This is a SOURCES SOUGHT NOTICE only. This is not a request for formal proposals or quotes. No formal solicitation document exists at this time. This request is for planning purposes only and shall not be considered as an invitation for bid, request for quotation, request for proposal, or as an obligation on the part of the Government to acquire any products or services. Your response to this notice will be treated as information only. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of contractor submission of responses to this announcement or the Government use of such information. The NAICS Code is 562112 HAZARDOUS WASTE COLLECTION PURPOSE AND OBJECTIVES: The intent of this Sources Sought Notice is to identify potential offerors capable of providing as needed Collection, Documentation and Destruction of Controlled Substances for VHA VISN 2 Pharmacies. The proposed contract action is expected to be a Firm-Fixed Price BPA Contract with an anticipated period of performance of 09-01-2022 to 08-31-2027. STATEMENT OF WORK SUMMARY: Pharmacy departments across VISN 2 (New York/New Jersey VA Health Care Network) require a contractor services from a DEA licensed controlled substance destruction vendor to collect and destroy unusable controlled substance medications via incineration as per DEA requirements to include the required documentation and records. Unusable controlled substance medications are typically accumulated through expiration in original (opened or unopened) containers, unused repackaged medications, and damaged medications in non-original containers. The minimal requirement for the service is a quarterly on-site pickup, but a more frequent interval and as needed services may be required by some locations. Additionally, some locations have patient return bins for unwanted or no longer needed controlled substances which may require service. Place of performance: 1) WNY (Buffalo) and Batavia (2 pharmacies) a. Monthly pickup, Buffalo + MedSafe Return Bin b. Quarterly pickup, Batavia + MedSafe Return Bin 2) Finger Lakes (Canandaigua), (Bath) and Rochester Outpatient Clinic (3 pharmacies) a. Quarterly pickup all locations 3) Syracuse (1 pharmacy) a. Monthly + MedSafe Return Bin 4) Albany (1 pharmacy) a. Every other month 5) New Jersey (East Orange), (Lyons) and (Brick) (3 pharmacies) a. Quarterly, All locations 6) JJ Peters -Bronx (1 pharmacy) a. Quarterly 7) NY Harbor ( Manhattan), (Brooklyn) and (St Albans) (3 pharmacies) a. Every other Month + MedSafe Return Bin (Monthly) 8) Northport (1 Pharmacy) a. Quarterly RESPONSE: Responses to this Sources Sought Notice should demonstrate the firm's experience in delivering the service listed above, as well as capability of performing these services in the areas listed above. Responses should include the following information: Business name, address, SAM Unique Entity ID, business size and Socioeconomic Category (HUBZone, 8(a), Service-Disabled Veteran-Owned Small Business, Veteran-Owned Small Business, Small Business, Large Business, etc.), FSS Contract # if applicable, and point of contact to include e-mail address and telephone number. Offerors will be required to be registered in the System for Award Management (www.sam.com) prior to any future offer. Any interested capable parties shall submit their response by email prior to the response date and shall include the information requested herein.
