- THIS IS NOT A SOLICITATION ANNOUNCEMENT. THIS IS A REQUEST FOR INFORMATION ONLY. This Request for Information (RFI) is intended for information and planning purposes only at this time; and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs. Because this is a Request for Information announcement, no evaluation letters and/or results will be issued to the respondents. No solicitation exists. Therefore, do not request a copy of a solicitation. STATEMENT OF NEED DESCRIPTION: Brand Name or Equal to Marco Optometry Suite for two Community Based outpatient Clinics (CBOC) Delivery and Installation Locations West Norriton VA Clinic 2495 General Armistead Avenue West Norriton, PA 19403 Delaware County VA Clinic 4883 West Chester Pike Marville Center Newtown Square, PA 19073-2213 The Optometry Department, located within the Coatesville VAMC, Coatesville, PA proposes to purchase a full room of examination equipment and auxiliary testing technology to provide complete eye exam services for Optometry patients in the Delaware County and West Norriton CBOCs in Pennsylvania. This equipment is necessary for the Optometrists to provide eye care at these two locations. Description Quantity 1 Marco M4 Ultra Slit Lamp 2 2 Marco AT-8 Applanation tonometer for M2 and M4 Ultra 2 3 Tonometer Mount for AT-8 2 4 Marco Encore Automatic Chair 2 5 Marco Deluxe stand, Warm Grey include refractor arm 2 6 Marco RT-700 Black Illumination Refractor 2 SPECIFICATIONS: Memory Positioning Switch Auto return button returns the chair to the upright position. Power base and top. Chair can be raised, lowered, or reclined Membrane switches and rotation locks control all operations of the chair from a sitting or standing position. Safety Switch The vendor will install these items so they are fully operational PERFORMANCE, DELIVERY, INSPECTION AND ACCEPTANCE: 3.1 Backorders: If there is a backorder on any of the requested items, the contractor must contact the VA no later than 24 hours. The contractor will offer the VA substitute of equal or higher quality. If acceptable to the VA, the substitute may be made part of the contract with a bilateral modification. 3.2 Warranty: The contractor warrants/guarantees all items against defective material and/or workmanship for a minimum of one year from the date of receipt. The contractor agrees to furnish without additional cost to the government, all labor and materials necessary to correct defects that were detected during the guarantee period. 3.3 The Contractor shall immediately notify all affected VA s and contracting activities in the event of a product recall, removal, required adjustments, or required modification that is suggested or mandated by the contractor, distributor, manufacturer, or any regulatory or official agency. The information that shall be included in this notification include: A complete item description, identification. Administrative identification data including contract number, order numbers, and order date. Reasons for recall, removal, adjustment, or modification. 3.3.4 Instructions for appropriate corrective action. 4. SPECIFIC TASKS: Contractor shall: Provide for proper disposal of packaging materials. 4.2 At a minimum, the following waste categories shall be diverted from landfills: cardboard; paper and packaging; plastics (e.g., ABS, PVC); clean dimensional wood and palette wood. 4.3 Provide on-site instructions and supervision of separation, handling, salvaging, recycling, reuse, and return methods to be used by all parties during waste generating stages. 4.4 Provide the Coatesville Point of Contact (POC) the names and locations of mixed debris reuse and recycling facilities or sites to be used. 4.5 Provide the (POC) the names and locations of trash disposal landfill facilities or sites. 4.6 Provide the (POC) documentation that these facilities or sites are approved to receive the materials to be disposed. 4.7 Provide the (POC) a monthly summary of debris diversion and disposal, quantifying all materials generated at the work site and disposed of or diverted from disposal through recycling. Specifically, the Contractor shall maintain records to document the quantity of waste generated; the quantity of waste diverted through sale, reuse or recycling; and the quantity of waste disposed by landfill or incineration. 4.9 Prior to beginning work, Contractor shall check -in with Primary Care Business Manager. 4.10 Work shall be completed during the normal business week of Monday through Friday, 8:00 am to 4:30 pm, excluding Federal holidays. Contractor shall provide COR with a proposed schedule. 4.11 Performance Monitoring. Contractor shall keep Coatesville POC informed of installations completed each day. POC will routinely inspect completed installations for quality of mounting and operation of televisions. POC will also randomly inspect work in progress. 4.12 Security Requirements. All contractor representatives who will perform this work shall acquire the appropriate VAMC identification badges. No other special security requirements are anticipated. 7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). None is anticipated under this contract. 4.13 Identification of Non-Disclosure Requirements. Contractor will be working in an area where patient care takes place with potential for incidental exposure to patient information. Contractor shall not disclose any patient information. 4.14 Packaging, Packing and Shipping Instructions. See above regarding storage of materials. No other special requirements. Inspection and Acceptance Criteria. Contractor shall keep POC informed of installations completed each day. COR will routinely inspect completed installations for quality of mounting and operation of televisions. COR will also randomly inspect work in progress. Mounting installations are expected to be performed by the agreed upon method. 4.15 Risk Control. Contractor shall provide all applicable training and protective equipment to protect Contractor s employees from any hazards, including blood borne pathogens, which may be encountered in the performance of this contract. 4.16 Place of Performance. All work is to be performed at the West Norriton CBOC and Delaware County CBOCs. 4.17 Period of Performance. Contractor shall complete installations within 30 days of notification to begin work. 5. EDUCATION/ TRAINING: 5.1 The contractor shall provide any and all technical training required in order for VA staff to fully utilize the equipment. The contractor will provide installation and face to face training on the equipment at the Delaware County and West Norriton CBOCs. Request for Information Instructions The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications mentioned above. Responses to this RFI should include company name, address, point of contact, phone number, and point of contact e-mail, DUNS Number, Cage Code, size of business pursuant to North American Industrial Classification System (NAICS) 339112 (size standard of 1000 employees). Please answer the following questions: (1) Please indicate the size status and representations of your business, such as but not limited to: Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, Woman Owned Small Business (WOSB), Large Business, etc.)? (2) Is your company considered small under the NAICS code identified under this RFI? (3) Are you the manufacturer, distributor, or an equivalent solution to the items being referenced above? If not, Can you provide additional information shown below. This is to confirm compliance with the non-manufacturer rule IAW 13 CFR 121.406(b) Nonmanufacturers. Does your company exceed 500 employees Primarily engaged in the retail or wholesale trade and normally sells the type of item being supplied; Take ownership or possession of the item(s) with its personnel, equipment or facilities in a manner consistent with industry practice; Obtained an individual or class waiver? (4) If you are a large business, do you have any designated distributors? If so, please provide their company name, telephone, point of Contact and size status (if available). (5) If you re a small business and you are an authorized distributor/reseller for the items identified above or an equivalent solution, do you alter; assemble; modify; the items requested in any way? If you do, state how and what is altered; assembled; modified? (6) If you intend to subcontract any work on this contract, what portion of the total cost will be self-performed/will be performed by your organization? Please provide estimated detailed percentage breakdowns related to sub contracted work and completion of job. (7) Does your company have an FSS contract with GSA or the NAC or are you a contract holder with any other federal contract? If so, please provide the contract number. (8) If you are an FSS GSA/NAC contract holder or other federal contract holder, are the items/solution you are providing information for available on your schedule/contract? (9) General pricing of your products/solution is encouraged. Pricing will be used for the purpose of market research only. It will not be used to evaluate for any type of award. (10) Please submit your capabilities in regard to the salient characteristics detailed above and any information pertaining to equal to items to establish capabilities for planning purposes? (11) Please review salient characteristics/statement of work (if applicable) and provide feedback or suggestions. If none, please reply as N/A. (12) Please provide your DUNS number. This RFI will be conducted in accordance with Federal Acquisition Regulation (FAR) Part 13. Telephone responses will not be accepted. This notice will help the VA in determining available potential sources only. All firms responding to this Request for Information are advised that their response is not a request for proposal, therefore will not be considered for a contract award. If a solicitation is issued, information will be posted on the beta.sam web site for all qualified interested parties at a later date and interested parties must respond to the solicitation to be considered for award. This notice does not commit the government to contract for any supplies or services. The government will not pay for any information or administrative cost incurred in response to this Request for Information.
