Description
Install Pesticide Storage Shed (Site Prep) Project Number: 583-22-513 Project Scope of Work The Contractor shall provide professional Services to conduct a site preparation to receive a Pesticide storage shed for EMS procured by the VA. The location of which shall be adjacent to Buildings 6, 19 and 22, on the Richard L. Roudebush VA Medical Center (VAMC) campus, located at 1481 West 10th Street, Indianapolis, IN 46202. The contractor shall furnish all supervision, labor, transportation, supplies, equipment, licenses, chemicals, and materials necessary for the accomplishment of the site preparation for the Pesticide Storage shed at the indicated location on the Medical Center campus, between Buildings 6, 19, and 22. Work Requirements: Technical Requirements The contractor shall prepare the indicated site to receive a premanufactured shortage shed. The contractor will demolish the current metal shed. The contractor shall remove debris from the demolition of the current metal shed and disposed of the material in accordance with all Federal, State, and Local regulations. Material and debris WILL NOT be disposed of on-site in any dumpster or trash receptacle not provided by the contractor. The contractor shall provide electrical service in accordance with stamped drawings to be provided BY the VA. Drawings will be provided once the VA has completed the procurement of the Storage Shed. Power will be provided from Building 6. The contractor will provide a white plastic fence similar in style as the current fence surrounding the HAZMAT storage area. The fence will have one Locked Gate The fence will be approximately 80 linear feet and enclose a roughly 17 x13 area. The contractor will provide a Schedule at least 10 days prior to beginning any demolition or construction. General Requirements: All contractor furnished materials, equipment and tools used shall be standard products of manufacturers regularly engaged in the production of such items. The Contractor shall secure all materials before, during and after service. The Government assumes no liability for loss of or damage of Contractor-owned property held on Government premises. RLR VAMC is a federal facility. RLR VAMC, its Contractors, and subcontractors shall comply with all federal, state, and local laws. The most stringent law shall apply regardless of agency hierarchy. The Contractor shall reimburse the Customer for any fines and costs incurred due to non-compliance by the Contractor. The Contractor shall obtain all necessary licenses and/or permits required to perform this work. The contractor shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. The contractor shall be responsible for any injury to the contractor and employees, as well as for any damage to personal or public property that occurs during the performance of this contract due to fault or negligence and shall maintain personal liability and property damage insurance having coverage for a limit as required by laws of the State of Indiana. All tasks accomplished by the Contractor s personnel shall be responsible for all damage or disfigurement of government owned furnishing, fixtures, equipment, and architectural or building structures. Contractor shall report any damage or disfigurement of these items when caused by the. Contractor shall report any damage to the COR and/or their appointed person within the same day, not to exceed 12 hours. Period of Performance (beginning three (3) days after Notice to Proceed (NTP)) Scheduling Start/Finish Dates: Anticipated schedule of when work is to be performed shall be submitted to the Contracting Officer Representative (COR) 10 days prior to the construction start date. Daily Work Schedule: Contractor shall provide the COR with a scheduled plan of the specific areas in which the contractor shall be working, at least forty-eight (48) hours in advance. Any requirements for utilities interruption or hot work permits that will be required must be submitted to the VA COR at least 21 calendar days prior to work. The contractor will provide a complete plan at least 10 days prior to the beginning of any demolition of construction for VA approval. The VA will have 5 days to review and approve the plan The contractor shall have 30 days from the beginning of construction to complete the project. Work Hours Maintaining the operational capability of the medical center is of paramount importance. The A/E shall coordinate all activities with the COR. Work will be allowed to take place during normal business hours. Normal business hours for the medical center are defined as Monday through Friday, 0700 1630, excluding Saturday and Sunday. Designated off-shift hours are defined as 1630 0700, Monday through Friday, excluding federal holidays. Contracting Officer Representative (COR): A COR shall be designated by the Chief Engineer. The Contracting Officer (CO) shall delegate limited authority to the COR to generally direct the work of the contract, including but not limited to, work schedules, an inspection of services, acceptance of services, and certification of payments. The COR shall coordinate access to required areas in a timely manner and report to the CO problems resolving performance issues or other problems encountered. The Contractor shall notify the COR in a timely manner of any area(s) that they are unable to access, and any other problems encountered that prevent the satisfactory performance of any part of the work specified herein. The Contractor shall be responsible for damages caused by its personnel and shall notify the COR promptly of any damages to government property. Deliverables Contractor shall provide all technical literature and/or descriptions of all materials, equipment and tools proposed for used by the Contractor and shall be submitted to the Contracting Officer Representative (COR) or employee designee for approval prior to contract start and whenever a change is requested. Contractor shall provide a proposed daily time schedule indicating where and when they will be performing the service. The daily time schedule shall be submitted prior to commencing any work. The Contractor shall submit a daily progress report to the COR at the completion of the daily routine. Richard L. Roudebush VA Medical Additional Requirements Project number and the contract number shall be in the subject line of all correspondence. Comply with all Richard L. Roudebush VA Medical site-specific requirements and SOPs including and not limited to: Key Policy, Badging Policy, Rules of Behavior, and Smoking Policy. Parking is available in the F-lot. Only one (1) parking space will be available for the project, with an additional park located at the CSR Campus. Additional Information The C&A requirements do not apply, and a Security Accreditation Package is not required NOTE that current As-Built Drawings may not represent actual and current existing Conditions and shall not be relied upon for this study.
