any VA owned equipment the equipment shall be returned to the respective VA Medical Center within 24 hours in the same condition as it was released at no addition cost to the Government. 6. NUMBER OF PATENTS: It is understood and agreed that only ONE (1) patient will be transported on a trip. 7. WAITING TIME: For time lost in waiting at either end or both ends of a trip due to causes beyond the contractor's control, reimbursement may be authorized and claimed. The Contractor will be reimbursed at the rate of one quarter hour (15 minutes). Definition: Waiting time starts from the specified time the authorized representative of the VA requested the patient to be picked up. If the requested time of pick up is 4 a.m. and your driver arrives at 3 a.m. and the patient is not ready to be transported, waiting time will not start until 4 a.m. If the delay is caused by the Government, waiting time will be invoiced in 15-minute increments, any time over 8 minutes will be an additional 15 minutes, under 8 minutes will be dropped. If the contractor anticipates a delay and expects to claim reimbursement a call to the designated employee at the VAMC shall be made to verify the waiting time charges. (This call will verify the arrival at the point of pick up.) Any delay must be reported as soon as the driver is aware of the situation. If the pickup is other than at the VA Medical Center, the Contractor shall call the Travel Clerk or the Administrative Officer of the Day (AOD) on duty as soon as he/she anticipates that a delay may develop for which the contractor expects to claim reimbursement. This call is only for the purpose of 'verifying the arrival time at the pickup point and is not necessary if the Contractor anticipates no delay for which he/she will claim reimbursement. A. Patient s name must be given for verification. B. For delays during the day, the Contractor shall call the Travel Clerk at the medical center to report the delay. C. For delays during irregular tours, evenings, nights, weekends or holidays, the Contractor shall call the Administrative Officer of the Day (AOD) at the medical centers to report the delay. D. Waiting time must be validated by the Travel Clerk or AOD at the time of occurrence to be reimbursed. 8. RATES: Payment of flat rate or mileage rate will be limited to ""one way only"" - the distance over which the patient is transported. The mileage rate will only apply to those miles traveled beyond the flat rate mileage limitation. Local Mileage Limits. The local mileage limit is any mileage within a 30-mile radius of the VA Medical Center, 1030 Jefferson Ave Memphis TN. In the event fraction miles result, the VA will pay the rounded off amount to the nearest mile. Trip mileage will be determined at www.randmcnally.com online Driving Directions from VA to patient's home address. The Rand McNally online Driving Directions (fastest route) will be used to calculate the mileage not the odometer of the vehicle. If the online directions do not go all the way to patient's address, VA will consider the actual mileage within a 5-10-mile difference. NOTE: For all one-way trips ordered under this solicitation, the Contractor will receive the base rate quoted (the ""Base Rate""). The Base Rate will constitute full compensation for one-way trips which do not exceed the ""city radiuses"" as defined herein. The Contractor will also receive the mileage rate quote (the ""mileage rate"") for any one-way trip which extends beyond the ""city radius"". The mileage rate is applicable only for mileage beyond the city radiuses and in no event will the Contractor receive reimbursement of the mileage rate for any mileage within the base rate. 9. ORDERING PROCEDURES. A. Requests for services will be made by telephone (confirmed in writing) and transmitted via facsimile from the VA's designated representatives to the Contractor. All trip tickets signed by designated/authorized VA employees will constitute written confirmation of services rendered. B. Confirmation that vendor can meet our request will be confirmed within 30 minutes of receiving the order (if it is not confirmed at the time the request is placed). If the contractor is unable to provide the service at the time required and wishes to subcontract with another company, the VA shall be notified within 30 minutes of the request of the company responsible for transporting the patient on their behalf. If contractor has not arrived or called within one hour of the time requested, and has not provided this service through another company, the VA reserves the right to request service from another source. The VA will be the sole judge in determining when to order service from another source. The VAMC will contact the contractor first before another ambulance is called. Being unable to provide services may result in letter of concern, cure notice or show cause to terminate the contract. 10. REQUESTS FOR SERVICE A. In connection with LIGHTS AND SIRENS (STAT) Vehicles, the services called for in this group shall be made available by the Contractor within thirty (30) minutes after written or verbal notification by the authorized representative. B. In connection, with URGENT (ACLS OR BLS) Vehicles, the services called for in this group shall be made available by the Contractor within NINETY (90) minutes after written or verbal notification by the authorized representative. C. In connection, with ROUTINE Vehicles, the services called for in this group shall be made available by the Contractor for Next Day service after written or verbal notification by the authorized representative D. Only the VA Medical Center shall determine what type of vehicle is to be ordered. Contractor shall verify the specific type of patient care vehicle requested and supply same within the time appropriate under this contract, otherwise the VA reserves the right to order service from another source. Contractor s evaluations will be negatively impacted. E. At no time, shall a patient be required to wait arrival of a pre-scheduled trip beyond 15 minutes. Failure of the ambulance to arrive at patient's residence or the Medical Center within 15 minutes of the scheduled time may result in cancellation of the trip, services to be secured from other sources. Contractor s evaluations will be negatively impacted. F. In rare instances, it may be necessary for the Contractor to be called to transport the remains of a deceased veteran between the various hospitals for postmortem purposes. Contractor agrees to accept such assignment when called under the provisions of the contract and to supply personnel normally assigned non-emergency vehicles in performing this service. The contractor further agrees to handle such remains with all due reverence and respect. G. The Contracting Officer's Representative (COR) may, when in the best interest of the beneficiary, allow a relative or other person to accompany the patient at no additional cost to the Government. H. Any vehicle can and will be inspected periodically upon arrival at the VA. If determined by VA personnel to be unsafe to transport patients it will be reported to the contractor as unacceptable and the reason why. The contractor will have 30 minutes to send a replacement vehicle. I. Service under this contract will be authorized by the Memphis VA Medical Center, Memphis, Tennessee, only when it has been determined to be in the best interest of the Government and only by designated Travel Approving Officials. A written memorandum with individuals delegated this authority will be provided to the contract holder at the time the contract is awarded. J. If the contractor's place of business is located beyond the local telephone call /one of the VA Medical Center, the contractor agrees to accept telephone orders for service on a collect basis or provide a toll-free number. 11. PATIENT WELFARE AND ABUSE A. The contractor shall be held responsible for patient welfare during transport. The contractor shall be held responsible for patient and/or Government property during transport. Any damaged or lost wheelchairs, walkers, crutches, or personal belongings will be replaced by the contractor at no additional cost to the Government. B. The Government requires the contractor to exercise extreme caution and care in the handling of patients. Any abuse of patients will be documented and could lead to termination of the contract. C. In the event a patient's condition worsens during transport, the Contractor shall transport the patient to the closest facility appropriate for the required care. In the event a patient should expire during transport the Contractor shall return to the VA Medical Center. D. Under no circumstances are photos/videos to be taken of patients. Taking of Photos/Videos/images are prohibited while on VA premises including those of patients/Veterans during transport to/from designation without written permission. Approved ID badges must be always worn above the waist while on VA premises and while picking up patients to transport. 12. CANCELLATIONS: On trips within the mileage thresholds there will be no charge to the Government for cancellations of at least thirty (30) minutes prior notice to scheduled pick-up time. Outside the Mileage Thresholds there will be no charge to the Government prior to the vehicle being dispatched. A ""no-show""' is defined as a trip which is canceled upon the contractor's arrival at the pick-up point. Contractor may request reimbursement for a one-way trip. 13. TOLL CHARGES: It is agreed and understood that the prices quoted in the schedule of services/supplies do not include any toll charges, i.e., ferry, bridges, tunnel; or road. Any such legitimate toll charge incurred shall be limited to ONE WAY ONLY and listed as a separate charge on the VA FORM 10-1511. Receipts are required for reimbursement of all tolls/etc. 14. VEHICLE REQUIREMENTS: A. The emergency medical care vehicles (ambulances), under the terms of this contract, will be licensed and will meet the minimal vehicle requirements as established by the Federal Specification KKK-A-1822E. Dated June 1, 2002, and the rules arid regulations relating to emergency Medical Services (both Federal and State) in effect at the time of this contract. B. Subject specifications are mandatory as minimum requirements for such vehicles, but color, chassis, or compartment modifications are permissible when they clearly exceed the minimal specifications. The basic emergency care vehicle contracted for will be type I. II. and III model, i.e., conventional cab chassis with modular ambulance body (1,2.1 of KKX-A-1822E). Each emergency and non-emergency medical care vehicle will have patient compartment facilities. Oxygen, suction systems and equipment, heart monitor, environmental climatic equipment, communications and additional systems equipment accessories and supplies required by Section 3.11.4 of Federal Specifications KKK-A-1822H. The contractor also must meet all requirements established by the state of Tennessee. C. Each offeror must submit, with their proposal, a letter fully describing the following items used in the performance of this contract. (1) The make of vehicles, model, and year. (2) Certify that all vehicles also have the following features: (a) Tie down straps, (b) Oxygen transport carrier, (per DOT guidelines and regulations) (c) Fire extinguisher, (d) First aid kit, (e) Toolkit. (f) Emergency traffic triangles, and (g) Furnished with essential medical supplies and equipment (Including IV infusion devices and disposable tubing) (3) Certify that the vehicles and equipment planned to provide service under this contract are in good working order and meet the industry standards for design and construction, meets safety standards, equipped with a communication system, air conditioner and heater must be operational to transport ill patients, and vehicles shall meet sanitation and maintenance industry- standards. (4) Include the location end telephone numbers of the establishment where calls are received, and vehicles are available for dispatch. The letter must contain information as to the metering devices or methods offeror proposes to use in determining mileage. E. Contractor should submit a preliminary schedule for VAMC personnel to inspect, at a mutually agreeable time, the vehicles to be used in the performance, of this contract. F. Contractor shall provide Ultraviolet (UV) Lights for the ambulance cleaning daily. 15. PROCEDURES TO FOLLOW WHEN PICKING UP A PATIENT: The contractor's employee will report to the location designated by the travel clerk, or MAA(AOD). The contractor shall be responsible for picking the patient up at the bedside (for stretcher patients) or the location given by the Travel Clerk. All medications or supplies required by the patient will be ready upon Contractor s arrival to avoid unnecessary delays and a waiting time fee. If medications, records, or supplies should be required and are not ready when the patient is being picked up, the contractor's employee should at that time contact the Travel Clerk and let them know there will be a waiting charge if he stays. The Travel Clerk will give the authority to stay or leave at that point in time. VA will provide basic medical, medication, and treatment information for the contractor to provide the appropriate level of care and appropriate response during transport. Contractor shall adhere to HIPPA regulations. UNDER NO CIRCUMSTANCES IS THE CONTRACTOR'S EMPLOYEE TO BECOME AN ATTENDENT FOR THE GOVERNMENT AND BE SENT TO OTHER AREAS OF THE HOSPITAL TO PICK-UP MEDICATIONS ETC. FOR THE PATIENT. 16. INCIDENTS TO BE REPORTED - All accidents, malfunctions, injuries, deaths, and equipment recalls, related to the provision, will be immediately reported in writing to the Contracting Officer or COR and to the manufacturer of the equipment, if applicable. The VA will conduct quarterly patient satisfaction surveys to ensure quality service is being provided. Contractor will be provided a copy of that report for their files. 17. CONTRACTOR FURNISHED SERVICES, PROCEDURES. FORMS AND REPORTS: The following information shall be provided by the contractor' prior to commencement of work, and upon any change thereof during the performance of the contract. A. List of all Vehicles. New/loan vehicles must fee reported before using. B. Inspection report of all vehicles. C. Drivers Licenses, DOT permits and other licenses. D. Education and training records of personnel, oriented and ongoing. E. Written operating plan and procedures for: (1) Accident and incident reporting. (2) Safe operation of vehicles. (3) Quality Control. (4) Equipment maintenance and management (5) Fire and emergency preparedness planning. (6) Handling patient complaints (7) Infection control. (8) Universal precautions (policy and procedure) procedures. (9) Safety and occupational health procedures. (10) Emergency procedures to be followed in case of break down when in route. F. List of all Drivers. New/relief drivers must be reported and meet all requirements before transporting VA patients. 18. PERFORMANCE REQUIREMENTS A. INITIAL STARTUP "" (1) A post-award conference will be held at a time and place designated by the Contracting Officer, to be attended by the Contractor, COR and other officials designated by the Contracting Officer. The conference agenda shall include, as a minimum: (a) Performance Requirements (b) Training Requirements and Schedules (c) Certification of Personnel Qualifications (d) Government-Furnished, Services, Procedures, Forms, and Reports (e) Contractor-Furnished, Services. Procedures, Forms, and Reports (f) Quality Assurance (2) The Contractor shall visit the VA Medical Center with VAMC personnel before performance under the contract for the purpose of performance orientation and site/equipment operation orientation. TRAINING (1) To be eligible to perform under this contract, each contract employee must meet the minimum training required outline below and minimum contractor furnished training outlined in paragraph (2) (c). (1) Through (8). (2) Contractor Furnished Training. (a) The Contractor shall submit to the Contracting Officer written certification that each contract employee has been trained in the subject areas outlined in paragraph C prior to assignment under this contract. For replacement or new employees documentation of prior training must be submitted or the individual contract employee must complete the required training before being employed in support of this contract. (b) All formal training required shall be administered (i.e., taught, presented) by persons who are certified as being qualified to instruct or teach, the specific subjects or topics required. Certification to instruct the specific subject shall be in the form of a certificate issued by an accredited institution of learning (school, college, university) a governmental (Federal, State, County, etc.), Educational certification body (agency, board, commission), or by documentation that the person instructing has sufficient experience in/with the subject to be able to instruct the subject in an authoritative, practical, and current manner. (c) In addition to the formal training, the Contractor shall provide 16 hours of orientation training for each employee fourteen (14) days after his or her initial assignment to duty. This orientation may be accomplished while the employees are on duty. The Contractor shall certify to the COR as to the completion of the orientation for each employee within 45 days following assignment to duty. The follow-up orientation should include the following subjects and instructional time periods as well as other subjects as may be required: (1) Accident and incident reporting (2 hours) (2) Safe operation of emergency vehicles (2 hours) (3) Quality Control (2 hours) (4) Fire and emergency preparedness planning (2 hours) (5) Handling patient complaints (1 hour) (6) Infection control (2 hours) (7) Universal precautions (policy and procedure) (2 hours) (8) Safety and occupational health procedures (3 hours) (3) Contractor Furnished Training - The Contractor shall submit to the Contracting Officer written certification of the follow-up orientation. (4) The EMT/Paramedics must be enrolled in '""refresher"" continuing education, or advanced training programs as required by the local or state government entity in which the service is rendered to veterans or, a yearly basis. Such ""refresher"" training must be equivalent to that developed by the Department of Transportation, National Highway Safety Administration. Evidence of successful completion for ""refresher"" training must be provided to the Contracting Officer for inspection. 19. PERSONNEL REQUIREMENTS: Contractor shall have a minimum of two (2) year experience in this field. Minimum Qualifications Standards (1) Contractor - Education/Experience: Possess a high school education or equivalency and have two (2) years of experience demonstrating the ability: to meet and deal with the public; to read, understand and apply printed rules, detailed orders, instructions, and training materials: to maintain pose and self-control under stress; and to construct and write clear, concise, accurate and detailed reports as determined by successful, high level of performance with other customers. Any type of military service may be credited toward meeting the requirements. In lieu of the above, each employee shall have two years of relevant education at a residence school above the high school level, or any combination of relevant education above the high school level and relevant experience totaling two (2) years. (2) Drivers: (a) Education/Experience: Possess a high school education or equivalency and have two (2) years of experience demonstrating the ability: to meet and deal with the general public; to read, understand and apply printed rules, detailed orders, instructions, and training materials: to maintain pose and self-control under stress; and to construct and write clear, concise, accurate and detailed reports as determined by successful, high level of performance with another clientele. (b) Possesses a valid Tennessee operator s license issued by the State of Tennessee. Has not, within the last three years, been convicted of reckless driving or driving under the influence of alcohol or controlled substances and has not had a driver s license suspended under the point system issued by the state. Possess a valid American Red Cross standard first aid and personal safety course card or its equivalent. Drivers must provide full name, date of birth, and social security number to VA prior to operating vehicles with VA beneficiaries. Proof in the form of a current certificate that such first aid training has been successfully completed must be available upon request. (c) Possess a working knowledge of route specific to the area and knowledge of the nearest hospital while in route. (3) Attendants-Emergency Medical Technician (EMT) and Paramedics Prices quoted in the schedule shall include the services of a minimum of two (2) Emergency-Medical Technicians (EMTs) for BLS and a minimum of one EMT and one Paramedic for every ALS Service. In the event the VA Medical Center determines it necessary for the patient to have more than one ALS attendant at bed side during transport, the Contractor must provide the additional attendant. If the patient being transported requires mechanical ventilation, and the Contractor does not supply the mechanical ventilator a respiratory therapist will accompany the patient during transport and wilt assist with care of the patient as needed. If the Contractor has brought his or her own mechanical ventilator, it is not necessary for a VA respiratory therapist to accompany the patient. If the patient being transported requires accompaniment by an RN, and the Contractor does not supply the RN, an RN will accompany the patient during transport. The RN's responsibilities during transport will be clearly outlined in written physician orders. In the event of emergency, the care of the patient in transport will be the responsibility of the EMT/Paramedic. NOTE: VA physicians RNs, and/or Respiratory Therapists are not required to be licensed in the State of Tennessee for VA purposes. These individuals are covered under the Federal Tort Claims Act. EMT's and Paramedics must provide full name, date of birth and Social Security Number to VA prior to assignments on vehicles with VA Beneficiaries. Paramedics in addition to B (2) (a) above shall be licensed in advanced and basic life support and having completed an appropriate training course in the last two years. Attendant should have a minimum of four hours of training in crisis intervention (documented), since some of the patients requiring an attendant will be a psychiatric patient. An attendant shall always be required for stretcher patients strictly for their safety during the trip and help during transfer to and from van. EMT's in addition to B (2) (a) above licensed in basic life support and having completed an appropriate training course in the last two years. Attendant should have a minimum of four hours of training in crisis intervention (documented), since some of the patients requiring an attendant will be psychiatric patients. An Attendant will always be required for stretcher patients strictly for their safety during the trip and help during transfer to and from van. (4) Age: Contractor and employees performing work under this contract shall be a minimum of eighteen (18) years of age or as required by local laws. (5) Health and Physical Fitness Requirements: (a) All employees assigned by the Contractor to work under this contract shall be physically able to perform all general duties, functions and activities; shall be free from any communicable disease: shall be in good general health without physical defects or abnormalities which would interfere with the performance of these duties: shall possess binocular vision correctable to 20/30, (Snellen chart): shall be free of color blindness, and be capable of hearing ordinary conversation at 15 feet with either ear without benefit of a hearing aid. Certify under oath that he is not addicted to alcohol or any controlled substances. Physical examination report to describe above requirements for each driver shall be provided upon request. (b) All employees shall be literate and able to communicate (speak) with clients in English to the extent of reading and understanding printed regulations, detailed written orders, training instructions and material, and shall be able to compose reports which convey complete information. The Contractor is to certify the ability of all employees designated to provide service for this contract. 20. CONFIDENTIALITY a. VA will provide the contractor with access to pertinent patient medical information, with the existing privacy rules and regulations, for the purpose of providing coordinated comprehensive primary care. Contractor shall ensure the confidentiality of all patient information and shall be held liable in the event of the breach of confidentiality. b. Any contract resulting from this solicitation will he subject toot the Privacy Act of 1974, Federal Acquisition Regulations 52.224-1 Privacy Act Notification and 52.224-2 Privacy Act apply to this contract. The contractor is not authorized to release any medical record information. The VA is the sole entity authorized to release this information upon written request from the patient. 21. HIPAA Compliance: Contractor shall adhere to the provision of Public Law 104-191, Health Insurance Portability and Accountability Act (I11PAA) of 1996 and the National Standards to Protect the Privacy and Security of Protected Health Information (PHI). As required by HIPAA, the Department of Health and Human Services (HIIS) has promulgated rules governing the security and use and disclosure of protected health information by covered entities, including the Department of Veterans Affairs (VA). In accordance with HIPAA, the contractor may be required to enter into a Business Associate Agreement (BAA) with VA. HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT OF 1996 (HIPPA): In accordance with 45 CFR 164.502(e), the Privacy Rule includes exceptions to the Business Associate standard. This contract and its requirements meet the following exception and does require a Business Associate agreement in order for Covered Entity to disclose Protected Health Information to: a health care provider for treatment. The C&A requirements do not apply, and that a Security Accreditation Package is not required. The use of tablets for tracking trips/runs must be always secure and are required to have VA approved encryption such as  Symantec Endpoint Encryption 8.2.1 MP16 is currently baseline. Symantec Encryption Desktop (formerly PGP Desktop), Soon to be approved Microsoft s BitLocker All printed documents with patient s information must be destroyed after use. Document destruction must meet the following requirements. THE GOVERNMENT HAS DETERMINED THAT ALL RESIDUES MUST BE REDUCED TO NOMINAL EDGE DIMENSIONS OF FIVE MILLIMETERS (5 MM) AND SURFACE AREA OF TWENTY-FIVE SQUARE MILLIMETERS (25 MM2). THIS WILL ENSURE THAT NO PERSONALLY IDENTIFIABLE INFORMATION (PII) OR VA SENSITIVE INFORMATION IS PRESENT AND EASILY READABLE AS IT WOULD BE ON A LARGER PARTICLE SIZE. Proof of destruction must be maintained and made available upon request from the COR or privacy officer. Contractors, contractor personnel, subcontractors, and subcontractor personnel shall be subject to the same Federal laws, regulations, standards, and VA Directives and Handbooks as VA and VA personnel regarding information and information system security.
