Description: ___________________________________________________ Amendment 1: Adding attachment ""SCA Wage Determination 2015-4095 (v21)"".___________________________________________________ �(i) This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number FA660622Q0008, is hereby issued as a Request for Quote (RFQ) for the drilling of a water well at Westover Air Reserve Base. A contract will be issued without discussions as a Cost Reimbursement award utilizing FAR Part 13, Simplified Acquisition Procedures.� � (iii) This solicitation document incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2022-06, Defense Federal Acquisition Regulation Supplement (DFARS) Publication Notice 20220526, and AFAC 2022-0502. It is the contractor's responsibility to become familiar with applicable provisions and clauses by visiting https://www.acquisition.gov/. (iv) This solicitation is a 100% small business set aside and is restricted to small businesses only in accordance with FAR 19.502-2(b). The North American Industry Classification System Code (NAICS) is 237110 � Drilling water wells, Small Business Size Standard is $39,500,000.00. (v) � �Price list: items, quantities, and units of measure: �� CLIN 0001 MOBILIZATION, SITE SUPERVISION, FLOW TEST, GRADING, 1 Job �� CLIN 0002 DRILLING, 6"" WELL ENCASED IN PIPE TO BEDROCK WITH SANITARY SEAL, Price per Linear Foot� �� CLIN 0003 DRILLING, 6"" WELL IN BEDROCK, Price per Linear Foot� The unknown variables of bedrock depth requires the vendor to provide a pricing of ""per linear foot"" to drill through sill and bedrock to a 450' depth. The government can only provide estimates on sill depth to reach bedrock. Current estimates are 60' - 100'. Evaluation will be based on vendor's price per foot. Invoicing will reflect actual depth drilled through sill earth and corresponding required piping and actual depth drilled through bedrock. *See attachment ""SF18 - REQUEST FOR QUOTATION""* (vi) Description of Requirement: The vendor will provide well drilling and flow testing services as outlined in the attachment �PWS Water Well� dated 03 June 2022. (vii) Date(s) and place(s) of delivery and acceptance and FOB point. Acceptance is FOB destination: 439 BCE, 20 Patriot Avenue, Westover ARB, Chicopee, MA, 01022-1507 (viii) Provision 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition.� Quotes must include the following information with their quote: Pricing, Company Name & Address, Point of Contract, Telephone, Email, Cage Code, UEI number, and Business Size. If Vendors have not self-certified their status at SAM.gov, they MUST certify provision 52.204-24 and 52.204-26 by completing the attached PDF titled, �Provisions and Clauses� and submitting it with their quotes.� Vendor Solicitation amendments, if any, need to be acknowledged. (ix) Provision 52.212-2, Evaluation-Commercial Items, applies to this acquisition. See attachment �Provisions and Clauses� for full text. The following factors are to be used to evaluate offers: Lowest Price; Technically Acceptable (LPTA).� Technical acceptability - proposals will be evaluated on an acceptable or unacceptable basis. To receive an acceptable rating, vendors MUST provide with their proposal proof of Well Driller Certification for Massachusetts. Proposals submitted without this information will be considered nonresponsive and will not be considered for award.� Price - Evaluated price will reflect the total pricing of items 0001 thru 0003. Item 0001 is a Fixed-Price item and shall reflect the total cost to mobilize, supervise, grade, and provide a flow test. Items 0002 and 0003 should reflect vendor's pricing per linear foot of specific type of drilling. Technical and price, when combined, are equal. (x) If Vendors have not self-certified their status at SAM.gov, they must certify provision 52.212-3, Offeror Representations and Certifications-Commercial Items, by filling in the appropriate fields in attached PDF titled, �Provisions and Clauses� and submitting with your quote. (xi) Clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition.� (xii) Clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition. � (xiii) IAW FAR 52.204-16, prospective awardees shall have an active UEI number prior to award of a government contract. To register for a UEI number and to complete Online Representations and Certifications, go to the System for Award Management (SAM) at https://www.sam.gov. All vendors interested and capable of obtaining contract award must be registered with the Wide Area Work Flow (WAWF) located at https://piee.eb.mil. Vendors are required to create and submit invoices electronically through WAWF and receive payment via electronic funds transfer (EFT) for supplies or services rendered. (xiv) Defense Priorities and Allocations System (DPAS) and assigned rating is not applicable to this acquisition.
