Description
STATEMENT OF WORK Neurology EEG Upgrade VA Central California Health Are Systems Neurofax PC upgrades, Persyst 14 and on-site Installation, Clinical Configuration &It support must be compatible with Nihon Kohden. Training will also need to be provided for the new upgrade. EMG3x Neurofax EEG-1200 Instruments and 1x Reader Station are past lifespan and using the unsupported Windows 7 operating system. The new computers connect to the existing instrumentation purchased by the Department of Neurology in 2015 and 2017 for the purposes of collecting brain wave data. Several analysis software features that are co-dependent on operating systems are also upgraded and the proposal includes fees for deployment from an IT, installation, and clinical perspective. OBJECTIVE: VA Central California Health Care Systems (VACCHCS) Neurology department seeks to upgrade its EEG and Persyst system. BACKGROUND: As part of mitigating cyber security risks, Healthcare Technology Management (HTM) and the Office of Information Technology (OIT) require any system that uses a high-risk Operating System (OS) such as Windows 7 be removed from the Veterans Health Administration domain. The current fleet of 4 EEG systems are currently running the unsupported Windows 7 OS and needs to be replaced with a supported OS such as Windows 10. Persyst 14 is an application that assist neurologists to complete their readings on the EEG workstations. The Persyst 14 licenses had expired and need to be upgraded for 3 EEG workstations and 1 EEG reviewing station. ACRONYMS AND DEFINITIONS: CO Contracting Officer: Person with the authority to request, approve, and authorize any changes on the contract. Contractor: The person or company that provides materials or services based on this Statement of Work. COR Contracting Officer Representative: Individual that facilitate proper development of requirements and assist the Contracting Officer in developing and managing the contract. Equipment Entry #: VACCHCS equipment asset unique identification number. HTM Healthcare Technology Management: the field responsible for managing the selection, maintenance, and safe use of all health technology and medical equipment found in healthcare. OIT Office of Information Technology: the department that is charged with establishing, monitoring, and maintaining information technology systems and services. OS Operating System: The system software that manages computer hardware, software resources and provides services needed to operate other computer programs. PM Preventative Maintenance: Planned routine inspection and maintenance of equipment. VACCHCS Veterans Affairs Central California Health Care System: The customer requesting services. LIST OF ITEMS TO BE PROCURED: The following Table list the equipment and items that are to be purchased. License sold per PC to be remotely accessed. Remote Configuration may be substituted to meet schedule requirements 1 #NEU-CFG-4T1610 Includes travel time, configuration, and training for 2-4 acquisition systems. Table 3 On-site Installation, Clinical Configuration & IT Support REQUIREMENTS: Contractor shall provide the professional installation of the 3 NK-1200-PCPR EEG systems and 1 1200R-PC/ZA for the upgrade/replacement upon the delivery of the system to the requesting customer facility. Contactor shall provide and install the upgraded 3 A/CPA-EXP-1001 and 1 A/AES-EXP-1000 Persyst 14 license. Contractor shall provide necessary equipment and tools required to complete the installation, replacement, or upgrade as described above. Contractor is to provide training for 2-4 acquisition systems. All travel, parts, and labor necessary to complete the above requirement is included in this contract package. SPECIFICATIONS AND STANDARDS: Identification, Parking, Smoking, and VA Regulations: The contractor will need to acquire a badge for each worker and ensure that the badge be always worn in a visible location on the worker while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police. The VA will not invalidate or make reimbursement for parking violations of the contractor under any conditions. Smoking is prohibited throughout the VA property including the parking lot and sidewalk. Possession of weapons is prohibited. Violations of VA regulations may result in a citation answerable in the United States District Court. Method and Source of Acceptance: All deliverables and services rendered under the contract are subject to inspection by the COR under the delegation of the CO. Should any deliverables not be to the satisfaction of the Customer, as submitted, the Contractor will be notified of the concerns and give the Contractor a chance to respond to and correct each issue. The Contractor shall respond within 48 business hours upon receiving the concerns. Should a response not be received, or corrective actions were not taken in a timely manner, the Customer shall have the right to reject it or require correction before payment is authorized. Property Damage: The contractor shall take all necessary precautions to prevent damage to any government property and will notify the COR and CO immediately if damages occur. The CO will authorize the contractor to remedy the situation in one of the following ways: be assessed current replacement costs for damaged property, replace damaged property in a timely manner, or correct the damages with like materials at no additional cost. Infection Control: There is a potential for exposure to blood borne or other infectious material with equipment throughout the hospital. All contractor personnel are cautioned and must use ""Universal Precautions"" (i.e., hand washing, wearing protective gloves, aprons, and goggles, etc.) as appropriate during the performance of this contract. Location: The services provided shall be completed at the VA Central California Health Care Systems facility located at 2615 East Clinton Ave, Fresno, CA 93703. A place for the installation/replacement/upgrade to complete shall be coordinated with the requesting customer.
