- NOTICE OF INTENT TO AWARD SOLE SOURCE Department of Veterans Affairs, VISN 05, 50 Irving Street, NW, Washington, DC VA Medical Center intends to award a sole source contract award to Medivators, Inc, 14605 28th Avenue North, Minneapolis, MN 55447. The period of performance will be from July 25, 2022 to July 24, 2023 with four (4) option years. THIS NOTICE IS NOT A REQUEST FOR COMPETITIVE QUOTES; however, any firm that believes it can meet the requirements may give written notification prior to the response due date and time. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to perform the requirements. Responses received will be evaluated; however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the Government will proceed with the award. Responses are due by July 14, 2022, 1:00pm (EST). Responses shall be submitted via e-mail to Charles.willis@va.gov. Telephone requests or inquires will not be accepted. STATEMENT OF WORK SCOPE OF WORK: The Contractor shall provide all labor, personnel, equipment, tools, materials, supervision and other items and services necessary to maintain the Medivator DSD- Edge endoscope reprocessors (see table below) at the Washington DC Veterans Affairs Medical Center, 50 Irving Street NW, Washington, DC 20422. The Contractor shall provide one annual PM visit, remedial repairs (both service and parts), and one annual operator training by a clinical specialist as part of this agreement. The Contractor shall provide services to the equipment in the table found below. The serial numbers correspond to the current inventory only. Serial numbers may change based on replacements made as part of this contract. The contractor shall provide the base year plus two option years for future service contracts. EQUIPMENT: The contractor shall provide all tools and equipment necessary to properly and safely perform the services set forth within this statement of work. This shall include, but is not limited, to all Personal Protective Equipment (PPE) which may be required for the work to be completed in a safe manner. 2. AREAS TO BE SERVICED: (one or all as necessary and appropriate to this contract) Washington DC VA Medical Center 50 Irving Street NW Washington, DC 20422 3. TASK FREQUENCY AND INSTRUCTIONS: Annual Preventive Maintenance One operator training provided by a Clinical Support Specialist, Remedial service (labor, travel and parts) if required in between Preventive Maintenance service calls Telephone technical support The Contractor shall be available from 8:00 am to 5:00 pm, Monday through Friday, excluding holidays, to provide services. The contractor shall be available for unlimited technical phone support from 6:00am to 6:00 pm (CST), Monday through Friday, excluding holidays. Each instance of service will be a single visit, be it a single day or multiple days, at the facility for a period sufficient to complete the work set forth in the scope of work. This visit is to be scheduled in advance with the Washington DC VA Medical Center s Biomedical Engineering Section. Service reports for any and all work done are required to be submitted to the Biomedical Engineering Section within three business days of completion of the work. Any and all hard drives used by the Department of Veterans Affairs shall remain in the possession of the VA. All mobile media used by the Contractor must be scanned by Biomedical Engineering Section prior to its use. 4. SPECIAL WORK REQUIREMENTS: None PROPERTY DAMAGE: The contractor shall take all necessary precautions to prevent damage to any government property. The contractor shall report any damages immediately and shall be assessed current replacement costs for property damaged by the contractor, unless corrective action is taken. Any damaged material (i.e., trees, shrubs, lawn/turf, cubs, gutters, sidewalks, etc.) will be replaced in a timely manner or corrected by the contractor with like materials, at no extra cost to the government, upon approval of the Contracting Officer. 6. ATTACHMENTS: None. 7. IDENTIFICATION, PARKING, SMOKING, CELLULAR PHONE USE AND VA REGULATIONS: The contractor's employees shall wear visible identification at all times while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate desig nated park ing areas. Information on parking is available from the VA Police-Security Service. The VA will not invalidate or make reimbursement for parking viola tions of the con tractor under any con ditions. Smoking is prohibited inside any buildings at the VA. Cellular phones and two way radios are not to be used within six feet of any medical equipment. Posses sion of weapons is prohibited. Enclosed containers, including tool kits, shall be sub ject to search. Violations of VA regulations may result in a citation answer able in the United States (Federal) Dis trict Court, not a local district state, or municipal court. 8. COMPLIANCE WITH OSHA BLOODBORNE PATHOGENS STANDARD: The contractor shall comply with the Federal OSHA Bloodborne Pathogens Standard. The contractor shall: A. Have methods by which all employees are educated as to risks associated with bloodborne pathogens. B. Have policies and procedures which reduce the risk of employee exposure to bloodborne pathogens. C. Have mechanisms for employee counseling and treatment following exposure to bloodborne pathogens. D. Provide appropriate personal protective equipment/clothing such as gloves, gowns, masks, protective eyewear, mouthpieces for the employee during performance of the contract.
