This announcement constitutes the only solicitation for quote to be requested with no other form of written solicitation to be issued. This solicitation RFQ-36C250-22-Q-0936 is issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-95/ 01-13-2017. The North American Industry Classification System (NAICS) number is 339113. This solicitation is set-aside for Service-Disabled Veteran-Owned Small Business. This is a solicitation only pertaining to brand name only item (s). Supply necessary materials and/or parts for CHAMPION: 85/86 Series Recliners per the attached requirements. Materials and parts will be delivered to; VA Indianapolis, 1481 W. 10Th St., Indianapolis, IN 46202, Nursing Department, FOB destination. Fully assembled, installed and replacement of chairs. Both sides need ability to put the chair into Trendelenburg. STATEMENT OF WORK (SOW) Dialysis Recliners Module 85 and 86 for Nursing Department New dialysis treatment recliners to provide dialysis patient care 6 days per week. These are to be consistent with existing recliners in the Dialysis center. Champion Module 85 and 86 treatment chair, heat & massage, premium upholstery, premium tables on both sides. Recliners must be: 1-Durable, able to withstand daily repeated use, movement, adjustment without breaking down or tears in chair material. Easily moved from place to place and easily locked to be stationary. 2-All surfaces conforming to infection prevention guidelines (No exposed wood, nonabsorbent material) 3-Cleanable with a bleach solution without breaking down the chair material over time. Easily cleanable from blood spills and other liquids (break away arms, easy to clean entire chair) 4-Conforming to emergency patient needs. Specifically able to be easily and quickly transferred to Trendelenburg or flat position to alleviate blood pressure drops or to provide CPR. 5- A height that is conducive to providing patient care to a patient, so the nurse/tech does not have to bend over for prolonged periods of time thus increasing chances for back or other staff injuries. 6- Comfortable for patients to remain seated for 4+ hours at a time. Able to supports average patient weight and 4 recliners able to support up to 500lb. They also need to be easily switched to a rescue/Trendelenburg position in case of complications with blood pressure or need for CPR. Delivery and installation of 32 Champion Module 85 and 86 Recliners with removal of old chairs from the installation area to loading zone for the VA Logistics material handlers, coordination with nursing and logistics is required. Offerors must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The following additional provisions apply: 52.212-3, Offeror Representations and Certifications-Commercial Items, a completed copy of this provision shall be submitted with the offer unless SAM registered which must be noted in the quote. 52.212-5, Contract Terms and Conditions-Commercial Items. The following additional clauses apply: Purchase order (SF 1449) will be awarded to the winning bid with submission of Invoice through Tungsten upon delivery of ALL items listed on spreadsheet. From the VAAR the following apply: 852.232-72 Electronic submission of payment requests. All invoices must be submitted to Tungsten for payment processing. The PO number must appear on all invoices, packing slips, shipping labels, and/or correspondences to the contracting officer.
