- NOTICE OF INTENT THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE BLANKET PURCHASE AGREEMENT (BPA) AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. Reference#: HT941022N0006. The Defense Health Agency Contracting Activity (DHACA) Western Markets - Contracting Division (WM-CD) on behalf of Naval Hospital Japan and Okinawa, Ophthalmology Departments, intends to award a firm fixed price BPA on a sole source basis to Alcon Vision, LLC., 6201 South Fwy, Forth Worth, TX 76134-2001. The period of performance for the anticipated BPA shall be a one year base period and four one year option periods. The anticipated award will be made under the statutory authority permitting other than Full and Open Competition, FAR 13.501 (a), Sole source acquisitions. The North American Industry Classification System (NAICS) code for this supply procurement is 339113 Surgical Appliance and Supplies Manufacturing; the business size standard is 750 (# of employees). The objective of this sole source award is to provide various Medical Treatment Facilities at US Naval Hospital Japan Ophthalmology Departments with Intraocular Lens (IOL) implants for their cataract surgeries. It has been determined that the IOL�s from Alcon Vision, LLC are proprietary to Alcon, the only IOLs that can meet the required specifications of the Government. The minimum requirements for Intraocular Lens Implants are the following: Specific modelds that are specified by intraocular lens master physiology analyzer Length (0t): 13.0mm Optic (0b): 6.0mm Must be UV absorbing Location of implantation: Posterior chamber Shape: Bioconvex Optic Attachment devices: Planar Haptics Must be acrylic foldable Must be sterile Must be compatible with Alcon Infinity Machine The Contractor shall deliver the products to the several MTFs and Ophthalmology clinics will submit partial orders when required for patient surgery. Capability statements shall not exceed 6 (8.5 x 11 inch) pages using a font size no smaller than 10-point.� All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order.
