- This Sources Sought Notice is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation or commitment by the Government. This notice is intended strictly for market research. The purpose of this Sources Sought notice is to obtain information from qualified vendors with special interest relative to the North American Industry Classification Code (NAICS) 424490 - Other Grocery and Related Products Merchant Wholesalers, and 311812 - Commercial Bakeries. This sources sought is set-aside for Small Businesses. The Government will not pay for information received in response to this sources sought, nor will the Government compensate any respondent for any cost incurred in developing the information provided to the Government. This sources sought does not constitute a commitment by the Government. Information provided in response to this market survey will be used for informational purposes only and will not be released. Vendor participation is not a promise of future business with the Government. Requirement The Veterans Integrated Services Network (VISN) 12, Great Lakes Health Care System, Nutrition & Food Services (NFS) Division, has a requirement for bread and bakery products needed for food production and meal service for the following VA Medical Centers: Jesse Brown VA Medical Center Capt. James Lovell Federal Health Care System 820 S. Damen Avenue 3001 Green Bay Road Chicago, IL 60612-4223 North Chicago, IL 60064-3048 Edward J. Hines, Jr. VA Hospital Clement J. Zablocki VA Medical Center 5000 S. 5th Avenue 5000 W. National Avenue Hines, IL 60141-3030 Milwaukee, WI 53295-0001 Tomah VA Medical Center William S. Middleton Memorial Veterans Hospital 500 E. Veterans Way 2500 Overlook Terrace Tomah, WI 54660-3105 Madison, WI 53705-2286 Oscar G. Johnson VA Medical Center 325 East H Street Iron Mountain, MI 49801-4760 The Government is seeking a contractor/s capable of providing the services of delivering a variety of fresh (never frozen) bread and bakery products. Interested contractor/s must be able to provide an uninterrupted source of supply in sufficient quantities to meet the estimated requirements. Further details can be found in the attached Statement of Work. The Government intends to award multiple Blanket Purchase Agreements (BPA) with a base 12-month period and (4) 12-month option periods. All prices awarded under the BPA shall be fixed for a one-month period. Interested Contractors must include the following information when responding to the sources sought: 1. Company name, address, point of contact and email address. List of facilities interested in and capable of providing an uninterrupted source of supply for the period of the anticipated agreement. 2. Size status and representation/s of the company (i.e., Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), HUBZone, Woman Owned Small Business (WOSB), Large Business, etc.) 3. Is the company considered a small business under the NAICS code 311812, identified under this RFI? 4. Does the company produce the referenced items or products being offered? There is no guarantee, expressed or implicit, that the market research for this acquisition will result in a particular set-aside or sole source award, or any other guarantee of award strategy. All information is to be provided on a voluntary basis at no charge to the Government. Interested vendors are reminded that in accordance with FAR 4.1102(a) Offerors and quoters are required to be registered in SAM at the time an offer or quotation is submitted in order to comply with the annual representations and certifications requirements.
