- Description
- The Albuquerque Area Indian Health Service, Santa Fe Indian Health Center, has a requirement for ID Now Abbott Now Strep, Influenza and RSV Tests. This is a combined synopsis for commercial items or services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is 75H70722R00012 and is issued as a Request for Proposals (RFP), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-06. The associated North American Industrial Classification System (NAICS) code for this procurement is 541380 with a small business size standard of $16.5. This requirement is a Set-Aside for Small Business Indian Firms and only proposals from qualified offerors will be considered. If we do not receive enough competition and/or reasonably priced offers from Small Business Indian Firms set-aside, we will evaluate proposals and consider for award to other Small Business concerns. Any award resulting from this cascading set-aside will be from such a concern as follows: First Tier: Small Business Indian Firms; Second Tier: Small Business; Third Tier: Service Disabled Veteran-Owned Business; Fourth Tier: HubZone; Fifth Tier: All Other Large Business Concerns. Only one award will result from this solicitation. FOB Destination shall be for the following Albuquerque Area Indian Health Service facility: Santa Fe Indian Health Center (SFIHC) Attn: Nursing Department 1700 Cerrillos Road Santa Fe, New Mexico 87505 All responsible offerors that respond to this solicitation must submit their proposals by July 20, 2022, by COB 3:30 p.m., (MDT). The proposal can be sent by U.S. Postal Service or by Email to: patricia.trujillo@ihs.gov. Failure to comply with the below terms and conditions may result in offer being determined as non-responsive. Question Submission: Interest Offerors must submit any questions concerning this solicitation at the earliest time possible to enable to respond. Technical questions must be submitted by Email to: Kimberly Jones, kimberly.jones2@ihs.gov, five (5) days prior to closing of the solicitation. All Acquisition questions must be submitted by Email to: patricia.trujillo@ihs.gov five (5) days prior to the solicitation, July 13, 2022, by 3:30 p.m., (MDT). Questions not received within a reasonable time prior to close of the solicitation will not be considered.
