VA HANDBOOK 6500.6 MARCH 12, 2010 APPENDIX C C-12 Pre-solicitation Notice of Intent to Sole Source Pre-Solicitation Notice of intent to award a sole source contract.  This is not a solicitation. The Department of Veterans Affairs intends to negotiate a sole source contract with:    DGA Medical 419 W Aire Libre Avenue Phoenix, AZ 85023-7922 Unique Entity ID (SAM): GZT6NJ8HJLC3 The Department of Veterans Affairs VA Medical Center intends to award a purchase order, Firm Fixed Price, Sole Source contract for 1 (one) Captus 4000e Thyroid Uptake System with 1"" Well Counter to DGA Medical, SDVOSB. The intent is to procure these services on a sole source basis.   The NAICS 334516.
