Indian Health Service (IHS), Phoenix Area Office is conducting this Sources Sought to identify Native American or Indian Owned businesses who could provide a tool with the capability to administer after all patient visits via online survey within 48 hours of the patient visit. The designated NAICS code is 541611 Administrative Management and General Management Consulting Services, with a size standard of $16.5M. �Responses shall reference this Sources Sought announcement number, �75H71222Q00025 Sources Sought - Elko Patient Experience Survey Tool. �� This is not a Request for Proposal (RFP) or Request for Quotation (RFQ). Indian Health Service is utilizing this tool as part of market research.
