Description
STATEMENT of WORK: PROVISION AND MAINTENANCE OF ISP SERVICES FOR STVHCS CBOC WIFI SERVICE SCOPE: The Department of Veterans Affairs South Texas VA Medical Center Audie L. Murphy has a requirement for Patient Wi-Fi equipment and services for various facilities located within the South Texas Region. The Contractor shall provide all service, tools, equipment, personnel, transportation, and lodging necessary to provide and maintain internet service provider (ISP) services and installation of Demarcation for the WiFi system that will be in use at the South Texas Veterans Health Care System (STVHCS) Community Based Outpatient Clinics (CBOC) locations listed below. The Contractor will provide installation of Fiber or copper DEMARC, monthly Internet services and any applicable modem / router to connect VA provided equipment to the internet as defined in the Scope of Work (SOW). The Internet Service Provider (ISP) will also need to be completely separated from the OIT VA Network to ensure safety and security of the OIT and Guest Network. The period of performance for this agreement shall begin as soon as this contract is awarded and is expected to last as a base plus four years. This will be a base plus 4 option year contract. Please ensure you quote the option years. Southcross Blvd., San Antonio, TX 78222 Suite 100 Victoria OPC: 1908 North Laurent St., Victoria, TX 77901 Suite 150 SCOPE OF WORK/SPECIFICATIONS: DESCRIPTION OF SERVICES: Contractor will provide the following: All materials, tools, and equipment necessary to provide internet services for the Guest WiFi network at the CBOC locations. The Contractor will provide monthly internet services and any applicable modem/router to connect VA-provided equipment to the internet as defined in the Scope of Work (SOW). The Internet Service Provider (ISP) will also need to be completely separated from the OIT VA Network to ensure safety and security of the OIT and Guest Network. SERVICES The Contractor shall provide: All equipment and materials must be new, not refurbished, not rebuilt, nor 3rd party substitutes are acceptable. Provide all materials, tools, and labor for total project scope. Provide 5 static IP addresses. Provide Dedicated Business Class Fiber Internet Access @ minimum 500 Mbps (to cover an average of 5000 8000 clients at any time) Internet hand-off: Fiber or copper Contractor shall provide Modem / Router equipment that will connect to VA-provided Firewall and associated equipment to the Internet. Contractor will configure Contractor-Provided Modem / Router and make physical connections from Fiber Demarcation to Contractor provided Modem / Router. VA Electronics Staff will make physical connection from Modem / Router to VA provided Firewall. Above ceiling, Pipe Space and interstitial cabling will be suspended in new J-hooks through-out the facility Suspension points should be adequately spaced so that cabling is neat, tight, and does not droop more than 3 inches at any point No cabling will lie directly on top of ceiling tiles or light fixtures at any point No cabling will be draped over or wrapped around any existing piping Zip-ties or other pressure ties will not be used all cabling shall be bundled with Velcro (type or brand) straps only Provide installation of all cabling and fiber components required herein Provide trash removal and clean-up of all debris through-out project duration Provide project management, oversight, planning, and technical support throughout the upgrade process. Dedicated Internet Access Requirements Service Availability: 99.99% (Outside of ISP scheduled maintenance or repairs) Average Time to Restore Service: Within 4 hours of outage regardless of the time of outage. Latency: 45ms Jitter: < 30ms Packet/Frame Loss: < 1% 500 Mbps minimum speed, 5 Static IP Addresses, Fiber or copper Demarcation (Tested and Certified) and Modem / Router provided by ISP. All security Appliances must be brand specific Cisco Meraki. All licenses must be a minimum of 3-years duration. Patch cables must be Cat6 or better. Fiber / Copper Pathway Underground from edge of VA Properties or VA leased properties to inside the buildings via interstitial spaces, pipe spaces and above ceiling tiles when necessary. Follow all specific VA requirements as detailed below for earthwork, cabling, construction, etc. Contractor will work with VA Engineering Design Team/ Leasing section and VA leased Building Owners to determine the best and safest route for underground fiber / copper installation from VA Property Line to Facility for demarcation Troubleshooting and Repair Any problems with the system must be isolated and repaired by the Contractor. The Contractor will troubleshoot the Contractor provided modem / router, software or Fiber from VA property line to demarcation to determine the best method of repair. If problems are found, the Contractor shall contact the appropriate and authorized maintenance organization or company and track the trouble call until it is resolved. Contractor Responsibilities During the term of this contract, the Contractor will provide support at the VA Electronics Shop Customer request and shall use its best efforts to provide prompt software & Contractor provided equipment support services to the Customer by telephone. The Contractor will provide telephone support to VA Customers 24 hours / 365 days a year. The Contractor will use its best efforts to respond promptly to all inquiries via E-Mail or phone by the customer within one (1) hour of notification that service is required. Reliability The Contractor shall be responsible for maintaining the operation of the contractor provided hardware e.g. modem / router and all fiber infrastructure to DEMARC. The Contractor shall provide service availability with an expected 99.9% uptime outside of ISP maintenance and repairs 7 days a week 365 days a year. The Contractor shall provide enough bandwidth to provide internet service that meets facility usage requirements while performing normal internet functions (e.g., streaming video, email, network searches). Maintenance and Support Maintenance and support will begin after the installation and testing is complete and accepted by the VA site. Maintenance and support will be provided throughout the option year periods, if exercised by the Department of Veterans Affairs. Maintenance and support will be provided throughout the period of performance. The Contractor shall provide 24 hours / 365-day user support through a call center. The Internet access shall be available to the facility on a 24x7x365 except during periods of ISP scheduled maintenance when and if necessary. The Contractor shall provide the VA the means to send failed items back i.e. modem / router to the Contractor for replacement. ISP determined maintenance shall be scheduled during off peak hours of 2200hrs 0400hrs seven days a week and 365 days of the year The Contractor shall provide 24 hours/365-day on-call technical support through a call center. The Contractor shall provide on-site support if deemed necessary by VA staff or the Internet Service Provider. The Contractor shall respond to VA Electronics staff via E-Mail or Phone for trouble tickets entered through the technical support call center. The Contractor shall physically respond onsite within 24 hours for issues that require onsite support determined by VA Staff or the Contractor. Contractor shall notify the COR at a minimum two hours prior to any onsite scheduled or repair work. There shall be no additional charge for time spent at the site (during or after the normal hours of coverage) awaiting the arrival of additional FSE and/or delivery of parts. GENERAL INFORMATION Hours of Operation Normal hours of operation are Monday through Friday from 8:00 am to 5:00 pm CST, excluding holidays. All services shall be performed during normal hours of coverage unless requested and approved by the COR. Federal and National Holidays observed by the VAMC are: New Year's Day, Labor Day, Memorial Day, Thanksgiving Day, Christmas Day, Martin Luther King s Birthday, Columbus Day, President s Day, Veterans Day, and Independence Day. Also included would be any other day specifically declared by the President of the United States to be a National Holiday. When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by U.S. Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a legal holiday. COMPLIANCE WITH SECURITY REQUIREMENTS Identification: All contractors working shall be fingerprinted prior to being allowed to work on campus. Contractor ID badges can be obtained from PIV office typically 3-5 days after approved SAC form is received and fingerprints clear. ID badges are required for contractor employees working at Audie L. Murphy Medical Center. Contractor Personnel Vaccinations: Seasonal Influenza Vaccination/Tuberculosis Testing: VHA Directive 1192.01 establishes policy and provides guidance for the prevention of seasonal influenza in VHA facilities, through the vaccination of health care personnel (HCP). HCP are individuals who, during the influenza season, work in VHA locations or who come into contact with VA patients or other HCP as part of their duties. HCP include all VA contractors who are expected to perform any or all of their work at VA facilities. Contracting Officers shall ensure the flu vaccination requirements outlined in this directive are expected in all HCP contracts. VA contracting officers are responsible for on-site HCP contract and will inform the contractors about this directive and its requirements. Tuberculosis Testing: All Contractor personnel who will be working onsite must provide documentation of a completed tuberculosis (TB) skin test within the prior year before beginning work. Parking: It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Section. The VAMC will not validate or reimburse for parking violations from the Contractor under any conditions. Smoking, Weapons, and VA Regulations Smoking and vaping are prohibited inside any building at the VAMC and on the STVHCS campus (including parking areas). Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of the VA regulations may result in citation answerable to the United States (Federal) District Court, not a local district, State, or Municipal court. Vaccinations and Trainings All contractors must show proof of NEGATIVE TB test results, annual Flu vaccination, and any other requirements deemed necessary by Infection Prevention or the Secretary of the Departments of Veteran Affairs. All contractors must attend a mandatory Infectious Control Training held every Tuesday at 10 am at the Audie L. Murphy Memorial Hospital located at 7400 Merton Minter , San Antonio, Texas 78229, room E002. All contractors must present any additional information deemed necessary at the time of project by Infection Prevention. TECHNICAL REQUIREMENTS The Contractor shall deliver the hardware, software and services necessary to provide Internet access for ALM Guest WI-FI that meets the following requirements: The Contractor shall provide the modem / router necessary to connect to VA Firewall / WLC to the Internet. Contractor shall provide at least 5 static IP addresses. The Contractor shall adhere to the standards outlined by the National Fire Protection Association (NFPA-101 Life Safety Code). The Contractor shall work with Engineering Design Team to determine the best route for underground installation of Fiber. Contractor provided modem must be able connect to the following VA equipment: Firewall Model: CISCO # ASA5525-FPWR-K9 Wireless LAN Controller Model: CISCO AIR-CT5520-K9 SECURITY The contractor shall provide security for the vendor provided equipment. The Contractor shall provide for the physical security of Contractor equipment, as needed. VA shall provide the Contractor with escorted access to the wiring closets. Log files must be maintained for a one-year period. The Contractor shall adhere to all physical security and badge identification outlined by the VA Police while on site. DEFINITIONS/ACRONYMS Acceptance Signature VA employee who is authorized to sign-off on the ESR, which indicates that the PM has been concluded or is still pending completion, or that the Emergency Repair has been accomplished or is still in a pending status. Authorization Signature the COR s signature; indicates that the COR accepts work status as stated on the ESR. CO Contracting Officer COR Contracting Officer s Representative VAMC Department of Veterans Affairs Medical Center INFORMATION SYSTEMS OFFICE, INFORMATION PROTECTION The contractor will not have access to VA Desktop computers, nor will they have access to online resources belonging to the government while conducting services. If removal of equipment from the VA is required, any memory storage devices, such as hard drives, solid state drives and non-volatile memory units will remain in VA control and will not be removed from VA custody. PRIVACY OFFICER The contractor will not have access to protected Patient Health Information (PHI) nor will they have the capability of accessing patient information during the services provided to the VA and if removal of equipment from the VA is required, any memory storage devices, such as hard drives, solid state drives and non-volatile memory units will remain in VA control and will not be removed from VA custody. All research data available for Contractor analyses is de-identified. RECORDS MANAGER There will be no federal records created, maintained, used or dispositioned with this contract; regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.]. TYPE OF CONTRACT STVHCS anticipates award of a FIRM FIXED PRICE contract based on the contents of this solicitation. SECURITY REQUIREMENTS The C&A requirements do not apply and a Security Accreditation Package is not required.
