- Special Notice: Request for Information Only OFF-SITE DATA CAPTURE SERVICES The Department of Veterans Affairs (VA), VISN 08, Bruce W. Carter VA Medical Center (VAMC), Miami VA Healthcare System (MVAHS) is looking for certified, Service-Disabled Veteran-Owned Small Business (SDVOSB), Veteran-Owned Small Business (VOSB), and/or Small Business (SB) sources for Off-site Creative Design/Data Capture Services. The Miami VA Healthcare System (MVAHS) anticipates a need for Off-site Creative Design/Data Capture Services of a 16-page survey with responses, as well as to provide a digital file compatible with the Qualtrics study data analytics software used by the VAMC. It is anticipated that the 16-page survey will be mailed to 5,000 veterans which equates to 80,000 scanned pages. The VA conducts studies of many different chronic conditions and assessment of needs for our veteran population. Data Capture Services are required for completion of a national survey for veterans in support of VAMC s Elizabeth Dole Center of Excellence in Veteran and Caregiver (EDCoEiVC) research study. The VAMC is seeking an experienced vendor to provide quality direct marketing capable of securely managing 5,000 paper surveys for verifiable data-driven print, mail, and data capture services (reporting and analysis) that is compatible with the existing VA approved FISMA capable platform with Qualtrics study data analytics software to capture data using a secure solution for all mail and data management requirements (i.e. SOC2 Type 2 Audit + HITRUST Opinion; HIPAA compliant, ISO 27002 International Standards). The contractor must be able to include de-identified personalized barcode on each page of the mailed packet containing a personalized informational cover letter explaining the purpose of the research and the survey, two linked surveys (Veteran and Caregiver), and business reply mail envelopes. The service must be able to capture the data from the returned paper surveys and import into Qualtrics software. It must also maintain and track any missing surveys and send timed follow-up reminders to increase response rate. This is not a request for competitive quotes; however, any firm that believes it can meet these requirements, please provide written notification to the Contracting Officer within ten (10) calendar days from the date of the publication of this notice. SDVOSB or VOSB must be certified and verified in the U.S. Department of Veteran Affairs, Office of Small & Disadvantaged Business Utilization, Vendor Information Pages website located at https://www.vip.vetbiz.gov/. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government. Any vendor interested in contracting with the U.S. Federal Government must be registered in the Systems for Award Management (SAM) website at https://sam.gov/content/home. Any firm that believes it can meet the specifics of the requirement, shall provide the following in their response to this notice: - Capabilities Statement: Capability document should include general information and technical background describing in detail, contractor s capabilities, and any relevant experience that will demonstrate contractor s ability to successfully perform the requirement; specifically, with Qualtrics study data analytics software. - SAM Registered Unique Entity Identification (UEID) number; - Socio-economic status: (SDVOSB/VOSB/SB) Responses are due by 8/09/2022, 2:00 PM, Eastern Standard Time (EST).
