THIS REQUEST FOR INFORMATION (RFI) IS ISSUED SOLELY FOR INFORMATION AND PLANNING PURPOSES ONLY AND DOES NOT CONSTITUTE A SOLICITATION. THE SUBMISSION OF PRICING, CAPABILITIES FOR PLANNING PURPOSES, AND OTHER MARKET INFORMATION IS HIGHLY ENCOURAGED AND ALLOWED UNDER THIS RFI IN ACCORDANCE WITH (IAW) FAR 15.201(e). Responders are solely responsible for all expenses associated with responding to this RFI. SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a sources sought/RFI only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 334516. Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 5 Network Contracting Office, is seeking sources that can provide a Lab Analyzer (Cepheid GeneXpert System Brand Name or Equal) for the Louis A. Johnson VAMC, Clarksburg, WV. The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications. Statement of Need Lab Analyzer (Cepheid GeneXpert System Brand Name or Equal) Background The Louis A. Johnson VAMC has a need to purchase and install one new lab analyzer brand name Cepheid GeneXpert GXXVI-16-D system or equal.   Product Requirements Cepheid GeneXpert GXXVI-16-D (or equal) Dimensions: width: 22.75"" height: 25.80"" depth: 13.25"" Required Tests The GeneXpert will be able to perform the following tests: Xpert Xpress CoV-2 plus Xpert Xpress CoV-2/Flu/RSV plus Xpert Xpress SARS-CoV-2/Flu/RSV Xpert Xpress SARS-CoV-2 Xpert Xpress Strep A Xpert Xpress Flu/RSV Xpert Xpress Flu Xpress MTB/RIF Xpert EV Xpert CR-ABL Ultra Xpert BCR-ABL Ultra p190 (RUO) Xpert FII & FV Xpert TV Xpert CT/NG Xpert GBS Xpert GBS LB Xpert MRSA NxG Xpert MRSA/SA SSTI Xpert MRSA/SA Blood Culture Xpert SA Nasal Complete Xpert C. difficile/Epi Xpert C. difficile Xpert Carba-R Xpert Norovirus Xpert vanA for VRE Training Requirements On-Site User and Biomed Training Clinical-User Training - The vendor providing the system will be responsible for training all clinical users on their system and use. The vendor prior to and at the time of installation shall provide staff education at the Louis A. Johnson VAMC. On-Site or Go-Live training shall be a minimum of 8 hours. Four training manuals are to be provided. A clinical education professional shall administer the training. Education curriculum must include but is not limited to the following: A full system session, to go over the entire instrument, PCR refresher, and maintenance A more general system session that just includes the instrument and running (no maintenance or PCR refresher) Assay review Maintenance specific tasks Installation Services Installation and Implementation The vendor shall provide a description of the installation services offered specifically addressing each of the following requirements: The vendor shall provide professional project management services to organize the training, installation, and go-live activities at the Louis A. Johnson VAMC. The vendor shall identify a single project manager to oversee the Louis A. Johnson VAMC GeneXpert installation and include an organization chart identifying all installation team members. The vendor shall be responsible for testing the system at the Louis A. Johnson VAMC to ensure that the system is functioning as defined by the SOW. The vendor shall be responsible for managing all waste created by the product installation, including clean-up and disposal. The vendor shall define all site preparation requirements of the VA in writing as part of the submittal package. This shall include high voltage electrical requirements, network requirements, and any other required utilities not explicitly specified. The vendor shall comply and install the system per the established VA network requirements, assisting with the ERA/MOU/and other required processes. One year warranty to cover online tech support, repair costs/labor, replacement parts, shipping costs, and software upgrades. Service Requirements Manuals The vendor shall provide the following documentation for the proposed system: Three copies of the Operator Manuals. Three copies of complete technical Service Manuals including troubleshooting guides, necessary diagnostic software and equipment, schematic diagrams, and parts lists. Service The vendor shall address how they will meet the following service requirements: Service during the guarantee period shall be provided within 1 hour of notification. A routine service request will be issued upon any failure which degrades system performance but does not prevent continuation of patient care. Emergency service by qualified personnel must be provided within 1 hour via telephone communication and within 24 hours via onsite service.  An emergency service request will be issued upon any failure which prevents systems operation and disrupts continued patient care. Prior to and during the guarantee period, all services shall be performed at no charge to the Government. Post Warranty Maintenance For maintenance service after the warranty period, the vendor shall provide a service agreement proposal addressing at minimum the following items: PM and CM services. Guaranteed period of availability of replacement parts Delivery time for replacement parts upon receipt of order Normal service hours for field service Qualified service personal should be available to respond onsite to issue within 24 hours from notification.
