- SOURCES SOUGHT NOTICE Page 2 of 2 This Request for Information is for market research purposes only to determine the availability of potential businesses with capabilities to provide the service described below. This is not a formal solicitation, and no contract award will result from this Request for Information. All responses may be used to determine the appropriate acquisition strategy for a potential future acquisition. Potential offerors are invited to provide feedback via e-mail no later than August 2, 2022, 3:00 PM EST to Cynthia Gregg at Cynthia.Gregg@va.gov . The Department of Veterans Affairs, Network Contracting Office 08 Tampa, is requesting information on the availability of potential sources and customary marketplace terms and conditions for prescription eyeglasses and secure website for Veterans to select and submit their frame selections directly to the offeror for fabrication. Background Information The Veterans Integrated Service Network (VISN) 8 Health Care System has an ongoing program to provide an estimate of 153,000 prescription eyeglasses per year for eligible military veteran, and the operation is growing. The information received in response to this RFI may be used to help formulate the government's overall contracting strategy for the potential contract. NAICS Code 339115, Ophthalmic Goods Manufacturing, with a size standard of 1000 employees, has been identified as the NAICS code most likely to apply to this procurement. The government is practically interested in website services and the Associated Price/Cost Schedule. The government intends to roll out the website access over the course of the contract to the seven (7) facilities and the associated clinics at the established quantities reflected in the Price/Cost Schedule. The government is requesting responses not only address a firm's interest and ability to complete the website requirement - but is also seeking comments on the planned rollout and pricing structure of the requirement Potential contractors are asked to provide the following information: 1. Company's name and point of contact information (including phone number, e-mail address, and SAM s unique identifier number) 2. Your level of interest in submitting an offer in response to a solicitation for this requirement 3. Business size (small or other than small) and applicable socioeconomic classifications if small (i.e. veteran owned, service-disabled veteran owned, woman owned, 8(a), etc.). 4. Brief Capabilities Statement of your company, including: 5. Information outlining the assortment of lenses you can provide 6. The country/countries of origin of the eye glass frames you currently provide 7. The country/countries of origin of the lenses you currently provide 8. What are your company's order handling capabilities to process daily orders (or more frequent)? 9. What is your company's typical lead time for order delivery? 10. What quality control standards does your company employ to ensure only high-quality products are delivered? 11. Could you please provide product literature that describes the assortment of products you sell (catalogs, etc.)? 12. What is your company's ability to provide web based secure ordering portal that would allow VA PSAS and opticians to track the status of each eyeglass order (fabrication and shipping and delivery confirmation) and to exchange/manage the billing and payment information. The portal shall have/show a log of all work transmitted via fax, email and telephone. Offeror shall send orders to their fabrication lab via secure mail only in an emergency affecting fax and email capabilities or when a frame must accompany the order. 13. What is your company's ability to provide a Veteran website that meets VA 508, FedRamp, Federal Information Security Management Act (FISMA), privacy, and security requirements for Veterans to select and submit their frame selection directly to the offeror for fabrication. This type of website is already available in the private sector. Website frame selection shall be phased in as determined by VA and the VA has sole discretion regarding when or if the website option will be expanded to other sites or terminated (Expected implementation for the website service expected to begin in option year two (2)). Procedures for start-up of Veterans website ASSESSMENT, AUTHORIZATION, AND CONTINUOUS MONITORING The information system solution selected by the Contractor shall comply with the Federal Information Security Management Act (FISMA). The Contractor shall comply with FedRAMP requirements as mandated by Federal laws and policies, including making available any documentation, physical access, and logical access needed to support this requirement. Following guidance from the Federal CIO, VA will utilize existing JAB ATO or agency ATO issued by another agency as a starting point for FedRAMP requirements. If neither of those exist, VA will sponsor FedRAMP ATO. VA will be using the FedRAMP baselines as a starting point, since they are specifically tailored for cloud services. The Contractor shall, where applicable, assist with the VA ATO Process to help achieve agency authorization of the cloud service or migrated application. The Contractor shall complete a FedRAMP System Security Plan (SSP) and supporting documentation within 45 days after contract award. (If Data Security Categorization is High Impact, this will be due 85 days after contract award.) The Contractor shall complete a Third-Party Assessment Organization (3PAO) Security Assessment Plan (SAP) within 90 days after contract award. (If Data Security Categorization is High Impact, this will be due after 130 days after contract award.). The Contractor shall complete a 3PAO Security Assessment Report (SAR) within 140 days after contract award. (If Data Security Categorization is High Impact, this will be due 180 days after contract award.) The Contractor shall afford VA access to the Contractor s and Cloud Service Provider s (CSP) facilities, installations, technical capabilities, operations, documentation, records, and databases. If new or unanticipated threats or hazards are discovered by either VA or the Contractor, or if existing safeguards have ceased to function, the discoverer shall immediately bring the situation to the attention of the other party in accordance with the security addendum B. The Contractor shall not release any data without the consent of VA in writing. All requests for release must be submitted in writing to the Contracting Officer s Representative (COR)/Contracting Officer (CO). In order for live VA data to be used in this system, a FedRAMP Authorization and Agency ATO will be required. Deliverables: FedRAMP System Security Plan (SSP) and supporting documentation VA Implementation Diagram: This is a VA specific architecture diagram demonstrating the proposed implementation of this system at VA (VAID) 3PAO Security Assessment Plan (SAP) 3PAO Security Assessment Report (SAR) Additional information regarding customary marketplace terms and conditions: 1. Return/exchange policies 2. Payment terms 3. Shipping terms and lead times 4. Economic ordering quantities/volume discounts 5. Any other relevant information that may help improve service levels and taxpayer value The Capabilities Statement for this source sought is not expected to be a Proposal or for Bid, nor does it restrict the Government to an ultimate acquisition approach, but rather short statement regarding the company's ability to provide the service above. Submission of capabilities statement will assist our office in tailoring the requirement to be consistent with industry standards. This RFI is for information and planning purposes only and is neither to be construed as a commitment by the Government, nor will the Government pay for information solicited. Respondents will not be notified of the results of the evaluation. All interested contractors should notify this office via email by the response date/time listed in this notice.
