Description
STATEMENT OF WORK FOR TREE TRIMMING AND REMOVAL SERVICES 1. DESCRIPTION OF SERVICES.� The contractor shall provide all management, tools, equipment and labor necessary to perform on-call tree trimming and removal services at Naval Support Activity Monterey, in a manner that will provide satisfactory roadway clearances, building clearances, response to fallen trees and branches, removal of dead and living trees and� stumps, and trim trees to maintain their health visually attractive. BASIC TREE TRIMMING AND REMOVAL SERVICES.� The contractor shall accomplish all tree trimming and removal tasks to meet the requirements of this SOW. CREW WORK.� Work shall include all removal, trimming, pruning, stump grinding and removal, grubbing, brush removal and clean up.� Work to be performed per IAW ANSI Z133.2012, all ISA guidelines, and the accepted Safety Plan. Work areas to be left safe and clean.� All debris to be picked up and disposed of on government property. CRANE WITH OPERATOR.� Contractor shall provide crane with operator will be used in conjunction with crew work for large removal of trees.� Crane rigging and operator must be approved by the Government�s crane inspector prior to operation. ARBORIST CONSULTATION. �Contractor shall provide a certified arborist for tree consultation and inspections.� The arborist shall inspect trees for diseases, pests, structure, health, safety, or other tree related issues.� Contractor will respond in a timely manner and submit a concise report. EMERGENCY SERVICE CALLS.� Contractor is available to respond to emergency tree service calls twenty-four hours a day, seven days a week.� The contractor will be on-site and assess the emergency situation within sixty (60) minutes emergency request/notification.� Emergency tree crew will respond within 2-hours.� The tree emergency will be completed when the tree hazards have been arrested.� Site will be left clean and safe. 2. SERVICE DELIVERY SUMMARY. The contractor service requirements are summarized into performance objectives that relate directly to mission essential items.� The performance threshold briefly describes the minimum acceptable levels of service required for each requirement.� These thresholds are critical to mission success. Performance Objective SOW Para Performance Threshold BASIC TREE TRIMMING AND REMOVAL SERVICES.� Work shall include all tree removal by tree crew and cranes, including trimming, pruning, stump grinding and removal, grubbing, brush removal and clean up. 1.1 Zero worker injuries.� Trees clear of roadways and buildings.� Removal of designated trees and/or limbs.� Contractor respond to emergency service requests within required time limits.� 3. GOVERNMENT FURNISHED PROPERTY AND SERVICES.� There are no government-provided facilities, equipment, utilities, services, and materials for this contract. 4. GENERAL INFORMATION.� 4.1. QUALITY CONTROL.� The contractor shall develop and maintain a quality program to ensure tree trimming and removal services are performed in accordance with commonly accepted commercial healthcare practices.� The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. 4.2. QUALITY ASSURANCE.� The government will periodically evaluate the contractor�s performance in accordance with the Quality Assurance Surveillance Plan. 4.3. GOVERNMENT REMEDIES.� The CO shall follow FAR 52.212.4, Contract Terms and Conditions-Commercial Items (May 1997), for contractor�s failure to perform satisfactory services or failure to correct non-conforming services. 4.4. HOURS OF OPERATION.� 0700 to 1600, Monday through Friday excluding Federal Holidays. 4.5. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE NATIONAL COMMAND AUTHORITY OR OVERSEAS COMBATANT COMMANDER.� Basic tree trimming and removal (1.1.), shall continue as normal unless the base has been closed.� Notification of base closure will be made within 24 hours of the base closure.� Notification of resumption of services will occur no less than 24 hours prior to resumption. 4.6. ACCEPTANCE CRITERIA.� The Performance Assessment Representative shall inspect the finished result of tree trimming work. 4.7. PERIOD OF PERFORMANCE.� This contract is for one base year and four optional years.� Start of contract will be 01 August 2022. 5.0. Appendices. ����������� A.
