Pacific Standard Time, on June 03, 2022 Amended Solicitation No.�75H71322Q00057 due date from 11:00 A.M. Pacific Standard Time, on May 27, 2022 to 1:00 P.M. Pacific Standard Time on June 2, 2022.� This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation no. 75H71322Q00057 has been issued as a Request For Quotation for a firm fixed price commercial items supply contract for laboratory supplies at one lump sum price for items listed on Attachment no. 2- Schedule of Items. The scope of work includes procurement of laboratory supplies required for collection, testing, diagnosis and treatment of diseases including COVID-19 as needed. The estimated performance period is from date of award through June 30, 2027. The performance period includes a one (1)-Base year and four (4) one-year term options. �The required delivery date for supplies is as needed basis. The delivery location and place of acceptance is Chemawa Indian Health Center, Western Oregon Service Unit, 3750 Chemawa Road NE, Salem, Oregon, 97305, F.O.B. Destination. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-06 dated April 26, 2022. �The provision at 52.212-1, Instructions to Offerors � Commercial, applies to this acquisition. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 60 months. As used in this clause�Payment request means a bill, voucher, invoice, or request for contract financing payment with associated supporting documentation. The quoter shall remit a completed schedule of items (Attachment no. 2). The quoter shall enter the unit price and extended totals for each item listed on Attachment no. 2. The price shall include all necessary supervision, management, labor, transportation, equipment, materials, any other direct incidental costs, overhead and profit, unless the cost is identified as a separately priced contract line item. The basis of award is the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Other factors for this solicitation include past performance of the quoter. Past performance information may be based on the Government�s knowledge of and previous experience with the quoter, or other reasonable basis. Award will be made to the responsible respondent pursuant to FAR subpart 9.1.
