- Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, and FAR subpart 13.5, Simplified Procedures for Certain Commercial Products and Commercial Services , as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular: 2022-06, 05/26/2022. This solicitation is set-aside for Small Businesses (SB). The associated North American Industrial Classification System (NAICS) code for this procurement is 334510, with a business size standard of 1,250. The FSC/PSC is 5963. The Network Contracting Office 16 (NCO 16) is seeking a source to furnish all materials, parts, equipment, and tools for the below listed Brand Name Reichert Avia Tono-Pen Tonometer s, for the Michael E DeBakey VA Medical Center, 2002 Holcombe Blvd, Houston, Texas 77030. All equipment shall be brand new. The contract price shall be firm-fixed, all-inclusive. See attached Statement of Work for specifications. All interested companies shall provide quotations for the following Reichert Manufactured items: Supplies/Services LINE ITEM DESCRIPTION QTY UNIT OF MEASURE UNIT PRICE TOTAL PRICE 0001 Stk #230650, REICHERT AVIA TONO-PEN TONOMETER SYSTEM W/CASE. The following are the decision factors: Lowest Price Technically Acceptable (LPTA). PLEASE NOTE: Responses shall contain estimated lead-times, as well as your best terms and conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition.""
