- Amendment #3 to this combine synopsis/solicitation it to provide clarification to CLIN X004 for quoting purposes. CLIN X004 is for Over and Above work discovered throughout the life of the contract. This CLIN has a Not To Exceed (NTE) value of $5,000.00, which is a plug number provided by the Government that shall be used by offerors within their quotes. When quoting, please ensure all Over and Above CLINs are stated as quantity 5,000 and unit price $1.00 to ensure we�receive�an accurate contract value. Version II of the Performance Work Statement� (PWS) is provided in the attachments section of the Combine Synopsis/Solicitation, highlighting this Over and Above information in yellow on page 19. No other sections of the PWS were changed apart from correcting page 1 to read ""Performance Work Statement"" vs. ""Statement of Work.""� Offerors are permitted to submit 1 corrected quote to reflect the Over & Above information provided above as needed. Only one corrected quote will be allowed, and the due date for submission of quotes remains unchanged with this Amendment #3. Amendment #2 to this Combined Synopsis/Solicitation is to provide the completed Questions from offerors and the Governments responses to those questions.� The Q&A document is provided in the attachments section of this Combined Synopsis/Solicitation. No other changes were made apart from posting the document and this note.� Amendment #1 to this combine synopsis/solicitation it to provide the ""2022 EAFB Installation Pest Management Plan Final"" document, to annotate that a Site Visit at AF Plant 42 will take place, and provide direction on submission of questions. Please see the attachments section of this posting for the Installation Pest Management Plan, and see added section ""Amendment #1 Additional Information""�below for information on the Site Visit & question submissions. ALL changes made with this Amendment #1 are annotated herein with Bold, Underlined and Italicized font.� This is a Combined Synopsis/Solicitation for the procurement to perform weed control and weed abatement services at Air Force Plant 42 (AFP 42) in accordance with the attached Performance Work Statement (PWS). (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Simplified Acquisition Procedures (FAR subpart 13.5) will be used for this requirement. (ii) Solicitation number: FA9301-22-Q-0016� *Please provide the full solicitation number on all packages* This solicitation is issued as a Request for Quote (RFQ). Interested parties who believe they can meet all the requirements for the items described in this synopsis are invited to submit, in writing, a complete quote. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2022-06, DPN 20220318, AFAC 2022-0502. (iv) This solicitation is provided for a small business set-aside. The North American Industry Classification System (NAICS) number for this acquisition is 115112 with a size standard of $8,000,000. (v) Period of Performance (PoP): estimated to begin 1 July 2022 and end 30 September 2026. The CLIN structure is located in attachment �Solicitation � FA930122Q0016"" (vi) �Air Force Plant 42 Weed and Abatement services. (vii) n/a (viii) The provision at FAR 52.212-1, Instructions to Offerors � Commercial Items (Oct 2018), applies to this acquisition. Provisions and clauses can be viewed via internet at Acqusition.gov, https://www.acquisition.gov/ Offerors shall prepare their quotations in accordance with FAR 52.212-1. In addition, the following information shall be included: PROVIDE THE FOLLOWING CONTRACTOR INFORMATION WITHIN YOUR QUOTE: The solicitation number; The time specified in the solicitation for receipt of offers; The name, address, and telephone number of the offeror; DUNS Number: CAGE Code: A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents if necessary; Terms of any express warranty Price and discount terms �Remit to� address, if different than mailing address; A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically; Acknowledgement of solicitation amendments; If the offer is not submitted on the�SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. �(ix) The provision at FAR 52.212-2 Evaluation � Commercial Items (Oct 2014) applies to this acquisition. ADDENDUM to FAR 52.212-2, Evaluation � Commercial Items (OCT 2014) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) Price [The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s)]. (ii) Technical capability of the time offered to meet the Government requirement; (Your offer package MUST clearly show and demonstrate your company�s ability to meet the stated requirements outlined in the Statement of Work. Be advised at the time of your offer you must demonstrate your pest control manager is�State of California certified and licensed.� Failure to provide enough information will result in a determination of offer being technically unacceptable to the solicitation.) This will be awarded to the Lowest Price Technically Acceptable Offeror. In order to be found technically acceptable the quote must demonstrate the contractor�s ability to provide the product and related service as outlined in the Statement of Work attachment. In addition, the Program Manager will evaluate quotes to determine technical acceptability. It is a pass/fail basis. If a quote fails, it will be determined to be not technically acceptable. Quotes will be reviewed in sequence with the lowest priced offer being evaluated first. All offers will be evaluated on their proposed Total Price. If the initial lowest price is found to be technically acceptable, award will be made to that vendor. Only one award will be made under this solicitation. (x) Offerors shall include a completed copy of the provision at FAR 52.212-3, Alt I, Offeror Representations and Certifications �Commercial Items (Oct 2018), with its offer. Completion of FAR 52.212-3 electronically at sam.gov is acceptable. (xi) The clause at FAR 52.212-4, Contract Terms and Conditions � Commercial Items (OCT 2018), applies to this acquisition.
