- Statement of Work Adaptive Sports Trailer Equipment General Information Title of Project: Adaptive Sports Custom Trailer/ Recreation Therapy Scope: Purchase of custom trailers for the transportation of kayaks, one item (equipment information below) is for kayak transport and storage of related items. Items requested to be utilized by Central Texas Veterans Health Care System (CTVHCS) events. As in the VA hospital/clinic and Community Outreach treatment, competitions and skill building to which it is the veterans that is using the equipment. This equipment will need to be customized when built so that the trailer can safely secure the items needed to prevent damage to the trailer and equipment that is being transported. We do not currently have an enclosed trailer. For the safety of the equipment from theft, weather, damage and wear and tear an enclosed trailer is the best opportunity. We have a kayak program but are limited to where we can use the kayaks if we do not have a way to transport them. Along with the kayaks there are additional equipment that needs to be included such as but not limited to: safety gear, kayak seats, fishing poles, tackle, lures, chairs, paddles, seats, hydration coolers, personal floatation devices (PFD s) and kayak carts. The trailers will decrease the need for a bus from the motor pool just for equipment. It also allows for equipment to be secured during travel and necessary stops. Salient Information: Enclosed Cargo Trailer - bumper pull power assist electric - bulldog hitch 2 5/16 ball 7 way plug 8ft. wide x 16ft. long x 7.5 ft. interior height Flat front semi screwless sides 2 side wall vents Axle Size: 5200 Axle Type: Spring Electric Brake all four Through Wall Outlet Spare tire mounted with locking capabilities and spare tire cover Steal Wheel Rims, power wrench for lug nuts Rear Barn Door Opening Style with door holders One Side Door Open 32 x 75 (passenger side front area) with step indention Finished Interior Front Upper and Lower Cabinets with secured doors Finished Cabinet Countertop With metal backsplash that is magnetic ShadowBox on Passenger Side Locking Water Tight Door Opening 7 x5 with 2 metal hanging poles of 9 ft. each E Track tie down system welded to frame. 3 on each side and one in the center floor at least 12 ft in length each starting from the back. Marine Grade Plywood or Extruded Aluminum since this is a high water saturation area 8-12 inch storage bars in ceiling of trailer for paddle storage (paddles are 220-240 cm). Needs front stop block. LED strip lights in front, middle and back areas of the trailer interior 4 sets of keys for any trailer locks Delivery Location: CTVHCS Temple Campus: Warehouse, Bldg. 206, 1901 Veterans Memorial Drive, Temple Texas 76504, during the normal duty hours: Monday-Friday, excluding federal holidays, 8:00 am-4:30 pm. General Requirements Vendor will provide all qualified equipment necessary to complete requirements. Equipment maybe on back order and is acceptable. If no longer available, please contact POC for this request (Susan Wilson) so alternative options can be made. NARA Records Management Language Clause Citations to pertinent laws, codes and regulations such as 44 U.S.C Chapter 21, 29, 31, and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. Vendor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Vendor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. Vendor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Vendor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Vendor must deliver sufficient technical documentation with all deliverables to permit the agency to utilize requested equipment. Vendor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. Specific Mandatory Tasks and Associated Deliverables Description of Tasks and Associated Deliverables: The vendor shall provide the specific deliverables described below within the performance period stated in Section 2 a. of this SOW. Training: N/A Manuals: User manuals will be provided with the equipment being requested through the vendor at no additional cost to the government. Location/Inspection: Equipment will be inspected by CTVHCS BioMed staff after delivery, prior to installation. Vendor Requirements: Equipment items must not be shipped before purchase. CTVHCS cannot store uninstalled equipment/items for a significant length of time. Special Requirements: N/A Changes to Statement of Work: Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the vendor through the actions of parties other than the CO shall be borne by the vendor. Government Responsibilities: CTVHCS will provide: Answers to vendor questions during the purchasing process. (CO and COTR.)
