This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular  2022-06. This solicitation is set-aside for SDVOSB The associated North American Industrial Classification System (NAICS) code for this procurement is 334510, with a small business size standard of 1250. The FSC/PSC is 6515 The Buffalo VAMC- 3495 Bailey Ave, Buffalo, NY 14215 is seeking to purchase Stryker Laparoscopes. All interested companies shall provide quotations for the following: Supplies/Services Qty Part # Description 4 0502937010K - Stryker AIM Laparoscope 10mm 0 degree with Safelight adapter 4 0502937030K - Stryker AIM Laparoscope 10mm 30 degree with Safelight adapter 4 0502537010K - Stryker AIM Laparoscope 5mm 0 degree with Safelight adapter 4 0502537030K - Stryker AIM Laparoscope 5mm 30 degree with Safelight Adapte STATEMENT OF WORK (SOW) 1. Contract Title: Stryker 5mm and 10mm Green Laparoscopes (16 each total) for VHAWNYHS Procurement for Buffalo VAMC. 3. Background: For the O.R. For these laparoscopes, our current Stryker Green Lap Scopes inventory can not meet the increased demands of O.R. Services and surgeons. VA standards require clean and sterile Green Lap scopes x 2 of each size, for each case that requires their use. This request is for the the purchase of additional Stryker 5mm and 10mm Green Lap Scopes. 4. Scope. 4.1 The Buffalo VAMC is seeking a brand name (Stryker) purchase of additional units that meet the clinical, functional, and performance specifications specified in this Statement of Work (SOW). The Buffalo VAMC shall only consider new, latest-generation units. Refurbished, remanufactured, and grey market products will not meet the needs of the Buffalo VAMC. Devices shall have FDA clearance for utilization in the United States of America. 4.2 Proposals will be evaluated according to a Best Value protocol. An official Buffalo VA Medical Center Evaluation Team will utilize the Best Value protocol in order to evaluate proposals on the basis of price; conformance to the Technical Elements (detailed in section 5); conformance to the Terms and Conditions (detailed in section 6); Past Performance (detailed in section 7). 4.3 Instruments will be used in simple and complex O.R. minor, major, and trauma cases as additional needed Green Lap Scopes to existing stock of same vendor type scopes. Instruments will be used in the O.R. Instruments will be cleaned, assembled, sterilized and stored in SPS. 5. Specific Technical Elements: The proposed unit shall meet the following Buffalo VA Medical Center technical requirements; Brand Name Stryker 5mm and 10mm Green Lap Scopes. *Offering vendors shall provide complete, detailed responses to each of the following Terms and Conditions in their bid proposals. 5.1 The Stryker 500 and 10mm Green Lap Scopes must be compatible with the current Stryker 5mm and 10mm Green Lap Scopes. 5.2 The vendor shall provide complete technical information sheets and brochures describing features. 5.3 The vendor will supply a picture of such mentioned scopes. 5.5 The vendor shall supply the IFU for cleaning and sterilizing of Green Lap Scopes RMD. (Reusable Medical Device) 6. Technical Terms and Conditions: The offering vendor shall meet the following Buffalo VA Medical Center Terms and Conditions. *Offering vendors shall provide complete, detailed responses to each of the following Terms and Conditions in their bid proposals. 6.1 The Vendor certifies that all devices meet or exceed AAMI (including ST-79 and others), VA requirements; 6.2 It shall be the sole responsibility of the Vendor to deliver all equipment in good condition and delivery of scopes in full, in 1 shipment. Any equipment damaged in shipping or delivery shall be the responsibility of the Vendor. 6.3 The Vendor will provide the VA with all service publications that are used to inform or update SPS personnel on RMD issues, i.e., newsletters, alerts, and/or procedure changes or enhancements. Such publications will be sent to the VA when they become available to the Vendor s field service personnel; 7. Place of Performance: The place of performance for this contract is the Buffalo VAMC.
