- COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS CONTRACTING OFFICE ADDRESS: Department of Veterans Affairs, Regional Procurement Office Central, Network Contracting Office (NCO) 10, 6150 Oak Tree Blvd., Suite 300, Independence, OH 44131. DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2022-06, effective May 26, 2022. This procurement is a 100% Service-Disabled Veteran Owned Business set-aside. This requirement will be awarded on all-or-none basis. The Government will issue a Firm-Fixed Price, brand name only purchase order resulting from this solicitation. The associated North American Industrial Classification System (NAICS) code for this procurement is 337124, Patio furniture Manufacturing with a large business size standard of 1000 employees. Dayton VA Medical Center 4100 W. Third St. Dayton Ohio 45428 Furniture, CARPENTRY SHOP STORAGE, Dayton. Background The Veterans Health Administration (VHA) has a requirement for healthcare furniture for the CARPENTRY SHOP STORAGE, located at Dayton VA Medical Center. This requirement includes healthcare furniture and professional installation, to include receive, delivery, and installation. Healthcare furnishings include STORAGE SYSTEMS The contractor shall provide all labor, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity. 2.0 PERFORMANCE REQUIREMENTS The contractor shall provide and be responsible for the technical assistance, development, and generation for final specifications. Furniture delivery and installation shall be required in 2 PHASES (see attached plan) Duty hours will be Monday Friday 8:00am 4:00pm. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on Sunday, it is observed on Monday. The contractor shall: Coordinate and provide project management of all products and services to manage, design, order, ship, deliver and install new furniture from the manufacturer until final acceptance by VA Interior Designer. a. Contractor shall be responsible for receiving product into their bonded or insured warehouse, or a warehouse under their lease with the same bonding or insurance, checking to ensure all product has been delivered and is undamaged. If product is damaged or parts missing, it is the responsibility of the contractor to contact the manufacturer. Contractor shall be responsible for coordinating delivery and installation with Interior Designers at the Dayton VA. Contractor shall delivery furniture to the VA Medical Center and be responsible for delivering each piece to the correct office/room, uncrating, assembling and installing per drawings provided to the contractor by the dealer of record with whom the order was placed. Contractor shall take care in movement of all items within the building to ensure no damage is done to walls, floors, ceilings or any other interior finish or fixture. Contractor is to notify VA Designers of any damage or issues immediately. Depending upon construction schedules and possible delays, contractor or dealer must be able to store product in their warehouse for up to 90 days before installation is scheduled with no additional charge to the VA. b. Any items that are received damaged or incorrectly it shall be the responsibility of the dealer to notify the manufacturer and make arrangements for replacement or repair and communicate that with the Interior Designers at the Dayton VA. c. Once contractor is notified and a date for installation to begin has been determined, the contractor will have 10 days to complete installation. Every piece of furniture will have to be installed, in place and ready to be used in the 10 days period. This time period is not negotiable as there are multiple domino moves and other installs that must take place in order to avoid a delay in the construction contract. d. Contractor shall be responsible for uncrating all product, removing crating or debris associated with the delivery and installation from VA property. VA will not provide dumpsters or trash service for debris associated with this job. Contractor shall be responsible for removal of all trash and debris from VA property. Contractor shall leave area clean and neat and is responsible for general cleaning of all product installed for removal of dust and other packing materials. 3.0 SUBMISSION OF QUESTIONS Questions: All questions must be received no later than [date determined by acquisitions]. Bill of Materials with subtotals and tagged per TYPICAL OR ROOM#/ floorplan department and room etc. b. PDFs of detailed and dimensioned: Plan, 2-D and 3-D drawings of each workstation (and/or) products with multiple parts list to represent exactly what is being included in task order and to determine accuracy. c. Cut sheets with product detail for each line item. d. Labor/Services appropriate to the scope of work. 6.0 PRODUCT General All finished surfaces shall be free of scratches, marks, dents, or blemishes, and withstand staining, and exhibit to flaking, cracking, or loss or adhesion. Furnishings shall have smooth finishes with no hazardous projections, sharp corners, or detail, which can be hazardous and cause personal injury or damage to clothing. The contractor may be requested to attend a minimum of three (3) in-person meetings at the Dayton VA Medical Center to review final award with VA Interior Designer and End Users, and to make necessary revisions. b. During first meeting the contractor shall provide the Interior Designer with the Auto-Cad drawings to [1/4 1/8 ] scale drawings showing layouts of awarded product. c. The contractor shall allow for three (3) revisions per line item included in design services including updating Auto-Cad Drawings and PDFs as request by VA Interior Designer. d. The contractor shall be responsible for taking and applying accurate field measurements to ordered product for verification of correct sizing. Auto-Cad drawings may not be accurate finished dimensions and may not be used in place of field measurements. The Vendor shall be liable for any incorrect field measurements leading to incorrect product order. e. The contractor shall be responsible for providing all sample materials for awarded product (2-Day Delivery) as requested by VA Interior Designer. f. The product must not be placed into production without clearance from the VA COR. g. The contractor shall provide final, clean [1/4 1/8 ] scaled drawings of product layout for sign-off prior to scheduling manufacturing. h. The contractor shall track manufacturing schedule and notify VA Interior Designer with updates of estimated completion date by email. i. The contractor shall attend a post-installation meeting to assess, address, and document any punch-list items and shall submit to the VA Interior Designer a remediation plan within 5 business days. j. The contractor shall provide 90 days free storage. The Government maintains the option to modify the normal work week, days and hours, as necessary for the accomplishment of VHA mission. Work Required by Others All work required by other trades shall be identified under this section by the Bidder A.
