- Page 3 of 3 NOTICE OF PROPOSED CONTRACT ACTION The Department of Veterans Affairs, New York/New Jersey VA Health Care Network (VISN 2 NCO), James J. Peters VA Medical Center, Bronx New York, intends to award a sole source contract under the authority of 41 U.S.C 253(c) (1) - FAR 6.302-1 Only one responsible source and no other supplies or services will satisfy agency requirements, to ThinkForm Design Architects, LLC, 15 Seminary Avenue, Hopewell, NJ 08525-2016, to perform Post Award Construction Period Services for the existing Renovate 6C for Mental Health Project 526-13-102 (including Site visits and post-construction service duties). The place of performance is the James J. Peters DVA Medical Center, Bronx, NY. This action will result in a Firm-Fixed Price contract with a period of performance of Twenty (20) months. This notice of intent is not a request for a quotation; interested parties may express their interest by providing a capabilities statement no later than Tuesday, November 10, 2020 at 4PM EST. The capabilities statement must provide clear and unambiguous evidence to substantiate the capability of the party to provide the required services. A determination not to compete this proposed contract upon responses to this notice is solely within the discretion of the Government. Verbal responses are not acceptable and will not be considered. If after Tuesday, November 10, 2020 at 4PM EST, no viable responses have been received in response to this announcement, Department of Veterans Affairs shall negotiate solely with ThinkForm Design Architects, LLC. This notice of intent to award a sole source contract is not a request for competitive quotes. There will be no solicitation available for competitive quotes. Phone calls will not be accepted. A determination by the Government whether to compete this proposed requirement based upon responses to this notice is solely within the discretion of the government. Information received will normally be considered solely for the purpose of determining whether to conduct a competition. SCOPE STATEMENT OF WORK Title: Renovate 6C for Mental Health AE Continuation of Services 526-13-102A Background: The following A/E services are in continuation of project 526-13-102. This project renovates Area 6C; Bldg. 100 to accommodate inpatient Mental Health according to specifications and drawings. The design includes a collaborative design that is aesthetic, energy efficient, cost effective and comprehensive for the modern work and Mental Health care environment. Scope: The A/E Construction Period Services (CPS) will include but not limited to the following: Twenty (20) site visits. RFI responses and corrections/updates to existing plans. Review of Phasing and coordination plans. commissioning verification and support, Punchlist inspections and re-inspections. and Update of As-Built drawings all based upon the original scope of work relating to project 526-13-102 Specific Task: Referencing the scope above, The A/E will continue to make on-site inspections of all areas within the active construction project as per the existing design awarded to ThinkForm Architects. PERIOD OF PERFORMANCE: The period of performance for construction will be Twenty (20) months. EXACT PLACE OF PERFORMANCE Work will be completed on the 6th floor of Main Building 100 currently under construction and being renovated at the James J. Peters DVA, 130 W. Kingsbridge Road, Bronx, New York 10468 END OF NOTICE
