Description
SOURCES SOUGHT SYNOPSIS ONLY This request for information (RFI) and sources sought is issued solely for information and planning purposes only and does not constitute a solicitation. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. The submission of pricing, capabilities for planning purposes, and other market information is highly encouraged and allowed under this RFI in accordance with FAR 15.201(e). The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources, their socioeconomic status, and their size classification relative to NAICS 339116 Dental Laboratories, size standard 500 employees. Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. This synopsis is neither a request for competitive proposal nor a solicitation of offers. This synopsis is neither a request for competitive proposal nor a solicitation of offerors. This synopsis is to assist the VA in determining sources only and a solicitation is not currently available. If a solicitation is issued it shall be announced at a later date, and all interested parties must respond to that solicitation announcement separately from responses to this RFI and sources sought. The Department of Veterans Affairs Veterans Integrated Service Network (VISN) 22 is seeking potential sources capable of providing Dental Laboratory Services. The proposed action is for a fixed price, indefinite delivery, indefinite quantity (IDIQ) contract for a One (1) year base period of performance and Four (4) one-year option periods. Please see DRAFT EXCERPT from the Performance Work Statement (PWS) on page two. The Government contemplates issuing one (1) solicitation for these services to be provided to the following facilities and their external clinics: VA Long Beach Healthcare System 5901 E. 7th Street Long Beach, CA 90822 VA Loma Linda Healthcare System 11201 Benton Street Loma Linda, CA 92357 VA San Diego Healthcare System 3350 La Jolla Village Drive San Diego, CA 92161 Contractor shall provide the following information: Vendor: Address: Telephone Number: Business Size: Socioeconomic Category (i.e. SDVOSB, 8a, VOSB, etc.): Duns Number: Facility you have the capability to Support: Facility Can Support Cannot Support VA Long Beach VA San Diego VA Loma Linda Responses to this sources sought are not request to be added to a prospective bidders list or to receive a copy of the solicitation. Information received as a result of this notice will normally be considered solely for the purpose of determining whether conduct a competitive procurement. This notice does not represent a commitment by the Government to pay for cost incurred in the preparation and submission of information or any other cost incurred as a response to this announcement. PERFORMANCE WORK STATEMENT (PWS) (DRAFT EXCEPT) General Requirements. The contractor(s) shall provide all labor, facilities, transportation, and management necessary to provide fixed and removable, including assisted/implant supported dental prosthesis, for Veteran beneficiaries. The contractor shall be a full-service laboratory that can integrate the combined treatment needs of the prosthetic patient requiring simultaneous fixed, removable, and implant supported/assisted prostheses. Contractor(s) will be required to attend Veteran s appointment often on short notice based on urgency to provide dental supplies and or assistance with dental lab prosthesis. Pick-Up and Delivery. The contractor(s) shall be required to pick-up or arrange to have delivered to their place of business prescriptions/specifications, and impressions between the hours of 8:00am and 3:00pm PST Monday Friday excluding national holidays at no additional cost to the Government. Orders placed prior to 12:00pm PST shall be picked up the same day, while orders placed after 12:00pm PST shall be picked up the following business day. Supplies under this contract shall be delivered to the requesting facility and all pick-ups and deliveries must be made in person directly to the requesting Dental Service Department. Contractor(s) shall deliver all completed orders no later than fourteen (14) calendar days from the date the order is placed or an agreed upon date by both the contractor and the requesting Dental Service Department for non-urgent orders. Products and Services. The following items shall be fabricated in the manner identified below: All metal crowns and bridges shall be cast in noble or high noble metal as indicated by dentist. Porcelain fused to metal crowns/bridges shall be cast in high noble white metal. Occlusal/lingual surfaces may be porcelain/metal, or full metal design per the prescription. Esthetic margins may be metal, porcelain/metal, or porcelain per the prescription. Full or partial coverage ceramic restorations with included options such as empress, procera, captek, zirconia, and lithium disilicate (E-Max) Intracoronal ceramic restorations with included options such as empress, procera, E-Max, zirconia. Cast removable partial denture frameworks shall be casted in Vitallium metal, unless otherwise prescribed and may include necessary wrought wire clasp or modification spaces. Resin Retained bridges shall be cast in non-noble metal and include one or two retainer of any design and one pontic of any design. Full or partial dentures may be returned for try-in or taken to finish as per prescription. Denture teeth shall be provided when requested by the Dentist. All dentures shall be processed using Lucitone 199 or equivalent materials unless otherwise prescribed and include patient identification processed in denture. All dental prosthesis supplied to the VA Dental Service Departments must comply with all regulations and standards of the Food and Drug Administration (FDA) https://www.fda.gov American National Standards Institute (ANSI) http://www.ansi.org/ and American Dental Association https://www.ada.org . Dental prostheses or appliances assembled or created under the contract will not be manufactured outside of the United States. The contract laboratory shall provide descriptions of metal alloy components and percentages used in the fabrication of each of the requested dental prostheses. The contractor(s) shall adhere to the Occupational Safety and Health Administration (OSHA) https://www.osha.gov standards and disinfect all dental impressions and prostheses. Repairs and Remakes. Repairs of new original restorations/prostheses will be at no charge as long as there has been no change from the original prescriptions (Rx) or preparation and if restorations do not fit original models. There will also be no charge if dentist changes the shade, but corrections can be made without remaking the restoration or stripping porcelain. The following are examples of no charge repairs: Add contacts Polish or re-size Shade correction (to comply with original prescription) Add porcelain Re-contour Add to pontic area No shipping charges will be charged if the contractor(s) did not meet the conditions of the prescription, or if the restorations/prostheses do not fit the original models. Warranties. The following warranties shall be provided by the contractor(s) as identified below: Remakes and Repairs: All remakes and repairs that fall within the warranty period will be remade at no charge to the Government as long as the material has not changed. The original restorations/prostheses must be returned to the contractors lab for remake cases. Conditional Warranties All warranties are conditional. Any damage due to trauma or normal wear and tear is excluded from warranty. Additional restorative procedures, including but not limited to endodontic treatment, addition of a partial denture and loss and/or failure of teeth may void warranty. All warranties will be remedied by remake of original class of restorations/prostheses at no charge to the Government. Each VA facility dentist shall at their sole discretion determine the quality of fabrication that is acceptable. The VA dental staff shall inspect each of the prostheses including fit, finish, esthetics, patient comfort and satisfaction. Products returned for subsequent adjustments and/or reworks shall be delivered no later than seven (7) calendar days after the re-order is placed at no additional cost to the Government.
