Description
1. This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. 1.A. Project Title: Medical Grade Power Strips 1.B. Description: The Department of Veterans Affairs (VA), VA Long Beach Healthcare System (VALBHS), has a requirement for has a requirement for Medical Grade Power strips. 1.C. Project Location: VA Long Beach Healthcare System, 1501 San Pedro Dr. Long Beach, CA 87108. 1.D. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26222Q1130 1.E. Set Aside: This solicitation is 100% set-aside for SDVOSBs. 1.F. Applicable NAICS code: 335999, Surgical & Medical Instrument Manufacturing. 1.G. Small Business Size Standard: 500 Employees 1.H. Type of Contract: Firm Fixed Price 1.I. Period of Performance: 90 days after receipt of order. DROP SHIP ONLY. 1.J. Wage Determination: N/A 1K. Key solicitation milestones are: 1.K.1. Submit written offers in accordance with the following instructions, as well as Addendum to FAR 52.212-1, Instruction to Offerors. Oral offers will not be accepted. Complete and submit enclosure 2 via email no later than 3:30pm PT Monday, July 11, 2022 to Jonathan.Ford1@va.gov. Also, see Enclosure 3 for a listing of salient characteristics. Ensure the following are completed and/or included in the offer packet: Offeror quotations must demonstrate meeting the salient physical, functional, or performance characteristic specified in this solicitation, otherwise it will be considered non-responsive. Ensure the following are completed and/or included in the quote packet: 1.K.2.A. Vendor Information 1.K.3.B. Acknowledgement of amendments 1.K.4.C. Concurrence/non-concurrence with solicitation terms, conditions, and provisions 2. Statement of Work. 2.A. Background: The Department of Veterans Affairs (VA), VA Long Beach Healthcare System (VALBHS), has a requirement for Medical Grade Power strips for The Long Beach VA Medical Center that meets the VHA medical specifications and high-quality threshold while integrating excellent environmental stewardship. 2.B. Scope. The purpose of this contract is to provide for the main hospital with Medical Grade Power strips for all processes across the hospital, to be used with medical equipment inside Patient Care Areas. The contractor will: 2.B.1. Provide the specified product that will meet or exceed the following specifications. 2.B.2. Only include products which are specified in the salient characteristics in Enclosure 3 2.B.3. Coordinate all products to order, ship, and deliver Medical Grade Power Strips from the manufacturer until final acceptance by the VA Point of Contact (POC). 2.B.4. Coordinate product delivery per instructions noted in Section 8: Delivery& Schedule. 2.B.5. Provide the VA POC with tracking information when it is available to vendor. Call to schedule delivery. One-week advance notice is needed to assure manpower is available to receive items. 2.B.6. Provide maintenance manuals for all products specified. 3.C. VENDOR RESPONSIBILITIES 3.C.1. All orders shall be delivered Free On-Board (FOB) destination to the delivery address listed. 3.C.2. The vendor shall contact VA POC minimum 14 days in advance to schedule delivery. 3.C.3. In the event there is a delay in delivery, Vendor must notify the VA POC immediately in writing via email. 4.E. OTHER REQUIRED DOCUMENTATION/EVALUATION CRITERIA 4.E.1. Complete manufacturer s product specifications 4.E.2. Warranty literature 5.F. PRODUCT 5.F.1. All products shall be free of scratches, mars, dents, or blemishes. 5.F.2. Power strips shall have smooth finishes with no hazardous projections, sharp corners, or detail, which can be hazardous and cause personal injury or damage to clothing. 5.F.3. See The Salient Characteristics in Enclosure 3 for technical requirement of products. 6.H. DELIVERY & SCHEDULE 6.H.1. All Medical Grade Strips shall be tagged with the contract number and purchase order (PO) number. 6.H.2. Trucks cannot be left unattended and must be removed from the dock or other-directed area after unloading. 6.H.3. Vendor shall provide the VA POC with scheduled date and time that the delivery truck will be arriving a minimum 24 hours in advance. 6.H.4. In the event there is a delay in delivery, the vendor shall notify the assigned VA POC immediately in writing via email. 6.H.5. Vendor shall ensure that the driver of the delivery vehicle has copies of the Bill Of Materials/ delivery orders/ Packing Slips and shall not attempt to deliver any items before the scheduled delivery date. 7.I. INSPECTION AND ACCEPTANCE 7.I.1. Vendor must provide a Bill of Materials with subtotals and tagged per CLIN. 7.I.2. Vendor must provide cut sheets with product detail for each line item marked on shipment. 7.I.3. Products will only be accepted if they are determined to be of acceptable quality and working order, as determined by the VA POC. 8.J. WARRANTY All manufacturers must offer a standard warranty that product is free from defects in materials and workmanship (Includes shipping, parts and labor for the repair or replacement of defective items.) Must meet MTR specifications, typically 10 years minimum. 9.K. Place of Performance: The items will be delivered to 149 receiving warehouse at the VALBHS. 10.L.
