- Department of Veterans Affairs is issuing this Request for Information/Sources Sought notice to identify entities that have the resources and capabilities to provide 6k Forklift Rental for the Veterans Health Administration Office of Emergency Management (OEM) located at 510 Butler Avenue, Martinsburg, WV 25405. Background - OEM has a requirement to rent one 6k forklift for 10 months to fill the current gap until the forklifts ordered are received. Once the ordered forklifts are received the rental forklift will be returned. By renting this unit, the logistics staff will be able to complete the necessary training on the unloading, set up, maintaining, take down and loading of the unit. Scope of Work - The contractor shall provide all one 6k forklift according to the following requirement: 6k capacity sit down diesel forklift Reaching height required is a maximum of 144 Mast can be single, two or three staged wide view Length of forks must be 48 at a minimum and no more than 1 and ¾ thick Side shift and fork positioner option required Tires will be all terrain for outdoor use (parking lot area) Back-up alarm and flashing beacon required Side and rear-view mirrors Front and rear flood lights The location of the servicing company needs to be within 35 miles of the facility to ensure prompt service is available in the event of equipment failure. If it is not, the vendor will make every effort to expedite a service call within 24 hours of notification of a mechanical breakdown. Interested & Capable Responses Department of Veterans Affairs is seeking responses from entities that are interested in this procurement and consider themselves to have the resources, capabilities, and qualifications necessary to provide forklift rental. Please respond with your: 1. Business name, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), and person of contact (including telephone number & email address). 2. Capability statement and summary of previous experience for the Department of Veterans Affairs or other similar healthcare facilities. 3. GSA contract number, if your company has one 4. Estimate cost for 6k forklift rental for 10 months. Responses must be submitted by 1:00 PM (EST) July 15, 2022. The results of this market research will assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., small business set-aside, full and open competition, etc.).
