Description
INTENT TO SOLE SOURCE Cable Television Services VA Montana Health Care System Fort Harrison MT 59636 IMPORTANTR NOTE: At this time, due to our current Market Research, it is the Government s intent to soles source this requirement (see below for specifics) for Cable TV Services for the VA Montana Health Care System at Fort Harrison MT 59636. Due to the historical nature of this facility satellites are not allowed. We the Government understand that the only Cable Television provider in this area is Charter Communications Operating, LLC. (also known as Spectrum Enterprise) and intend to sole source a Base Plus Four Option year contract to begin 1 July 2022. If your company can provide Cable Television Services as per this brief scope of work please communicate via e-mail to Lance.Davis2@va.gov by 2:00 PM (CST) June 16, 2022, and provide: Please state your company s business size as related to NAICS code 517311. As per the Limitations of Subcontracting: (see FAR 19.505): As Primary Contractor: what % of Total Contract Value will you retain? If using a Subcontractor: what % of Total Contract Value will go to them? Please provide SAM.Gov Unique Entity ID (UEID) number. General Information: The Montana VA Health Care System (MTVAHCS), 3687 Veterans Drive, Fort Harrison, Montana has a requirement for patient & family Cable TV access while in-patient at the VA Medical Center, Fort Harrison MT. 59636. This service must be consistent with a primary mission of the Department of Veterans Affairs to provide the highest level of customer service and satisfaction. The availability of Cable TV access will allow Veterans to maintain the same lifestyle as they would outside the facility. This will also help them to occupy their time during their stay and take their mind off their current situation and thus, ease any additional stress. Place of Performance: Contract shall furnish cable TV services at the following location and Buildings: Montana VA Health Care System VA Medical Center, Buildings 154 & 154A, Behavioral Health Unit (BHU) & Liberty House 3687 Veterans Drive Fort Harrison, MT 59636 Scope of Required Services: This Cable TV service will operate as a stand-alone system with no cross over to the medical center systems. Fort Harrison has a need for Cable TV service on 60 TV s located at Fort Harrison. There will be no opportunity for any breach of confidential information or access to medical records from any wireless network system. The system will work much like the access now provided when a guest has access at a hotel. All TV locations will be provided a remote control. Office of Information & Technology (OI+T) and Biomed will have no responsibility for troubleshooting or day to day operation of this TV service. However, OI&T will sign a Memorandum of Understanding with the Vendor to allow access to areas where TV equipment is located for technical trouble shooting, replacement, and/or repair of equipment. The remote controls will be left with each TV cable box, and available to the users via access to locations with TVs and/or by designated VA Medical Center personnel within the applicable services. The Cable TV service provided will include a variety of: local channels, sports channels, news channels and family viewing channels. Contractor Responsibilities: The system must meet the minimum specifications: The Vendor must conduct a functionality survey before proceeding with any installation. The contactor guarantees all equipment covered in this contract shall be in optimum working condition. Any equipment changes, required updates, retrofits, or new equipment required will be the responsibility of the vendor. If new equipment is suggested to the facility, it will be the decision of the Government as to the agreement to pay for additional cost of recommended equipment. It is the intent of this contract that cost of services remain as per identified line items at time of contract Award. Any cost or associated adjustments of the contract must be completed through a Supplemental agreement contract modification. The Government reserves the right that if additional equipment or cost are not agreed to, the contract will continue as originally awarded. The Montana Veteran Affairs Health Care System will not furnish part and/or test equipment for the performance of this contract. It is the responsibility of the contractor to provide the appropriate equipment and/or supplies necessary to complete the work as required within. The vendor will maintain and troubleshoot all issues regarding the equipment associated with the Cable TV service. The Vendor will repair equipment as needed to ensure the Cable TV service is operable 24/7 for patient and patient family use. Current equipment cannot be removed unless it is replaced with new equipment that has the same or greater performance qualities of service for patients. The hardware will not be connected to the VA Network or other Secure Networks. There will be no privacy or personal identifiable information shared. 24/7 technical support by Vendor to Biomed/user is required. A phone number must be provided to customers/users. Fort Harrison VAMC Biomed/COR (Contracting Officer Representative) will oversee the communication between the facility and the vendor. All request for additional work or anything not already agreed upon within the awarded contract must be accomplished through contract modification. The Government will not pay for services requested by someone other than the COR or the Contracting Officer that was not already agreed upon by contract modification. Vendor will provide installation of all hardware and cabling and install additional equipment, if replacement is needed, to provide current level of TV service, approval must be obtained for any repair/replacement by the Contracting Officer and COR. Cable is currently installed in the facility. We the VA understand that a fiber pull is required from the front gate of Fort Harrison VAMC to the building to provide service and upgrade our system in order to keep it running optimally. Satellite dishes cannot be mounted, and cabling cannot be run on the exteriors of the building/facility due to hospital being an identified Historical Site . If replacement is needed, the hardware (including cable, junction connections, and other equipment/items) will be the property of FHM VAMC and will remain onsite at the end of the contract. Hardware will be maintained by the vendor as part of the 24/7 technical support. If outages occur the Vendor will inform COR of action plan to fix/repair TV services within 24 hours if TV service is not made operable within 4 hours of incident (during normal working hours) or 12 hours of incident (during non-working hours).  The repair of the cable TV service must be completed within 3 days after the action plan is submitted.  Any day after 3 days must be an agreement between the COR and the Vendor.  The Vendor will credit the government/agency credit for each day the TV service is down after 3 days.  The Vendor will provide a toll-free line that will be available for the COR to report technical issues. Vendor will provide training on use of system to COR & facility POC. No training will be provided to patients or visitors. Vendor employees may need to access to data closets for repair/maintenance of hardware. Vendor is required to contact Biomed through the COR prior to their arrival to the Fort Harrison VA Medical Center. Vendor may be required escort by Biomed/OI&T personnel while on VA premises. The Vendor will provide a point of contact for Fort Harrison VA Medical Center designated personnel/COR. LIMITATIONS OF SUBCONTRACTING: The Limitations of Subcontracting may or may not apply to a resulting solicitation/contract from this notice. Vendors are advised to familiarize themselves with Federal Acquisition Regulation parts 19.505(a)(2) and 19.505(b)(1) as it pertains to subcontracting limitations. Vendors are also advised to familiarize themselves with 13 CFR 125.6 and the penalties described in this regulation.
