In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this notice. This notice is being issued in an attempt to determine the level of interest among all socioeconomic groups. Responses will assist VA planners in determining an appropriate acquisition and set-aside strategy which best fits VA requirements. The James E. Van Zandt VA Medical Center (JEVZ VAMC) located at 2907 Pleasant Valley Blvd, Altoona, PA 16602-4305 is performing market research to determine if there is a sufficient number of qualified: (1) Service-Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); (3) Small or emerging small business firms (SB or other); or (4) large business (LB) firms to set aside a single award contract for lodging services. The James E. Van Zandt VAMC is seeking a qualified contractor to provide post-warranty, full service agreement on the imaging injectors located at Altoona VAMC as well as its five (5) Community Based Outpatient Centers (CBOCs) with a regularly scheduled PM at intervals dictated by the original equipment manufacturer (OEM) and completed within the month due per said frequency interval and with CM anytime needed. Attached is the DRAFT Statement of Work for a description of this requirement. The anticipated period of performance is to start October 2022 for a total of five years. The audiology equipment is located at the main building of the JEVZ VAMC Facility as well as its five (5) Community Based Outpatient Centers (CBOCs) as stated in section 3 of the SOW. All interested firms who can meet the requirement should respond, in writing, and provide the following information: 1. Indicate if you can meet or exceed the government s requirements to provide Preventive and Corrective Maintenance for audiology equipment to the JEVZ VAMC Facility as well as its five (5) Community Based Outpatient Centers (CBOCs). Please keep to a minimum of two pages or less. 2. Eligible contractors must demonstrate how they meet the qualifications required in section 4 of the SOW. A summary of your skills, abilities, qualifications, relevant experience, and relevant past contracts/past projects (as it directly relates to this requirement) must be included in your response. 3. Eligible contractors must provide documentation of official OEM endorsed certification. 3. Eligible contractors must be able to perform the services and execute the required deliverables as annotated in sections 5 of the SOW. 4. The NAICS Code 811219 (Repairing and maintaining (without retailing) electronic and precision equipment) is applicable to this acquisition, with a Business Size Standard of $22 Million. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract. DRAFT Statement of Work (SOW) for Audiology Equipment Preventative Maintenance (PM) and Corrective Maintenance (CM) for Altoona VAMC. 1. BACKGROUND: The James E. Van Zandt VA Medical Center (hereafter referred to as JEVZ VAMC , Altoona VAMC or the Government ) has a requirement for a post-warranty, full service agreement on the imaging injectors located at Altoona VAMC with the period of performance beginning October 2022 with a regularly scheduled PM at intervals dictated by the original equipment manufacturer (OEM) and completed within the month due per said frequency interval and with CM anytime needed. 2. OBJECTIVE: The Government intends to award a single, firm fixed price contract for the services described in this work statement. The period of performance under this contract is one year from date of award, with four additional one-year option periods, to be exercised at the Government s discretion. 3. PERFORMANCE DELIVERABLES: PM fully according with the living documentation of manufacturer s specifications, up to date with all revisions that may be issued. Electronic documents reflecting the full set of parameters checked per manufacturer s specifications, results, and recommendations must be delivered within a week of PM. If any further follow-up is deemed necessary, field service personnel or other kind(s) of representatives performing PM, CM, or software updates are to notify Biomedical Engineering in person and/or phone call the same day and in writing (email) what is needed, if it will be fully covered under contract, and how soon it can be scheduled, if not able to be fixed on-site immediately, provided the guidelines of the contract (Altoona VAMC normal tour of duty and hours of operation are 8:00 AM - 4:30 PM EST Monday through Friday). Vendor is to provide a plan, any necessary quote, and schedule within one-week for repairs; services not covered under contract will not be conducted until such a time that a government-officiated purchase order is (if at all) issued for repairs, parts, or any additional work. Specialists will have documented certification and authorization that is up to date from the manufacturer to support software regarding all latest instructions for use (IFU). The vendor will have documented certification and authorization that is up to date from the OEM(s) to perform PM on the relevant equipment listed above. Vendor will perform all steps required for regular maintenance at the mandated intervals, per manufacturer s specifications, using the proper and fully calibrated equipment that OEM specifies. Vendor will ensure that VA Biomedical Engineering personnel are informed of proceedings and results of support or any other contract-authorized work before considering case completed for closure. Vendor will receive VA-issue PM labels that will be filled out completely and legibly and affixed to a visible surface of the device and components checked, next to the equipment entry (EE) barcode label that identifies tracking on the devices, if the vendor does not have their own comprehensive labeling (company, initials, date completed, and next date due) that would yield the VA s labels redundant. Vendor will also ensure that VA Biomedical Engineering personnel are informed of proceedings and results of PM and any other contract-authorized work before leaving premises. End of Document
